Culture Care Theory: A Major Contribution
to Advance Transcultural Nursing
Knowledge and Practices

This article is focused on the major features of the Culture
Care Diversity and Universality theory as a central contrib-
uting theory to advance transcultural nursing knowledge and
to use the findings in teaching, research, practice, and consul-
tation. It remains one of the oldest, most holistic, and most
comprehensive theories to generate knowledge of diverse and
similar cultures worldwide. The theory has been a powerful
means to discover largely unknown knowledge in nursing and
the health fields. It provides a new mode to assure culturally
competent, safe, and congruent transcultural nursing care.
The purpose, goal, assumptive premises, ethnonursing
research method, criteria, and some findings are highlighted.
This article is focused on the 2001 Pittsburgh Precon-
ference theme “Major Contributions of Book Authors to
Transcultural Nursing Knowledge and Practices.” As the
founder of the discipline and author of 28 books and 220 pub-
lished articles, I hold that my Culture Care Diversity and Uni-
versality theory has made a significant contribution to estab-
lish and advance transcultural nursing research knowledge
and practice since the mid-1950s. This article is a brief synop-
sis of culture care theory with its unique features and major
contributions to support transcultural nursing as a discipline
and practice field.
In establishing this new discipline, different lines of think-
ing and practice were essential. It necessitated futuristic
vision, risk taking, commitment, patience, and leadership to
challenge many traditional nursing ideas and practices.
Unquestionably new knowledge and practices were essential
for nurses to function in a rapidly changing multicultural
world. Substantive theory-based research knowledge was
greatly needed with a global and comparative focus to care for
people of diverse cultures. Culturally based care knowledge
was the major missing area in nursing in the mid-20th century
and still is in some places in the world. I coined the construct
of culturally congruent care, which is the central goal of the
theory.
In my first two books,Nursing and Anthropology(1970)
and Transcultural Nursing: Concepts, Theories, Research,
and Practice(1978), the nature, rationale, need, and theoreti-
cal base were given to establish transcultural nursing. Nurses
needed in-depth knowledge of cultures with an anthropologi-
cal view and in-depth, culturally based care phenomena. I
held that care was the essence of nursing and had meaning
within cultural contexts. Care was not fully known and valued
in nursing, and so it was a challenge to get nurses interested in
the Culture Care theory in the 1950s and 1960s as the medical
mind-body treatments and symptoms held nurses’ interests

Culture Care Theory: A Major Contribution
to Advance Transcultural Nursing
Knowledge and Practices

This article is focused on the major features of the Culture
Care Diversity and Universality theory as a central contrib-
uting theory to advance transcultural nursing knowledge and
to use the findings in teaching, research, practice, and consul-
tation. It remains one of the oldest, most holistic, and most
comprehensive theories to generate knowledge of diverse and
similar cultures worldwide. The theory has been a powerful
means to discover largely unknown knowledge in nursing and
the health fields. It provides a new mode to assure culturally
competent, safe, and congruent transcultural nursing care.
The purpose, goal, assumptive premises, ethnonursing
research method, criteria, and some findings are highlighted.
This article is focused on the 2001 Pittsburgh Precon-
ference theme “Major Contributions of Book Authors to
Transcultural Nursing Knowledge and Practices.” As the
founder of the discipline and author of 28 books and 220 pub-
lished articles, I hold that my Culture Care Diversity and Uni-
versality theory has made a significant contribution to estab-
lish and advance transcultural nursing research knowledge
and practice since the mid-1950s. This article is a brief synop-
sis of culture care theory with its unique features and major
contributions to support transcultural nursing as a discipline
and practice field.
In establishing this new discipline, different lines of think-
ing and practice were essential. It necessitated futuristic
vision, risk taking, commitment, patience, and leadership to
challenge many traditional nursing ideas and practices.
Unquestionably new knowledge and practices were essential
for nurses to function in a rapidly changing multicultural
world. Substantive theory-based research knowledge was
greatly needed with a global and comparative focus to care for
people of diverse cultures. Culturally based care knowledge
was the major missing area in nursing in the mid-20th century
and still is in some places in the world. I coined the construct
of culturally congruent care, which is the central goal of the
theory.
In my first two books,Nursing and Anthropology(1970)
and Transcultural Nursing: Concepts, Theories, Research,
and Practice(1978), the nature, rationale, need, and theoreti-
cal base were given to establish transcultural nursing. Nurses
needed in-depth knowledge of cultures with an anthropologi-
cal view and in-depth, culturally based care phenomena. I
held that care was the essence of nursing and had meaning
within cultural contexts. Care was not fully known and valued
in nursing, and so it was a challenge to get nurses interested in
the Culture Care theory in the 1950s and 1960s as the medical
mind-body treatments and symptoms held nurses’ interests

