Each SOAP Note is worth 100 points. The original grading criteria has been modified to make it
clear how points would be allocated. The point allocation for each major category was not
altered.
SOAP Notes should reflect new experiences related to your practicum experiences in this
course. Your submissions should be your original work and cannot be repurposed from previous
courses.
Your SOAP Note should be based on an individual patient and address the following:
Subjective: Details the patient provided regarding his or her personal and medical
history (15 points)
How points will be allocated
CC (1 point)
Pertinent positives (OLDCARTS) (5 points)
Pertinent negatives (from ROS) (5 points)
Pertinent PMH, SH, and FH (3 points)
Medications and drug/food allergies are not included (1 point)
Objective: Observations that you make during the physical assessment (15 points)

Assessment: Need to list your priority diagnosis first. For each priority diagnosis, if
possible list at least 3 differential diagnoses. Focus on the “horses” first, then “zebras”.
Support your selection with rationales keeping in mind the likelihood of each
subsequent diagnosis based on the patient’s age, gender, and race, risk factors, lifestyle
choices, and co-morbidities. (30 points – 10 points for each priority diagnosis. If less
than 3 are appropriate to include – simply no other diagnoses to consider – you will
receive all 30 points. Please do not “stretch” to find 3 if they are not actual possibilities.

The description here is a follow-up to the material presented in class. The SS template and assignment instructions are posted in Moodle. If you have questions after reading this please talk with me. – Thanks – Curt
To use the SS to its greatest advantage the following conditions for your business must be met:
1. It must be a start-up business
2. You must borrow money and demonstrate how you will repay your debt.
3. You must provide your own capital. This is the “sale of stock” row on the CF tab. Even if you are not a corporation, think of this as your owner financial contribution to the firm.
4. Your business must provide a physical product. You may not start a service firm such as an athletic facility (a running camp), an accounting firm (we’ll do your taxes), guide services (let me help you catch fish), or a consulting business (I charge $150 per hour with no appreciable cost of goods sold). A restaurant is OK because there is a definite cost per unit sold (that $6 burger has $1 worth of ingredients). I suggest that you manufacture items with easily identifiable revenues and expenses.
5. Pay yourself a decent living wage and show a profit. Why would you make up a failing company where you also get to be poor?
I believe this assignment is more valuable with a higher level of achievement and understanding if you are required to complete various portions on a regular basis throughout the term. As you complete the tabs on the SS you should also be completing a Word document explaining your assumptions. Later, as you need to change your SS values, be sure to ALSO CHANGE the assumptions on your Word document.
For the first submission you will submit two documents; a copy of your completed SS to-date and your corresponding Word document. In subsequent weeks you will REPLACE each of those documents with updated versions until you have completed the entire assignment. Only one partner should be submitting the assignments. Make sure all LAST names are in the title of the submitted documents so that I can review each and give credit to all group members.
Assignment portions are due within two hours of the completion of the last class period of the week. You may complete each portion of the overall assignment earlier than required, but late work will realize a modest penalty. Still, turning in your work late is vastly preferable to not turning in your work at all. The completed assignment is due two hours after the completion of the last regular day of class for the term (dead week).

Working With Survivors of Sexual Abuse and Trauma: The Case of Rita
Rita is a 22-year-old, heterosexual, Latina female working in the hospitality industry at a resort. She is the youngest of five children and lives at home with her parents. Rita has dated in the past but never developed a serious relationship. She is close to her immediate and extended family as well as to her female friends in the Latino community. Although her parents and three of her siblings were born in the Dominican Republic, Rita was born in the United States.
A year ago, Rita was sexually assaulted by an acquaintance of a male coworker. Rita and a female coworker met Juan and Bob after work at a local bar for a light meal and a few drinks. Because Rita had to get up early to work her shift the next day, Bob offered to drive her home. Instead of taking Rita directly home, however, he drove to a desolate spot nearby and assaulted her. Afterward, Bob threatened to harm her family if she did not remain silent and proceeded to drive her home. Although Rita did not tell her family what happened, she did call our agency hotline the next day to discuss her options. Because Rita’s assault occurred within the 5-day window for forensic evidence collection of this kind, Rita consented to activation of the county’s sexual assault response team (SART). Although she agreed to have an advocate and the sexual assault nurse examiner (SANE) meet her at the hospital, Rita tearfully stated that she did not want to file a police report at that time because she did not want to upset her family. The nurse examiner interviewed Rita, collected evidence, recorded any injuries, administered antibiotics for possible sexually transmitted infections, and gave Rita emergency contraception in case of pregnancy. The advocate stayed with Rita during the procedure, supporting her and validating her experience, and gave her a referral for individual crisis counseling at our agency.

To use the SS to its greatest advantage the following conditions for your business must be met:
1. It must be a start-up business
2. You must borrow money and demonstrate how you will repay your debt.

