Antidep (Generic Imipramine Hydrochloride Tablets) is used
for the relief of symptoms of depression. Using this
medication to treat depression may improve your mood,
sleep, appetite, and energy level and may help restore your
interest in daily living.
This medicine may be useful as temporary adjunctive
therapy in reducing enuresis in children aged 6 years and
older, after possible organic causes have been excluded by
appropriate tests.

Chemical Structure

The active ingredient contained in Antidep tablets is
Imipramine Hydrochloride.
Each Antidep tablet contains 25 mg or 75 mg of Imipramine
Hydrochloride.

Uses of Antidep 25 mg and 75 mg Tablets:
Antidep (Generic Imipramine Hydrochloride Tablets) is a tricyclic
antidepressant used to treat symptoms of depression. These
medicines are thought to work by increasing the activity of a chemical
called serotonin in the brain. Endogenous depression is more likely to
be alleviated than other depressive states. One to three weeks of
treatment may be needed before optimal therapeutic effects are
evident.
Using this medication to treat depression may improve your mood,
sleep, appetite, and energy level and may help restore your interest in
daily living.

Uses of Antidep 25 mg and 75 mg Tablets:
Imipramine is sometimes used to treat bed-wetting in children ages 6
and older after possible organic causes have been excluded by
appropriate tests.
In patients having daytime symptoms of frequency and urgency,
examination should include voiding cystourethrography and
cystoscopy, as necessary.
The effectiveness of treatment may decrease with continued drug
administration.

Recommended Adult Dosage
Depression: For most patients (except in elderly patients,
adolescents or children), the starting dose of Antidep Tablets
(Generic Imipramine Tablets) is 25 mg 3 times daily, initially,
increased up to 150 mg daily, if necessary. Dosage in excess of 200
mg daily is not recommended for outpatients. More severe,
hospitalized cases may require up to 300 mg daily.
In Elderly Patients and Adolescents: 30 to 40 mg daily, initially,
increased by 10 mg daily to a maximum of 100 mg in the elderly.

Recommended Adult Dosage
Childhood Enuresis:
Initially, an oral dose of 25 mg/day should be tried in children
aged 6 and older. Medication should be given one hour before
bedtime. If a satisfactory response does not occur within one
week, increase the dose to 50 mg nightly in children under 12
years; children over 12 may receive up to 75 mg nightly. A daily
dose greater than 75 mg does not enhance efficacy and tends
to increase side effects.

Contraindications
➔
Antidep tablets are contraindicated in patients with a known hypersensitivity
to Imipramine Hydrochloride or any component of the tablet. The possibility
of cross-sensitivity to other Dibenzazepine compounds should be kept in mind.
➔
The concomitant use of monoamine oxidase inhibiting compounds is
contraindicated. Hyperpyretic crises or severe convulsive seizures may occur
in patients receiving such combinations. The potentiation of adverse effects
can be serious, or even fatal. When it is desired to substitute imipramine
hydrochloride in patients receiving a monoamine oxidase inhibitor, as long an
interval should elapse as the clinical situation will allow, with a minimum of 14
days. Initial dosage should be low and increases should be gradual and
cautiously prescribed.

Contraindications

Antidep tablets are contraindicated in during the acute
recovery period after a myocardial infarction.
➔
This medicine is contraindicated in patients with Prostatic
hyperplasia, pyloric stenosis and other stenoses of the
gastrointestinal and genitourinary systems, liver diseases and
confirmed or suspected pregnancy and breast-feeding.
➔
Antidep is contraindicated for the treatment of depression in
patients 18 years of age and under.

Storage Instructions
Antidep Tablets (Generic Imipramine Hydrochloride
Tablets) have to be stored at controlled room
temperature i.e. from 20°C to 25°C (68°F to 77°F) with
excursions permitted between 15° to 30°C (59° to
86°F).
Keep this as well as all other medicines away from
children and pets.

Warnings
Patients with major depressive disorder (MDD), both adult and
pediatric, may experience worsening of their depression and/or the
emergence of suicidal ideation and behavior (suicidality) or unusual
changes in behavior, whether or not they are taking antidepressant
medications, and this risk may persist until significant remission occurs.
A major depressive episode may be the initial presentation of bipolar
disorder. It is generally believed (though not established in controlled
trials) that treating such an episode with an antidepressant alone may
increase the likelihood of precipitation of a mixed/manic episode in
patients at risk for bipolar disorder.

Warnings
The pupillary dilation that occurs following use of many antidepressant drugs
including imipramine hydrochloride may trigger an angle-closure attack in a
patient with anatomically narrow angles who does not have a patent
iridectomy.
A dose of 2.5 mg/kg/day of imipramine hydrochloride should not be
exceeded in childhood. ECG changes of unknown significance have been
reported in pediatric patients with doses twice this amount.
Extreme caution should be used when Antidep (Generic Imipramine
Hydrochloride Tablets) is given to patients with cardiovascular disease,
patients with history of urinary retention, hyperthyroid patients or those on
thyroid medication, patients with a history of seizure disorder, patients
receiving Guanethidine, Clonidine, or similar agents and patients receiving
Methylphenidate Hydrochloride.

Warnings
An ECG recording should be taken prior to the initiation of larger-than-usual
doses of imipramine hydrochloride and at appropriate intervals thereafter
until steady state is achieved.
Patients with any evidence of cardiovascular disease require cardiac
surveillance at all dosage levels of the drug. Imipramine may enhance the
CNS depressant effects of alcohol. Therefore, it should be borne in mind that
the dangers inherent in a suicide attempt or accidental overdosage with the
medicine may be increased for the patient who uses excessive amounts of
alcohol.
Since Imipramine Hydrochloride may impair the mental and/or physical
abilities required for the performance of potentially hazardous tasks, such as
operating an automobile or machinery, the patient should be cautioned
accordingly.

