Είναι ο 1ος
αντι-TNF που έλαβε ταυτόχρονα έγκριση για την ΡΑ,
την ΨΑ και την ΑΣ
Σε συνδυασμό με ΜΤΧ, βελτιώνει αποτελεσματικά τα σημεία και
συμπτώματα της ΡΑ και είναι γενικά καλά ανεκτή σε ασθενείς με
ΡΑ και ανεπαρκή ανταπόκριση στη ΜΤΧ
Ο συνδυασμός της με ΜΤΧ μειώνει σημαντικά τα επίπεδα της Ε-
σελεκτίνης, της IL-18, του αμυλοειδούς και του ΜΜP-9 στον ορό
Χορηγείται εφάπαξ 1 φορά τον μήνα υποδορίως
Το προφιλ ασφάλειας και η αποτελεσματικότητα του είναι

33. Certolizumab : Συμπεράσματα
Το Certolizumab σε συνδυασμό με MTX, ενδείκνυται για τη θεραπεία
της μέτριας έως σοβαρής ενεργού ΡΑ σε ενήλικες ασθενείς μη αντα-
ποκρινόμενους σε DMARDS (περιλαμβανομένης της ΜΤΧ)
Συγκριτικά με placebo, η μονοθεραπεία με 400 mg Certolizumab
pegol κάθε 4 εβδομάδες βελτιώνει αποτελεσματικά τα σημεία και
συμπτώ- ματα της ενεργού ΡΑ σε ασθενείς που δεν έχουν
ανταποκριθεί στη θεραπεία με DMARDs
Το Cimzia, σε συνδυασμό με ΜΤΧ, αναστέλλει την πρόοδο των
ακτινολογικών αλλοιώσεων και βελτιώνει την φυσική λειτουργία
34. Δράση αντι-TNF σε ασθενείς με ΡΑ
και συμπαρομαρτούντα νοσήματα
Οι αντι-TNF μπορεί να βελτιώσουν την διάθεση και τα νοσήματα
άγχους
Η θεραπεία με αντι-TNF είναι αποτελεσματική, ασφαλής και καλά
ανεκτή σε ασθενείς με ηπατίτιδα C
Οι αντι-TNF, σε συνδυασμό με περιοδοντική θεραπεία, βελτιώνουν
τα περιοδοντικά νοσήματα
Η αντι-TNF-α είναι πολύ αποτελεσματικοί σε ασθενείς με ενεργό ΡΑ
και συνυπάρχοντα νοσήματα του κολλαγόνου
35. Αποτελεσματικότητα αντι-TNF : Συμπεράσματα
Οι αντι-TNF παράγοντες βελτιώνουν αναμφισβήτητα την πρόοδο
της ΡΑ στο επίπεδο του οστού και τις κλινικές εκδηλώσεις της
νόσου, αν και μερικοί ασθενείς δεν ανταποκρίνονται ή δεν διατη-
ρούν την ανταπόκριση με την πάροδο του χρόνου
Η ανταπόκριση διαφέρει από ασθενή σε ασθενή πιθανώς λόγω
της διαφοράς στην περιοχή δράσης και της μοριακής δομής των

frequently (such as every 3 to 6 months) for patients in sustained low disease

6. The use of validated composite measures of disease activity, which include
joint assessments, is needed in routine clinical practice to guide treatment
decisions.

Δεν υπάρχουν ενδείξεις ότι η θεραπεία με αντι-TNF συνδέεται με την
ανάπτυξη κακοήθων νοσημάτων (καρκίνου)
Δεν υπάρχουν αντενδείξεις θεραπείας με αντι-TNF σε ασθενείς με ΡΑ

Οι ασθενείς με ιστορικό κακοήθους νοσήματος πρέπει να
παρακολου-θούνται με προσοχή δοθέντος ότι μπορεί να είναι
περισσότερο επιρ-ρεπείς σε αναζωπύρωση του κακοήθους
νοσήματος
Η ανάπτυξη όγκων του δέρματος πρέπει να παρακολουθείται με

