This document outlines a whanau (family) project at a Maori health provider serving a population of 2900 people that is 90% Maori/Pacific or high needs. The goals are to work with high needs families, extend care into the home, and address barriers to healthcare. Currently, these families frequently present with recurring conditions and have issues like overdue immunizations. The strategy involves a mobile whanau nurse building relationships in homes to provide holistic care, bring in specialists, and identify environmental factors impacting health. A team including GPs, nurses, community workers, and social services will support the nurse. The project aims to improve access to care, healthcare utilization, and health outcomes for these families.