Possible Side Effects
Infection And Infestations: Very rare: dental caries
Neurological: Very common: tremors. Common: dizziness, headache, paresthesia
(numbness, tingling sensation, symptoms suggestive of peripheral neuropathy) ,
delirium. Rare: convulsions. Very rare: tinnitus, incoordination, ataxia,
electroencephalogram abnormal, extra pyramidal disorder, myoclonus, speech
disorders, asthenia.
Behavioral: Common: somnolence, fatigue, insomnia, confusional states with
hallucinations, anxiety, agitation, restlessness, nightmares, hypomania, mania,
decrease in memory, feeling of unreality, disorientation. Rare: psychotic disorders.
Very rare: aggression.
Anticholinergic: Very common: dry mouth and rarely associated sublingual adenitis,
blurred vision, disturbances of visual accommodation, lacrimation decreased,
constipation, hyperhidrosis, hotflushes. Common: micturition disorders, dilation of the
urinary tract. Very rare: mydriasis, glaucoma, paralytic ileus, urinary retention.

Cardiovascular: Very common: hypotension, particularly orthostatic hypotension
with associated vertigo, sinus tachycardia, electrocardiogram abnormalities.
Common: arrhythmia,disturbances in cardiac conduction, palpitation, syncope. Very
rare: hypertension, congestive heart failure, myocardial infarction, heart block,
asystole, stroke, vasospams, QT interval prolongation, ventricular arrhythmia,
ventricular tachycardia, ventricular fibrillation, torsades de pointes.
Hematologic: Very rare: agranulocytosis, eosinophilia, leukopenia, purpura and
thrombocytopenia may occur as an idiosyncratic response.
Gastrointestinal: Common: nausea, vomiting, anorexia, abdominal cramps, liver
function test abnormal. Rare: diarrhea. Very rare: bitter taste, stomatitis, epigastric
distress, black tongue, dysphagia, increased salivation, hepatitis with or without
jaundice, abdominal disorders, tongue ulceration.

Respiratory: Very rare: bronchospasm.
Endocrine: Very common: Weight gain. Common: increased or decreased libido,
impotence. Very rare: gynecomastia in the male, hypertrophy breast and
galactorrhea in the female, testicular swelling, elevation or depression of blood
sugar levels, weight loss, inappropriate antidiuretic hormone (SIADH) secretion
syndrome.
Allergic or toxic: Common:dermatitis allergic, rash, urticaria, Very rare: petechiae,
itching, photo sensitization,edema, pyrexia, obstructive jaundice, nasal
congestion,alopecia, cross-sensitivity with desipramine, allergical veolitis
(pneumonia) with or without eosinophilia, systemic anaphylactic / anaphylactoid
reactions including hypotension, skin hyperpigmentation.
General disorders: Very rare: sudden death

Antidep Tablets Overdosage
Overdose symptoms may vary in severity depending upon factors such as the
amount of drug absorbed, the age of the patient, and the interval between
drug ingestion and the start of treatment. Critical manifestations of overdose
include cardiac dysrhythmias, severe hypotension, convulsions, and CNS
depression including coma. Changes in the electrocardiogram, particularly in
QRS axis or width, are clinically significant indicators of tricyclic toxicity.
Other CNS manifestations may include drowsiness, stupor, ataxia,
restlessness, agitation, hyperactive reflexes, muscle rigidity, athetoid and
choreiform movements.
Cardiac abnormalities may include tachycardia and signs of congestive
failure. Respiratory depression, cyanosis, shock, vomiting, hyperpyrexia,
mydriasis, and diaphoresis may also be present.

Treatment of Antidep Tablets Overdosage
ECG should be obtained and cardiac monitoring should be immediately
initiated. Protect the patient’s airway, establish an intravenous line and initiate
gastric decontamination.
A minimum of 6 hours of observation with cardiac monitoring and observation for signs
of CNS or respiratory depression, hypotension, cardiac dysrhythmias and/or conduction
blocks, and seizures is necessary.
If signs of toxicity occur at anytime during this period, extended monitoring is required.
All patients suspected of tricyclic overdose should receive gastrointestinal
decontamination. This should include large volume gastric lavage followed by activated
charcoal. If consciousness is impaired, the airway should be secured prior to lavage.
Emesis is contraindicated.
In patients with CNS depression, early intubation is advised because of the potential for
abrupt deterioration. Seizures should be controlled with benzodiazepines, or if these are
ineffective, other anticonvulsants (e.g., phenobarbital, phenytoin).

Antidep Tablets During Pregnancy
Imipramine Hydrochloride (Antidep tablets) has been classified by the
US FDA as Pregnancy Category C.
Experience with Imipramine in pregnancy is limited. Since there have
been isolated reports of a possible connection between the use of
Imipramine and adverse effects on the fetus, treatment with Imipramine
should be avoided during pregnancy, and only considered if the
benefits expected justify the potential risk for the fetus.
Epidemiological studies have suggested an increased risk of
congenital abnormalities associated with use of antidepressants in
pregnancy and the use of antidepressants in pregnancy may be
associated with an increase in pre-term delivery.

Antidep Tablets During Pregnancy
Babies whose mothers had taken Imipramine Hydrochloride
(Antidep tablets) up until delivery showed symptoms, such as
dyspnoea, lethargy, colic, irritability, hypotension or hypertension,
tremor or spasms, during the first month of life.
To avoid such symptoms, Imipramine (Antidep tablets) should, if
possible, be gradually withdrawn at least 7 weeks before the
calculated date of confinement.
