How GPs and mental health practitioners should work togetherKris Van den Broeck
In this slideshow, we first present a literature study, showing that guidelines on the treatment of major depressive disorder (MDD) only provide little information about how to organise collaborative care. An additional Pubmed search, however, may be inspirational for who would like to improve collaboration amongst caregivers regarding the care for severely depressed patients. A second (qualitative) study presented in this presentation outlines how collaborative care amongst general practitioners (GPs) and mental health care practitioners is organised today in Belgium and what can be improved according to practitioners in favour of severely depressed patients.
Overgeneral memory (OGM) refers to the tendency to recall categories of events from memory, rather than specific memories, even when it is explicitly instructed to recall specific events. Unlike depressed and traumatized patients, patients diagnosed with borderline personality disorder (BPD) inconsistently show OGM. Non-suicidal self-injury (NSSI) on the other hand, is very common in patients with BPD. Like OGM, NSSI is considered to be an affect-regulation mechanism. This study investigated how these strategies relate to each other. Based on earlier findings (Startup et al., 2001), we hypothesized that NSSI and OGM would be inversely associated. Fifty three BPD patients completed the Structured Clinical Interview for DSM-IV Disorders (Axis II as well as Axis I-mood modules), the Autobiographical Memory Test to assess OGM, and the Self-Injury Questionnaire - Treatment Related (SIQ-TR) to assess NNSI. We found that patients who engage in NSSI do not differ from patients who do not with respect to OGM. However, we found that participants who use more different NSSI methods showed less OGM, but this association disappeared when we controlled for age. We propose a balance-model of affect-regulation as one possible explanation for the relationship between these two affect-regulation behaviours, and discuss the clinical relevance of our findings.
The document summarizes a study on overgeneral memory in patients with borderline personality disorder (BPD). The study examined: (1) whether overgeneral memory (OGM) in BPD is associated with major depressive disorder (MDD) or post-traumatic stress disorder (PTSD) diagnoses or severity levels, and (2) whether the Capture and Rumination, Functional Avoidance, and Impaired Executive Control (CaR-FA-X) model applies to OGM in BPD. The study found that OGM in BPD was associated with age but not MDD, PTSD, BPD severity, trauma exposure, or the CaR-FA-X variables. OGM specificity was higher in B

Welzijnsbehandelingen zijn een onmisbaar
onderdeel, zowel voor chronische als voor
terminale patiënten. Het is inderdaad aan
alle hulpverleners, die met deze patiënten te
maken hebben, om uitnodigend te zijn voor
deze behandelingen en de weerstand wat te
helpen verminderen. Ik denk dat de
medische wereld hier een bocht moet
maken van "behandelen" van een diagnose,
naar zorg voor een cliënt in alle aspecten,
die hem/haar deugd kunnen doen. Vroeger
werd dit (oliën, massage,...) standaard
gedaan bij stervenden op de palliatieve
afdeling: mag/kan dit nog tegenwoordig?
Of is ook hier protocollering doorgedrongen
in plaats van tijd voor zorg? Een groot
probleem voor verplegenden aan huis: je
kan geen voeten masseren als je welgeteld
7 minuten hebt voor een wasbeurt... (DF13);
Essentieel is dat men elkaar respecteert. Iemand
die zegt: ik geloof in bachbloesems, en die
patiënt gelooft daar ook in... Heb daar respect
voor! Wat we vandaag zien: osteopathie, dat is
niet wetenschappelijk, dus daar doen we niet
aan mee. Da’s een foute instelling. Je kan alleen
maar vanuit respect voor elkaar zeggen: als dat
een zorg is, of een meerwaarde heeft aan de
patiënt, why not? […] En als je zegt:
homeopathie, dat werkt niet, en dat kan goed
zijn, want ik twijfel daar ook aan, maar als die
patiënt zegt: die homeopaat die mij dat komt
voorstellen, dat is zo’n toffe vent, daar wil ik het

Het spijtige aan de wet vind ik toch wel dat het horen
en beoordelen van de vraag enkel aan artsen
toegewezen is. Naar mijn bescheiden mening kan een
pastoraal werker, een psycholoog of misschien een
verpleegkundige ook veel betekenen. Ik denk dat de
artsen zich ook gesteund zouden voelen als zij kunnen
terugvallen op een multidisciplinair team. Want het
opnemen (en uitvoeren) van deze vraag is niet te
onderschatten. Het klinkt misschien raar als ik zeg dat
onze dierenarts het er al ontzettend moeilijk mee had
onze uitgeleefde hond te doen inslapen. Ook in ons
woonzorgcentrum merk ik dat de huisartsen tevreden
zijn te kunnen terugvallen op en heel erg steunen op

In 2015, a Dutch study was conducted, investigating what is considered (in)appropriate care at the end of life, its drivers and solutions.
The aim of this project was to replicate this study in Belgium and to install a societal discussion on the topic of end of life care.