When I was little, I dreamed of becoming a flight attendant, a lawyer, an architect, and a doctor. I never saw myself become a nurse someday. I must admit that my only motivation for pursuing a nursing degree in college was to get to the United States and make good money. But I didn't think that I would someday love the profession I never even imagined doing. It is for this reason that I believe that nursing is a calling. Being a nurse has its bittersweet moments and surely takes a lot of compassion, patience, empathy, and strength. While it's true that the long hour shifts can be physically exhausting, it's witnessing the most devastating situations in life that make this profession very challenging. On the contrary, our ability to heal, save lives, and make a difference in our patients' lives and their families, truly is very rewarding and incomparable to nothing. Being a nurse for almost five years has opened my eyes and changed my views about life and all other things. I first started working on a Telemetry/Neuro floor for about a year and a half before I decided to venture out and ended up working in an extremely busy ER in downtown Jacksonville, FL. I worked there for two years, and though it was a highly stressful environment, I enjoyed almost every minute of it. The ER has the kind of culture that is fast-paced, task-oriented, informative, and team driven. Having passed my certification in emergency nursing (CEN) recently, I can say that my knowledge base, assessment, and critical thinking skills, which I often use to guide me in my clinical decision making, have significantly improved since I became an ER nurse. It has molded me into a strong, hard-working, and competent nurse that I am today.

Background
Liquids contain molecules that have different kinetic energies (due to different velocities). Some of the
faster liquid molecules have enough kinetic energy to vaporize. At the same time, some of the slower
vapor molecules condense into liquid. In an open container, the rate of vaporization will be greater than
the rate of condensation—hence, the liquid will eventually evaporate. In a sealed flask, however, there
will be a point in which equilibrium is reached between the rate of vaporization and the rate of
condensation. To the eye, it seems that the liquid doesn’t change at equilibrium. But at the microscopic
level a vapor molecule enters the liquid phase for every liquid molecule that enters the gas phase.
The total pressure in the sealed flask is due to the vaporized liquid plus air molecules present in the flask:

In this experiment, you will investigate the relationship between
the vapor pressure of a liquid and its temperature. Pressure and
temperature data will be collected using a gas pressure sensor and
a temperature probe (Figure 1). Vapor pressures will be
determined by subtracting atmospheric pressure from the total
pressure.
The flask will be placed in water baths of different temperatures to
determine the effect of temperature on vapor pressure. You will
measure the vapor pressure of methanol and ethanol and
determine the enthalpy (heat) of vaporization for each liquid.
Objectives
In this experiment, you will
Investigate the relationship between the vapor pressure of a liquid and its temperature.
Compare the vapor pressure of two different liquids at the same temperature.
Use pressure‐temperature data and the Clausius‐Clapeyron equation to determine the heat of
vaporization for each liquid.
Caution!
The alcohols used in this experiment are flammable and poisonous. Avoid inhaling their vapors. Avoid
contacting them with your skin or clothing. Be sure there are no open flames in the lab during this
experiment. Notify your teacher immediately if an accident occurs.
Procedure
1. Wear goggles! You will work in pairs for this lab, but you may share water baths with your table.
2. Prepare four water baths: 20 to 25°C (use room temperature water), 30 to 35°C, 40 to 45°C, and 50 to
55°C. You should also have some hot water on a hot plate on reserve.
3. Obtain a temperature probe and gas pressure sensor. The sensor comes with a
rubber‐stopper assembly (Figure 2). The stopper has three holes, one of which
is closed. Make sure your tubing and valve are not inserted in the closed hole.
Insert the rubber‐stopper assembly into a 125 mL Erlenmeyer flask.
Important: Twist the stopper into the neck of the flask to ensure a tight
fit.

