• To train community based health and social care workers about malnutrition
and how to screen for it
• Implement the appropriate care pathways including advice and referrals
• Develop a means to share screening information between teams
• Monitor and improve the nutritional status of those at risk

• Development of an online form, hosted by Dorset CountyCouncil
• Second pilot area: Christchurch (2016)
• Project Manager employed (2016)
• Rolling out the programme to the rest of Dorset (2016 -2018)

5. 1st April to 7th December 2017
• Screened 1341 people
• 160 of which have received multiple screenings
• 34 of these have shown an improvement in their nutritional status
• 17 have worsened
• 109 have stayed the same
• 324 staff trained
6. Future Developments
• Move in to Poole and Bournemouth
• Achieve link to SystmOne
• Continue to support the work of the Health and Social care teams
• Continue to reduce the risk of malnutrition in the vulnerable of Dorset
