2. PERFORMANCE OBJECTIVES AND EVALUATION
List the performance factors/objectives and accomplishments expected during the rating period. Revise and add
performance factors/objectives, as necessary, throughout the rating period. Upon completion of the rating period,

Objective:
Demonstrates proficiency in providing both written and verbal communication that explains the facts and issues of a
case or the trends and positive behaviors in a report. Is able to present things in a clear, concise and objective
manner.
Evaluation:
Demonstrated this through monthly expenditure reports and tracking, monitor reports to both the counties and to the
BSC. Continued clear and concise communications with an emphasis on creating an acceptable level of
understanding. Expanded commuication with county and local leadership through various special projects.

Objective:
Completes individual assignments in a timely manner, willingly shares knowledge and information to assist the team.
Follows the direction and advice of their manager/trainer and addresses issues in a professional manner.
Evaluation:
Assignments were completed within the allotted time 100% of the time. Knowledge and information was acquired and
communicated in a professional manner to both team members and counties served.Assisted assigned counties with
local tracking of expenditures as well as other key statistics in areas outside of contracts.

Objective:
Demonstrates proficiency in preparing written reports/studies on review results with trend analyses and
recommendations for change. Checks for understanding and gives accurate, clear and concise information in terms
that can be understood by the customer.
Evaluation:
Demonstrated proficiency in preparing written reports. Trend analysis exceeded expectations, catching otherwise
unknown coding errors, expenditure report errors, and allocation errors throughout the fiscal year. Customer
understanding of BSC communication was satisfactory, and monthly scheduled County visits were completed.

Objective:
Establishes a liaison relationship between BSC and the designated field office that fosters improvement and
understanding about the contracting and flexible funds allocation process on one end and issues that are unique to
the field offices. Works to improve working relationships and understanding of the process. Provides positive
feedback and/or suggestions for improvement.
Evaluation:
A liaison relationship was improved with both the counties served and the Office of Contracts and Purchasing with
participation in County Bid Reviews, contract process reviews, and allocation tracking. Established as the go to
person for contract and expenditure related questions for the BSC Counties served. There continues to be room for
growth in anticipating changes in the contracting process.

Description:

Comments:
Mr. Johnson independently manages and organizes his workload. He takes on additional responsibilities readily. His
work is consistently high quality and completed timely, even with tight deadlines.
TECHNICAL/PROFESSIONAL KNLDG
Rating: ME - Meets Expectations
Description:
Having achieved a satisfactory level of technical and professional skill or knowledge in position-related areas. A
person possessing this competency keeps up with current developments and trends in areas of expertise.
Comments:
Mr. Johnson has excellent knowledge in the areas under his responsibility. he also works to gain knowledge in the
various programs in the Department to further his professional self - and to further the mission of the Department.
CONTRIBUTING TO TEAM SUCCESS

Description:
Actively participating as a member of a team to move the team toward the completion of goals.
Comments:
Mr. Johnson works to ensure the success of all of the counties in BSC 3. He helps to manage financial resources for
contracts and services to meet those needs.
Mr. Johnson often volunteers to assist in other program areas in the BSC, ensuring the success of the BSC as a
whole.

4. PROGRESS REVIEW / COMMENTS

Mr. Johnson continues to excel in his position. He provides excellent support to the counties for which he is
responsible. He spends time in each county office and is closely aligned with the local office staff responsible for
contracts and expenditures.
Mr. Johnson is knowledgeable in the area of contracts and expenditure monitoring. He is respected within the
counties, the BSC, and at the state level.
Mr. Johnson continues to be interested in additional projects and assignments that will further his knowledge and
professional future. He can be relied upon to complete projects and provide a quality product.
Mr. Johnson is a respected professional. As BSC Director, I rely on his expertise to ensure our contracts are
appropriately monitored, and that our expenditures are appropriate (& funds not overspent!).
Mr. Johnson has never refused or even questioned an assignment provided to him. He has met each deadline - even
when it was drastically reduced due to oversight on my part! He is a true asset to the team, and the MDHHS!
Mr. Johnson is a high-performing analyst in BSC 3. He has a bright future with MDHHS in whatever capacity he
chooses.