If you do some research online, you’ll find the terrible press and publicity that the company received regarding how they handled the situation.
For this project, you will imagine that you are a part of the crisis communications team at Chrysler. Your team has been charged with all communications related to the layoffs.
You must develop a coordinated company response to the crisis in twoparts:
1. First, you must review, revise and improve the actual letter that Nardelli sent to Chrysler’s employees (following the guidelines in the textbook for delivering bad news). You can find the letter by clicking on the link below:

The purpose of this paper is to identify, describe, and apply the concepts that underlie my personal nursing philosophy. I will give a brief overview of my nursing background, identify and describe the four metaparadigms of nursing, provide two other practice specific concepts that apply to my practice, and include a numbered list of five propositions that apply to those concepts.
Nursing Autobiography
When I was little, I dreamed of becoming a flight attendant, a lawyer, an architect, and a doctor. I never saw myself become a nurse someday. I must admit that my only motivation for pursuing a nursing degree in college was to get to the United States and make good money. But I didn't think that I would someday love the profession I never even imagined doing. It is for this reason that I believe that nursing is a calling. Being a nurse has its bittersweet moments and surely takes a lot of compassion, patience, empathy, and strength. While it's true that the long hour shifts can be physically exhausting, it's witnessing the most devastating situations in life that make this profession very challenging. On the contrary, our ability to heal, save lives, and make a difference in our patients' lives and their families, truly is very rewarding and incomparable to nothing. Being a nurse for almost five years has opened my eyes and changed my views about life and all other things. I first started working on a Telemetry/Neuro floor for about a year and a half before I decided to venture out and ended up working in an extremely busy ER in downtown Jacksonville, FL. I worked there for two years, and though it was a highly stressful environment, I enjoyed almost every minute of it. The ER has the kind of culture that is fast-paced, task-oriented, informative, and team driven. Having passed my certification in emergency nursing (CEN) recently, I can say that my knowledge base, assessment, and critical thinking skills, which I often use to guide me in my clinical decision making, have significantly improved since I became an ER nurse. It has molded me into a strong, hard-working, and competent nurse that I am today.

Background
Liquids contain molecules that have different kinetic energies (due to different velocities). Some of the
faster liquid molecules have enough kinetic energy to vaporize. At the same time, some of the slower
vapor molecules condense into liquid. In an open container, the rate of vaporization will be greater than
the rate of condensation—hence, the liquid will eventually evaporate. In a sealed flask, however, there
will be a point in which equilibrium is reached between the rate of vaporization and the rate of
condensation. To the eye, it seems that the liquid doesn’t change at equilibrium. But at the microscopic
level a vapor molecule enters the liquid phase for every liquid molecule that enters the gas phase.
The total pressure in the sealed flask is due to the vaporized liquid plus air molecules present in the flask:
Ptotal = Pvapor + Pair (1)
In this experiment, you will investigate the relationship between
the vapor pressure of a liquid and its temperature. Pressure and
temperature data will be collected using a gas pressure sensor and
a temperature probe (Figure 1). Vapor pressures will be
determined by subtracting atmospheric pressure from the total
pressure.
The flask will be placed in water baths of different temperatures to
determine the effect of temperature on vapor pressure. You will
measure the vapor pressure of methanol and ethanol and
determine the enthalpy (heat) of vaporization for each liquid.
Objectives
In this experiment, you will
Investigate the relationship between the vapor pressure of a liquid and its temperature.
Compare the vapor pressure of two different liquids at the same temperature.
Use pressure‐temperature data and the Clausius‐Clapeyron equation to determine the heat of
vaporization for each liquid.
Caution!
The alcohols used in this experiment are flammable and poisonous. Avoid inhaling their vapors. Avoid
contacting them with your skin or clothing. Be sure there are no open flames in the lab during this
experiment. Notify your teacher immediately if an accident occurs.
Procedure
1. Wear goggles! You will work in pairs for this lab, but you may share water baths with your table.
2. Prepare four water baths: 20 to 25°C (use room temperature water), 30 to 35°C, 40 to 45°C, and 50 to
55°C. You should also have some hot water on a hot plate on reserve.
3. Obtain a temperature probe and gas pressure sensor. The sensor comes with a
rubber‐stopper assembly (Figure 2). The stopper has three holes, one of which
is closed. Make sure your tubing and valve are not inserted in the closed hole.
Insert the rubber‐stopper assembly into a 125 mL Erlenmeyer flask.
Important: Twist the stopper into the neck of the flask to ensure a tight
fit.

