This study observed changes in perioperative safety attitudes among surgical staff at a major US hospital following an aviation-based crew resource management (CRM) training intervention. The training focused on communication, briefings, decision-making, and other teamwork skills shown to improve safety culture in aviation. Surveys before and after the intervention measured effects on error reporting, teamwork, and organizational safety culture. Results suggested the training correlated with positive post-intervention changes in perioperative safety attitudes.
Threat and Error Management in Aviation
In 1994, the University of Texas Human Research Project and Delta Airline developed the Line Operations Safety Audit (LOSA) program. With time, the LOSA program evolved into what is now known as Threat and Error Management (TEM).
The TEM framework is an applied concept which emerged from the observations and surveys of actual flight operations. It considers the various issues that a flight crew may encounter as a result of internal and external factors.
This model explores the contributing factors of the threat to aviation safety and, in turn, allows for the unearthing of ways to mitigate them and maintain proper safety margins. Now recognized and adopted across continents, the TEM framework aims to educate flight personnel on managing threats and errors before they degenerate into serious incidents or accidents. In 1994, the University of Texas Human Research Project and Delta Airline developed the Line Operations Safety Audit (LOSA) program. With time, the LOSA program evolved into what is now known as Threat and Error Management (TEM).

zkStudyClub - LatticeFold: A Lattice-based Folding Scheme and its Application...
Folding is a recent technique for building efficient recursive SNARKs. Several elegant folding protocols have been proposed, such as Nova, Supernova, Hypernova, Protostar, and others. However, all of them rely on an additively homomorphic commitment scheme based on discrete log, and are therefore not post-quantum secure. In this work we present LatticeFold, the first lattice-based folding protocol based on the Module SIS problem. This folding protocol naturally leads to an efficient recursive lattice-based SNARK and an efficient PCD scheme. LatticeFold supports folding low-degree relations, such as R1CS, as well as high-degree relations, such as CCS. The key challenge is to construct a secure folding protocol that works with the Ajtai commitment scheme. The difficulty, is ensuring that extracted witnesses are low norm through many rounds of folding. We present a novel technique using the sumcheck protocol to ensure that extracted witnesses are always low norm no matter how many rounds of folding are used. Our evaluation of the final proof system suggests that it is as performant as Hypernova, while providing post-quantum security.

The expansion of AI services necessitates enhanced computational capabilities on edge devices. Temporal Event Neural Networks (TENNs), developed by BrainChip, represent a novel and highly efficient state-space network. TENNs demonstrate exceptional proficiency in handling multi-dimensional streaming data, facilitating advancements in object detection, action recognition, speech enhancement and language model/sequence generation. Through the utilization of polynomial-based continuous convolutions, TENNs streamline models, expedite training processes and significantly diminish memory requirements, achieving notable reductions of up to 50x in parameters and 5,000x in energy consumption compared to prevailing methodologies like transformers.
Integration with BrainChip’s Akida neuromorphic hardware IP further enhances TENNs’ capabilities, enabling the realization of highly capable, portable and passively cooled edge devices. Dandelion Hashtable: beyond billion requests per second on a commodity server
This slide deck presents DLHT, a concurrent in-memory hashtable. Despite efforts to optimize hashtables, that go as far as sacrificing core functionality, state-of-the-art designs still incur multiple memory accesses per request and block request processing in three cases. First, most hashtables block while waiting for data to be retrieved from memory. Second, open-addressing designs, which represent the current state-of-the-art, either cannot free index slots on deletes or must block all requests to do so. Third, index resizes block every request until all objects are copied to the new index. Defying folklore wisdom, DLHT forgoes open-addressing and adopts a fully-featured and memory-aware closed-addressing design based on bounded cache-line-chaining. This design offers lock-free index operations and deletes that free slots instantly, (2) completes most requests with a single memory access, (3) utilizes software prefetching to hide memory latencies, and (4) employs a novel non-blocking and parallel resizing. In a commodity server and a memory-resident workload, DLHT surpasses 1.6B requests per second and provides 3.5x (12x) the throughput of the state-of-the-art closed-addressing (open-addressing) resizable hashtable on Gets (Deletes).

- 2. Cause of the crash investigated by The National Transportation Safety Board (NTSB): The final NTSB report cited the cause of the crash as pilot error, specifically: "the failure of the flight crew to monitor the flight instruments during the final four minutes of flight, and to detect an unexpected descent soon enough to prevent impact with the ground. Preoccupation with a malfunction of the nose landing gear position indicating system distracted the crew's attention from the instruments and allowed the descent to go unnoticed."
- 3. Consequence: In response to the accident, many airlines started Cockpit Resource Management (CRM) training for their pilots. The training was designed to make problem solving in a cockpit much more efficient, thus causing less distraction for the crew.
- 4. What is CRM? Cockpit Resource Management is the sharing of knowledge and best practices to reduce errors and accidents/incidents.
- 5. CRM Initiated by NASA in 1979; NASA discovered crashes due to lack of: 1. Interpersonal communication; 2. Poor decision making; 3. Lack of leadership skills.
- 6. CRM CRM has been used by commercial airlines since 1981; 1990s grew to include pilots, flight attendants, flight dispatch, maintenance (Crew Resource Management); Currently Federal Aviation Administration (FAA) Advanced Qualification Program (AQP) requires CRM to be incorporated into all aspects of training; Required by International Civil Aviation Organization (ICAO) for airlines in 185 countries; Used by high reliability organizations: aviation, military, fire services, oil production, nuclear operations, commercial shipping, medical field, etc.
- 7. Team Resource Management (trm) Has its origin in Cockpit Resource Management; It did not take long for ATM professionals to realize that ATM and Aviation shared common concerns: poor communication, team performance and inadequate behavior do not only occur in aircraft, but in ATM facilities too, and in both situations accidents/incidents can be the tragic result; The development of TRM in the 1990s was a first logical step towards improving teamwork and reducing team related errors.