68. Αντι-TNF και ηπατίτιδα
Οι χρόνιοι φορείς HBV (θετικό αντιγόνο επιφανείας) πρέπει να
παρακολουθούνται για σημεία και συμπτώματα ενεργού λοίμωξης
από HBV στη διάρκεια της θεραπείας με αντι-TNF και αρκετούς
μήνες μετά την διακοπή της
Οι αντι-TNF πρέπει να διακόπτονται εάν προκαλέσουν
αναζωπύρωση της ηπατίτιδας Β
69. Αντι-TNF και άλλες λοιμώξεις (π.χ. πνευμονία)
Εάν η λοίμωξη είναι σοβαρή, η θεραπεία με αντι-TNF πρέπει να
διακόπτεται
Εάν είναι ήπια, η θεραπεία με αντι-TNF μπορεί να συνεχισθεί μετά
την ύφεση της λοίμωξης (μετά από πόσο διάστημα ;;;)
Ο ασθενής πρέπει να ενημερώνεται για την πιθανότητα
υποτροπής της λοίμωξης μετά την επανέναρξη της θεραπείας με
αντι-TNF (;;;;)
70. Αντι-TNF και εμβολιασμοί
Οι ενβολιασμοί είναι προτιμότερο να γίνονται πριν από την έναρξη
της θεραπείας με ΒΠ ([π.χ. influenza, polysaccharide Pneumovax,
Hep B) και ύστερα στη διάρκεια της θεραπείας, εφόσον υπάρχει
σοβαρός λόγος
Ζωντανά εξασθενημένα εμβόλια (π.χ. BCG, κίτρινου πυρετού,
έρπητα ζωστήρα και πολιομυελίτιδας) αντενδείκνυνται
71. Αντι-TNF και χειρουργικές επεμβάσεις
Συνιστάται να διακόπτεται το etanercept 2-4 εβδομάδες και
τοadalimumab και το infliximab, 4-8 εβδομάδες πριν από
μείζονες χειρουργικές επεμβάσεις
Η θεραπεία με ΒΠ μπορεί να ξαναρχίσει μετεγχειρητικά
εφ΄όσον δεν υπάρχουν ενδείξεις λοίμωξης και η επούλωση
έχει συντελεσθεί κανονικά
72. Εργαστηριακή παρακολούθηση
Εργαστηριακές εξετάσεις κάθε 2-3 μήνες (ΤΚΕ, CRP, πλήρεις
εξετάσεις αίματος, έλεγχος νεφρών και ήπατος)
Α/α θώρακα (πριν από την έναρξη της θεραπείας και μετά εάν
εμφανισθούν αναπνευστικά συμπτώματα ή LTBI, ή σε καπνιστές).
Mantoux ή/και QuantiFERON assay (προθεραπευτικά) και μετά την
έναρξη της θεραπείας εάν ο ασθενής επανεκτεθεί σε ΤΒ
Δείκτες ηπατίτιδας (B και C) (πριν από την έναρξη της θεραπείας με

In rheumatoid arthritis, progressive functional disability is determined by 2 interrelated but distinct processes: synovial inflammation and joint destruction.
Looking at the timing of RA outcomes helps clarify the relationship between synovial inflammation, radiographic bone destruction and their long-term correlation with disability.
The first thing to note is that all three are seen early in the disease.
Although there is a high correlation between inflammation & impaired physical function in early RA, the strength of this relationship declines over time and joint destruction is the main driver of disability in late disease.
Indeed, fluctuations in disability will become less pronounced in later stage RA and become much more closely correlated with radiographic evidence of joint damage over time.

However, in the real world, poor compliance to therapy can impact therapy effectiveness.Objectives: To assess the compliance of patients from the Canadian provinces of Saskatchewan and Quebec administered either infliximab or etanercept for the treatment of rheumatoid arthritis (RA).Methods: A retrospective analysis of the administrative claims data from the Saskatchewan Drug Plan and Extended Benefits Branch database from September 2001 to September 2003 and from the Régie de l''assurance maladie du Quebec (RAMQ) from November 2002 to October 2004 was performed. Compliance of both patient cohorts was determined with compliance defined as the actual number of filled prescriptions divided by the expected number of prescriptions (based upon the manufacturers product monograph). Compliance to therapy is commonly defined as a patient who is compliant greater than 80% of the time. The interpretations and conclusions contained herein do not necessarily represent those of the Governments of Saskatchewan or Quebec or their respective Departments of Health.Results: Information was received on 501 RA patients (149 in Saskatchewan and 352 in Quebec) receiving at least one claim for infliximab and 593 RA patients (161 in Saskatchewan and 432 in Quebec) receiving at least one claim for etanercept. The compliance of those who remained on therapy for 12 months or greater from both Saskatchewan and Quebec are presented in Table 1. For lower compliance ratios (expected dosage/reported dosage) infliximab and etanercept were relatively similar, but for higher compliance, the percentage of infliximab patients remained high, whereas the etanercept patients decreased, resulting in substantial and significant (P<0.001) differences in compliance ratios. The percentage of patients receiving a minimum of 80% of the number of prescriptions was significantly higher for infliximab (96%) than for etanercept patients (76%). All of these differences were statistically significant (P<0.001).Table 1. Compliance to anti-TNF therapy is essential to not only control the symptoms of RA, but to also arrest or slow joint destruction. Eligible patients were randomized to 3 treatment arms: certolizumab pegol 400 mg every 2 weeks plus methotrexate, certolizumab pegol 200 mg every 2 weeks plus methotrexate (pay attention: in this arm 400 mg of certolizumab pegol was given at weeks 0, 2 and 4), and placebo plus methotrexate. The number of patients in both active drug arms was double the number of patients in the placebo arm (roughly 400 patients versus 200 patients).
Patients who failed to achieve an ACR20 response at both Weeks 12 and 14 were designated treatment failures and were withdrawn from the study at Week 16. Patients who withdrew at Week 16 or who successfully completed the trial were offered enrolment in an open-label extension study of certolizumab pegol 400 mg every 2 weeks. For patients who withdrew early for reasons other than withdrawal of consent, radiographic assessment was performed (and was mandatory) at the time of withdrawal and at Week 52.
Co-primary endpoints were ACR20 response rate at Week 24 and radiographic progression, as measured by the mean change from baseline in the van der Heijde-modified total Sharp score at Week 52.