University of the Southeast 25 was (and still is) one of the largest universities in the United States. It had been growing rapidly; that growth was spurred, in part, by information technology. The university embraced lecture capture technologies that allowed lectures to be streamed to students in a classroom, in dorm rooms, on the grass near the main campus central fountain, and at a variety of other places of the students’ choosing whenever they chose to watch. This made it possible to have sections of classes with over 1,000 students without having to build physical classrooms with enough seats to accommodate each person enrolled. It also made it possible to offer classes that were streamed to students at remote campuses. Each student was charged a technology fee (i.e., $5.16 for undergraduates and $13.85 for graduates per credit hour each semester), which was administered by the Information Technologies and Resources (IT&R) Office to help fund the costs of providing IT to students and faculty.IT&R was responsible for providing computer services, technologies, and telecommunications across the campus (Computer Services and Technology), helping faculty with their instructional delivery and multimedia support (Office of Instructional Resources), helping faculty develop and deliver Web‐based and lecture capture courses (Center for Distributed Learning), and the library. The IT&R Office developed IT‐related policies with very little input from the faculty and was responsible for deciding and implementing decisions concerning IT architecture and infrastructure. IT&R worked with the university president and other top administrators in making IT investment decisions. IT&R staff also worked with the various colleges, administrative offices, and an advisory board in making decisions about applications that needed to be developed. However, faculty were not consulted at all when the lecture capture system was selected. As was often the case at large universities, many decision rights on a wide range of issues had been allocated to the colleges.
The College of Business Administration had its own server and Technology Support Department (TSD). A recent survey of faculty and staff in the college indicated a high level of satisfaction with the TSD but far less satisfaction with the services provided by the university‐level IT&R. Some college respondents indicated their displeasure about IT&R’ s support of the technology for the lecture capture courses, help desk, and classroom technologies. The problems with the technology support for lecture capture software were particularly troublesome. The software would not authenticate students who had paid to enroll in some lecture capture courses, making it impossible for them to download the lectures even though they were registered in the course. Further, some university‐affiliated housing did not have adequate network bandwidth to allow students .

- 1. Based on this case study, consider to answer the questions that are at the end. Mrs. Smith was a 73-year-old widow who lived alone with no significant social support. She had been suffering from emphysema for several years and had had frequent hospitalizations for respiratory problems. On the last hospital admission, her pneumonia quickly progressed to organ failure. Death appeared to be imminent, and she went in and out of consciousness, alone in her hospital room. The medical-surgical nursing staff and the nurse manager focused on making Mrs. Smith’s end-of-life period as comfortable as possible. Upon consultation with the vice president for nursing, the nurse manager and the unit staff nurses decided against moving Mrs. Smith to the palliative care unit, although considered more economical, because of the need to protect and nurture her because she was already experiencing signs and symptoms of the dying process. Nurses were prompted by an article they read on human caring as the “language of nursing practice” (Turkel, Ray, & Kornblatt, 2012) in their weekly caring practice meetings. The nurse manager reorganized patient assignments. She felt that the newly assigned clinical nurse leader who was working between both the medical and surgical units could provide direct nurse caring and coordination at the point of care (Sherman, 2012). Over the next few hours, the clinical nurse leader and a staff member who had volunteered her assistance provided personal care for Mrs. Smith. The clinical nurse leader asked the nurse manager whether there was a possibility that Mrs. Smith had any close friends who could “be there” for her in her final moments. One friend was discovered and came to say goodbye to Mrs. Smith. With help from her team, the clinical nurse leader turned, bathed, and suctioned Mrs. Smith.
- 2. She spoke quietly, prayed, and sang hymns softly in Mrs. Smith’s room, creating a peaceful environment that expressed compassion and a deep sense of caring for her. The nurse manager and nursing unit staff were calmed and their “hearts awakened” by the personal caring that the clinical nurse leader and the volunteer nurse provided. Mrs. Smith died with caring persons at her bedside, and all members of the unit staff felt comforted that she had not died alone. Davidson, Ray, and Turkel (2011) note that caring is complex, and caring science includes the art of practice, “an aesthetic which illuminates the beauty of the dynamic nurse-patient relationship, that makes possible authentic spiritual-ethical choices for transformation—healing, health, well-being, and a peaceful death” (p. xxiv). As the clinical nurse leader and the nursing staff in this situation engaged in caring practice that focused on the well-being of the patient, they simultaneously created a caring-healing environment that contributed to the well-being of the whole—the emotional atmosphere of the unit, the ability of the clinical nurse leader and staff nurses to practice caringly and competently, and the quality of care the staff were able to provide to other patients. The bureaucratic nature of the hospital included leadership and management systems that conferred power, authority, and control to the nurse manager, the clinical nurse leader, and the nursing staff in partnership with the vice president for nursing. The actions of the nursing administration, clinical nurse leader, and staff reflected values and beliefs, attitudes, and behaviors about the nursing care they would provide, how they would use technology, and how they would deal with human relationships. The ethical and spiritual choice making of the whole staff and the way they communicated their values both reflected and created a caring community in the workplace culture of the hospital unit.
