The crew of Apollo 13, consisting of James Lovell, Jack Swigert, and Fred Haise, experienced a major problem in their spacecraft while on a mission to the moon in 1970. An oxygen tank exploded, cutting off the supply of breathable air, water, and power. NASA engineers worked tirelessly to solve issues of oxygen, water, carbon dioxide removal, temperature control, and re-entry without the normal spacecraft functions. Through the heroic efforts of the astronauts and ground control, the crew was able to safely return to Earth in what became known as a "successful failure." A post-accident investigation determined the cause was a short circuit in one of the oxygen tank fan circuits that ignited Teflon insulation
