Treat all electrical wires as being ―live.‖

When ever there is accident, some body injured , we used to ask ourselves that , how it happened
Make one thing clear that, accident are not happened, they are caused. Accident is an unforeseen/unplanned event and there are multiple contributery factors to an accidents, and majority of which are preventiable.
Although some accident are out of your control, there are someone others mistake or that you can not avoid it. Suppose you are sitting here and a plan crashes and hit your compound, and you got injuries. But here today we are talking about the accident which could be occurred at site when you are working, the accidents which may caused by negligence of any of your co-worker, your staff member here in your workplace. For sure they could be avoidable when you have proper safety management and safety oriented minds with you.
If you investigate any accident, there would be many human causes for that accident. Lets suppose, a person fell from his house stairs, there is no evil spirit behind that and or following shadow for him. There will be physical causes which are there to happened that accidents. That could be his own fault or fault of anybody else.