4. Your business must provide a physical product. You may not start a service firm such as an athletic facility (a running camp), an accounting firm (we’ll do your taxes), guide services (let me help you catch fish), or a consulting business (I charge $150 per hour with no appreciable cost of goods sold). A restaurant is OK because there is a definite cost per unit sold (that $6 burger has $1 worth of ingredients). I suggest that you manufacture items with easily identifiable revenues and expenses.
5. Pay yourself a decent living wage and show a profit. Why would you make up a failing company where you also get to be poor?
I believe this assignment is more valuable with a higher level of achievement and understanding if you are required to complete various portions on a regular basis throughout the term. As you complete the tabs on the SS you should also be completing a Word document explaining your assumptions. Later, as you need to change your SS values, be sure to ALSO CHANGE the assumptions on your Word document.
For the first submission you will submit two documents; a copy of your completed SS to-date and your corresponding Word document. In subsequent weeks you will REPLACE each of those documents with updated versions until you have completed the entire assignment. Only one partner should be submitting the assignments. Working With Survivors of Sexual Abuse and Trauma: The Case of Rita
Rita is a 22-year-old, heterosexual, Latina female working in the hospitality industry at a resort. She is the youngest of five children and lives at home with her parents. Rita has dated in the past but never developed a serious relationship. She is close to her immediate and extended family as well as to her female friends in the Latino community. Although her parents and three of her siblings were born in the Dominican Republic, Rita was born in the United States.
A year ago, Rita was sexually assaulted by an acquaintance of a male coworker. Rita and a female coworker met Juan and Bob after work at a local bar for a light meal and a few drinks. Because Rita had to get up early to work her shift the next day, Bob offered to drive her home. Instead of taking Rita directly home, however, he drove to a desolate spot nearby and assaulted her. Afterward, Bob threatened to harm her family if she did not remain silent and proceeded to drive her home. Although Rita did not tell her family what happened, she did call our agency hotline the next day to discuss her options. Because Rita’s assault occurred within the 5-day window for forensic evidence collection of this kind, Rita consented to activation of the county’s sexual assault response team (SART). Although she agreed to have an advocate and the sexual assault nurse examiner (SANE) meet her at the hospital, Rita tearfully stated that she did not want to file a police report at that time because she did not want to upset her family. The nurse examiner interviewed Rita, collected evidence, recorded any injuries, administered antibiotics for possible sexually transmitted infections, and gave Rita emergency contraception in case of pregnancy. The advocate stayed with Rita during the procedure, supporting her and validating her experience, and gave her a referral for individual crisis counseling at our agency.

As you might be aware, endorsements have some economic
effects which have some direct impacts on the firm. For
instance, I can use Tiger Woods as an example. When the
endorsement that dealt with Tiger Woods were announced, the
return on investment (ROI) raised by 1%. These endorsements
included Titleist, Nike, and American Express. This is simply
because his success in Golf enabled him to increase the sales of
Nike specifically. Again, when Michael Jordan announced his
comeback to basketball in 1995, the endorsing firms reported
having experienced an increase of 2% of stock returns, which
consequently resulted to $1 billion in market value. Therefore,
my point is, the concept applies to most of our activities for
income. Hence we will like to see you in the same position

Memo to the boss: This is a good draft, Zihan, but I felt lost
while reading all of the reasons and points you were presenting
in the memo. My impression is that you made the message
much more complicated than it needs to be; you are discussing
so many different points that your reader would probably feel as
confused and overwhelmed as I did. In addition to stating the
purpose in the purpose statement, it would be important to
format the body of your memo. Create two or three sections,
either two sections about the problem with the current policy
and the benefits of the proposed policy modification, or three
sections about the problem, proposal, and benefits. After that,
but back your coverage. In the new section about the problem
with the current policy, focus on just one problem and bring in
research evidence to back up your claim that this policy,
already, has harmed the company. In the new section about
13. your proposed policy modification, focus on proposing just one
type of policy change, explain how it would work, and point out
its primary benefits to the company.
I believe that with these revisions, all aimed at simplifying the
memo, you would improve overall clarity of your message and
the memo would become much more persuasive and convincing.

dozens of policies, so you need to be more precise about which
policy this memo is about
Sportstars is a very lucrative company that is well known across
the country, and a company that everyone would wish to work
for. The policies employed by the company are good, but it
needs some modification so that it can accommodate more and
more potential athletes who can be the major assets of the
company. For the few times, I have been in the company, I have
been encouraged and impressed by how you manage and engage
resources in different areas of our company. Comment by
Author: Follow this up with the purpose of this memo: “Below,
I propose a modification of our Rookie Representation Policy so
that we can increase our client pool and in that way, increase
our profits….” Comment by Author: This distracts attention
from the main purpose of your memo expressed in the previous
sentence, so I suggest that you move this sentence right after

