How GPs and mental health practitioners should work togetherKris Van den Broeck
In this slideshow, we first present a literature study, showing that guidelines on the treatment of major depressive disorder (MDD) only provide little information about how to organise collaborative care. An additional Pubmed search, however, may be inspirational for who would like to improve collaboration amongst caregivers regarding the care for severely depressed patients. A second (qualitative) study presented in this presentation outlines how collaborative care amongst general practitioners (GPs) and mental health care practitioners is organised today in Belgium and what can be improved according to practitioners in favour of severely depressed patients.
Overgeneral memory (OGM) refers to the tendency to recall categories of events from memory, rather than specific memories, even when it is explicitly instructed to recall specific events. Unlike depressed and traumatized patients, patients diagnosed with borderline personality disorder (BPD) inconsistently show OGM. Non-suicidal self-injury (NSSI) on the other hand, is very common in patients with BPD. Like OGM, NSSI is considered to be an affect-regulation mechanism. This study investigated how these strategies relate to each other. Based on earlier findings (Startup et al., 2001), we hypothesized that NSSI and OGM would be inversely associated. Fifty three BPD patients completed the Structured Clinical Interview for DSM-IV Disorders (Axis II as well as Axis I-mood modules), the Autobiographical Memory Test to assess OGM, and the Self-Injury Questionnaire - Treatment Related (SIQ-TR) to assess NNSI. We found that patients who engage in NSSI do not differ from patients who do not with respect to OGM. However, we found that participants who use more different NSSI methods showed less OGM, but this association disappeared when we controlled for age. We propose a balance-model of affect-regulation as one possible explanation for the relationship between these two affect-regulation behaviours, and discuss the clinical relevance of our findings.

2) It also found that the more an autobiographical memory cue was relevant or discrepant with the patient's ideal self, the more difficulty they had retrieving specific memories, as seen in depressed patients.
3) Depression severity appeared to play an important role in overgeneral memory in borderline personality disorder patients.
1. There were no associations found between borderline personality disorder (BPD) complaints and overgeneral autobiographical memory.
2. Higher BPD complaints were associated with an observer perspective during recall of highly discrepant autobiographical memories.
3. Effortful control was negatively associated with BPD complaints and mediated the relationship between BPD complaints and observer perspective during recall.

Begonnen in 2012, vanuit het veld, maar naar het voorbeeld van VIP²-AZ
Er is een complexe structuur opgezet, opnieuw naar het voorbeeld van VIP²-AZ.
Er werd een bureau geïnstalleerd, en OGn, bevolkt door u allen, het Forum.
QID = Quality Indicator Dataserver, die de TTP (waarover straks meer) aanstuurt. Belangrijk voor de kwaliteit en veiligheid van de data.
OGn hebben heel hard gewerkt. Gelezen, ontwikkeld, geselecteerd, verder ontwikkeld, gevalideerd.
Dat was niet steeds gemakkelijk: GGZ is een complexe sector, bovendien in volle beweging, en er zijn eigenlijk relatief weinig gegevens die centraal/uniform worden verzameld.
Vandaag staan we hier met enkele indicatoren, klaar voor gebruik.
Achtergrond
De ontslagperiode is een destabiliserende periode. Patiënten moeten met nieuw aangeleerde vaardigheden naar een oude, niet-gewijzigde context. We zien in deze periode een toename in suïcidepogingen, en patiënten zijn dan ook minder medicatie-trouw.
Vermoedelijk kan een snelle ambulante opvolging na ontslag ertoe bijdragen dat deze periode voor patiënten draaglijker is.
Een ervaringsdeskundige is een persoon met een psychische kwetsbaarheid die een GGZ-ervaring heeft (doorgemaakt) met een fundamentele impact op zijn leven.
Goede zorg is ook veilige zorg.
We werken met kwetsbare mensen, van wie veel suïcidale ideaties hebben. Het is belangrijk dat onze voorzieningen daar op voorzien zijn.
Er mag geen verwarring zijn rond bijv. de dosis van het geneesmiddel.
Meer dan louter patiënttevredenheid; in het algemeen geeft deze vragenlijst info over de mate waarin een voorziening de patiënt centraal stelt in het zorgproces – patiëntgerichtheid is immers en belangrijk kenmerk van kwaliteitsvolle zorg.
Doel was om breed toepasbare indicatoren te maken. Dat is gehaald.