The 10 statements
1.The primary target for treatment of rheumatoid arthritis should be a state of clinical remission.
2.Clinical remission is defined as the absence of signs and symptoms of significant inflammatory disease activity.
3.While remission should be a clear target, based on available evidence low disease activity may be an acceptable alternative therapeutic goal, particularly in established, long-standing disease.
4.Until the desired treatment target is reached, drug therapy should be adjusted at least every 3 months..
5.Measures of disease activity must be obtained and documented regularly, as frequently as monthly for patients with high/moderate disease activity or less frequently (such as every 3 to 6 months) for patients in sustained low disease activity or remission.
6.The use of validated composite measures of disease activity, which include joint assessments, is needed in routine clinical practice to guide treatment decisions.
7.Structural changes and functional impairment should be considered when making clinical decisions, in addition to assessing composite measures of disease activity.
8.The desired treatment target should be maintained throughout the remaining course of the disease.
9.The choice of the (composite) measure of disease activity and the level of the target value may be influenced by considerations of co-morbidities, patient factors and drug related risks.
10.The patient has to be appropriately informed about the treatment target and the strategy planned to reach this target under the supervision of the rheumatologist.

Background: Although it is well known that inhibition of TNF-alpha may increase the risk of tuberculosis (TB), this risk is assumed to: (a) largely occur shortly after start of TNF-antagonist therapy, and (b) to have been essentially removed by pre-treatment screening for TB. Objectives: In this study, we used national registers and the Swedish Biologics Register to assess whether the risk of anti-TNF associated TB is really a time-limited phenomenon counting from treatment start, and whether increased clinical awareness of the risk of TB has removed the occurrence of anti-TNF-associated TB in clinical practice. Methods: Using the Swedish Biologics Register (ARTIS), a pre-existing national cohort of patients with RA, and matched individuals from the general population register, we created three cohorts of (i) all 6,603 RA-patients starting TNF-antagonist treatment 1998 until July 2006, (ii) 67,705 biologics-naive contemporary comparator RA-patients, and (iii) 471,047 subjects from the general population. Information on occurrence of tuberculosis was gathered through linkage to the Swedish Tuberculosis Register (the mandatory Swedish notification system for tuberculosis) 1998 through January 2008. All cases of TB occurring in the anti-TNF treated cohort were subsequently validated against the medical files. Relative risks were estimated using Cox-regressions, taking age, sex, and county of residence into account. Relative risks for TB associated with anti-TNF therapy were assessed both vs biologics-naive RA and vs the general population, overall, by year of first anti-TNF treatment start, and by time since first anti-TNF treatment start. Results: 25 cases of tuberculosis were diagnosed following start of anti-TNF therapy, corresponding to a crude incidence of 77 per 100,000. The median duration between start of anti-TNF therapy and TB-diagnosis was 20 months. 21 of the 25 cases occurred during or within 3 months of discontinuation of anti-TNF therapy. Of the 25 cases, 9 were ever treated with etanercept, 5 with adalimumab, and 17 with infliximab (5 patients had been treated > one drug).
Compared to biologics-naive patients with RA (64 cases with TB, incidence = 20 per 100,000), the relative risk was 4.3 (95% CI 2.6-7.3). The relative risk were higher in individuals starting anti-TNF therapy 1998-2002 (RR=6.1, 95% CI 3.5-11) than among those starting later (RR for first starters 2003-2004 = 1.9, 95% CI 0.7-5.4, and RR for first starters 2005-2006=3.1, 95% CI 0.7-13). The relative risk remained increased even > 3 years after first start of anti-TNF therapy (not shown). When assessed by drug, the relative risk was 2.5 (95% CI 1.0-6.1) for etanercept, 5.2 (95% CI 2.7-11) for infliximab, and 3.1 (95% CI 1.1-8) for adalimumab, taking multiple treatments into account.
Compared to the general population cohort (161 cases of TB, incidence = 5 per 100,000), the relative risk associated with biologics-naive RA (64 cases with TB, incidence = 20 per 100,000) was 3.2 (95% CI 2.4-4.3) and the relative risk associated with RA treated with anti-TNF therapy was 15 (95% CI 9.6-24). Conclusion: These updated analyses extend our previous findings that Rheumatoid Arthritis is a risk factor for TB even in the absence of anti TNF therapy, and that in daily clinical practice in a low TB-incidence country, anti-TNF therapy is associated with increased risks for TB. In addition, this study suggests that risks are indeed elevated for all three anti-TNF agents, and that despite increased vigilance, single cases with TB continue to occur. Finally, at least for patients starting anti-TNF therapy during the first years following market entry, the elevated risk of TB is not transient.
