This document outlines a proposal for an organization to help low-income minority families manage diabetes. The organization would hire bilingual staff with experience in physical therapy to provide education on healthy eating, cooking, grocery shopping, and physical activity programs. Funding would come from grants, donations, and fundraisers. The goals are to help families better manage diabetes, lower healthcare costs for the government, and improve personal well-being through lifestyle changes.

suffering from diabetes, diabetes
would be better managed, it will cut
the expenses spent by the
government in healthcare, and
personal well-being will increase.

3. Inputs= hire employees preferably those experienced in physical

therapy, also employees who have their own transportation and
most will be bilingual (preferably Spanish and English). Grants ,
donations will be accepted. Fundraisers will be held. A space will
be rented. It will be advertised in clinics that have high rates of
diabetics.
Activities=As a fundraiser a 5K run will be held, we will also ask

local facilities to support by allowing to place canisters by their
cashiers to collect donations. The clients will be escorted to their
appointments and with the clients permission be there to translate
or simplify anything the physician states. The client will be taught
how to eat and cook foods properly and even be accompanied to
go grocery shopping. The client will also be apart of our physical
activity program where physical therapists will aid the client. We
will try to have a contract with organic stores where we will send
our clients for a small portion of their profits.

4. Outputs= Minorities in low-income neighborhoods that have diabetes

will have a longer life expectancy. They will be educated on their foods
and lifestyle serving as role models in their community and family. There
will be less stress on the family having to take care of their elder family
members with diabetes because in most minority families mostly
everyone in the household has to work providing less time to care for
some one. Ironically minority elders are the ones that take care of the
children in the family while the parents work, these are also the people
greatly affected by diabetes. With our program we could help families
succeed and help change lifestyles to stop the cycle of diabetes
Outcomes=By lowering the impact of diabetes in minority low income

neighborhoods, this will mean less doctor visits and medications,
lowering the amount of money the government spends on Medicare and
Medicaid. Also with more elder grandparents alive and well their family
can go and get a job not having to worry about their children also
lowering the money the government spends on programs like welfare.
Grandparents also serve as role models encouraging their family to eat
more healthy probably indirectly reducing diabetes.

5. Goal Alignment=Overall this program will benefit

society for generations to come. The reason why I
target minority elders is because they need the
most help having language barriers and having to
change customs that they have had throughout the
years. They are the ones that are ignored due to
financial circumstances and at the same time they
hold the family together. Even though the program
is primarily for people with type II diabetes, will
also help families who need help if their child or
loved one has type I diabetes. This will help the
governments financial spending and it will help
families have better lifestyles probably indirectly
decreasing obesity among minority children.

Increased the number of jobs given to minorities

provides experience for upcoming physical therapists

and those wanting to achieve in social work and the
medical field.
Access to information to those with diabetes and their

families
Past employees are better trained and educated

which will help them excel in jobs after this one

It is also convenient and empowering allowing those

with diabetes succeed and increase their personal
well being.