En hoe gaat het nu verder?
Weldra uitnodiging – registratieperiode tot 15/11.
Daarna formaliteiten regelen, zijnde engagementsverklaring (data-gebruik – ook uit secundaire bronnen –, levering van peers, …) en accounts (zie later)
Dan metingen organiseren
Hangende indicatoren worden afgewerkt, misschien nieuwe ontwikkeld (ideeën? kandidaten?)
Dit gezegd zijnde, nu inzoomen op enkele belangrijke kwesties.
Doelen
Allemaal goed en wel, maar waarom moet dat dan op deze manier? Ik kan dat ook in mijn eigen organisatie opzetten! Sterker nog, ik doe dat al, en ik heb daar verder niemand bij nodig.
Welnu, er zijn wel wat voordelen aan dit project voor uw organisatie:
1+1 > 2: kennis, know-how, focus (bijv. Co&Co), ruimte voor ontwikkeling / validiteit
Betrouwbaarheid: e.g., peer control
Maar u bent kritisch, en heeft ook wat vragen / bedenkingen:
Vertrouwelijke gegevens doorsturen, en dan laten verwerken door een externe?!
Wat gaat er met die data gebeuren? Wat als de Standaard een lijstje publiceert? En de overheid, kijkt die mee?
Dat zijn terechte opmerkingen.
Het klopt dat we soms vertrouwelijke gegevens zullen gaan verwerken, en dat we bestanden moeten gaan samenvoegen op basis van persoonsgegevens. Daar zijn strenge regels voor, en een privacycommissie, die over de plannen moet oordelen. In de huidige set die we vandaag lanceren, is dat nog niet van toepassing. Voor de overige indicatoren ondernemen we momenteel de nodige stappen. (wie verzamelt? Hoe wordt verzameld? Hoe anonimiseren? …)
Gerelateerd hieraan: er werd een TTP opgericht.
Om tegemoet te komen aan de bezorgdheid van de voorzieningen
Voor de bescherming van de persoonlijke levenssfeer van de burger
Bij technische infrastructuur: ook registratiehulpmiddelen, ontwikkeld in overleg met ontwikkelingsgroepen
Bij statistische verwerking: ook al in validatiefase evt. // + ook back-office ivm auditoren
Een laatste stap in het goed behandelen van de data betreft de zorg voor de transfer en verwerking van de data.

En dan, hoor ik u vragen, wat gaat er dan met die data gebeuren?
Welnu, uiteraard krijgt u op organisatieniveau feedback + benchmark (geen ranking!). Cijfertjes zeggen niet alles: nauwkeurig, lokale analyse is nodig. Dat ligt in de lijn van het opzet van VIP²-GGZ.
Delen mag. Gebruik begrijpbare taal, op niveau van de ontvanger, met duiding, via website, nieuwsbrief, jaarrapporten, …
Dat brengt ons bij de volgende groep van geïnteresseerden: de gebruikers van de zorg.
VIP²-AZ heeft recent een website gelanceerd. Dat behoort tot de mogelijkheden op termijn (!), maar is niet prioritair. De website laat toe om voorzieningen te vergelijken, maar biedt ook ruimte aan voorzieningen om hun resultaten te duiden.
Mogelijk zijn ook academici geïnteresseerd. Dat kan, maar niet zomaar. Het zou leuk zijn als ook zij in de toekomst een plekje rond de tafel krijgen.
En wat doet de overheid hier?
Het gaat over VAZG en ZI. De missie van VAZG stelt…. Daarom kiezen zij ervoor om een belangrijke ondersteunende rol te spelen in VIP².
Wat ZI betreft, deze dienst is geëngageerd om kwaliteitszorg na te streven. Bovendien kennen ze de plaatselijke situatie erg goed, vanuit hun inspecties. Daarom zijn zij goed geplaatst om inhoudelijk aan dit project te participeren.
Gebruiken zij de data van VIP² ook voor hun werkzaamheden?
De data van VIP² zijn uiteraard mogelijk ook interessant voor deze entiteiten in het kader van hun werkzaamheden. Dit is echter nog niet aan de orde, ook niet binnen VIP²-AZ. Zo-ie-zo zal dit niet zonder overleg gebeuren met het Bureau VIP²-GGZ.