The policies, however, need some modifications to play a fair
and productive role in the market. The policy focuses so much
on athletes who win a championship, who in turn ask for higher
fees than the rising athletes. Besides, the policy does not
support spending time and resources on young athletes who

higher stakes that in turn siphons resources and money on

headings. The first major section needs to focus on how the

It is very mindful of you to use the foolproof tactic, which has
been very helpful to the company. However, the tactic might
become extinct at some point leaving us with less option in
salvaging the situation. Comment by Author: Could you
expand about this? What makes you think that this is a
possibility?
Another problem is that the competing firms might become
competitive enough to maintain the young stars when they start
emerging as winners. As such, the policy jeopardizes the
reputation of our company by leaving away the rising stars who
might take this firm to the next level.
As much as the present policy might be very significant to the
company, modifying it to endorse rising stars might be of the
same importance. Thus, the rising stars (athletes) can be
represented in various ways that can benefit both the firm, the
clients, and athletes. The logic behind this is that rising stars
demand less amount of fees compared to athletes who have
15. already won the championship. The fact that they take less
amount of money leaves the firm to invest funds in projects that
can build the company to become even stronger. By utilizing the
observation trend of the rising athlete, the company can easily
forecast the future position of an individual hence decide
whether to endorse. Comment by Author: This begins the
second section about your proposed policy modification. After
the section heading, focus on your proposal – omit this and
replace it with “I propose that we modify our X policy to
endorse rising stars.
Then explain your rationale for proposing this plan and explain
how the plan can work.
Then present the third section where you can discuss benefits of
this proposed plan.
In addition, competition is always no less than gambling
between the competitors, and anything can happen. The first
might become the last, and the last might become the first.
Therefore, if the policy could accommodate the rising stars in
representing them for endorsement, it will motivate their
morale, and there might be some better outcome. Sportstars Inc.
also has resources and financial duties which can be a
constraint. However, if the negotiation on the endorsements
become successful, the firm can inject more resources and
finances to facilitate any help that might be required for the
activity to take place.
I also suggest evaluation of ethical standards that allow athletes
to either be signed or be declined depending on the ethical
standards of the firm. An athlete might be a great champion, yet
he/she might not meet the ethical standards. Thus, the athlete
might deliver the least contrary to what is expected of him or
her. Also, ethical standards will reduce or avoid the conflicts
that might be involved in signing one athlete in more than two
16. endorsements, which might bring about conflict of interest
between your firm and the endorsing company.
Sportstars Inc. also could involve the signing of multiple athlete
representations. This can be achieved by using many variables
during negotiation that is likely to come into play in
considering the athlete's value, which our company has some
duty in determining the value.
Ethically, the representation can be unfruitful if there is a
significant risk that limits the attorney’s ability to carry out
representation materially. As such, it will be very significant to
implement this policy to virtually include attorney’s power to
cure any potential conflict that might arise. The strategy can
help the firm from losses, since the attorney can provide
competent to each affected client and if the attorney gets the
client’s informed consent. Attorney’s power is always limited to
conflicts that might arise from agent’s continuous
representation of athletes who are mainly interested in
capitalizing on their salary, or companies that are interested in
conserving money spent on athletes. Personally, I am very
aware Sportstars is using its strategies, but it seems it falls into
this trap that we did not foresee.
There is also the consideration of the fees that our firm charge
for representation. Basing on this, I would like to suggest
ethical consideration on the reasonableness of fees charged.
Most representatives charge a percentage of the amount of the
negotiated contract. But with the negotiation, million dollar
contracts of the agent fees might come down to thousands of
dollars hourly, which might be considered unreasonable, hence
being unethical without our intentions. Thus, I was suggesting if
anything could be done about that situation.
I also wanted to point out some concerns I have about firms in
the same industry.
Representatives allow negotiation involving star athletes to take
17. priority over the negotiation involving rookie stellar athletes.
However, what I noticed is that the agents get into situations
where the more athletes they represented, the more they cost
people jobs. This is because the agent represented more than
one athlete on the same team that was competing to play in the
same position.
Policy on the number of athletes is also very important since a
smaller number of athletes per agent could allow the agent to
get experience while reducing the likelihood of high expenses
and conflicts of interest which can harm the athletes.
I am very delightful and grateful to serve as an employee in
Sportstars Inc., and I’m very passionate and optimistic that
Sportstars is going to achieve its best to become one of the most
powerful and known firms in the world, that is trusted by iconic
superstars and endorsing firms. I hope what I have delivered

Proposal
The Proposal will be worth 30% of your final grade in this
course and will be due by
midnight, Wednesday, January 18. For this assignment, you are
required to write about
an actual or potential problem for an actual business; this
business could be a
18. small/medium/large company, any religious institution (a
mosque, synagogue, church,
etc.), any social service agency, any educational institution, or
any other company or
agency that serves people in some way.
Please do NOT write about problems that involve large-scale
problems that your
proposal could not reasonably resolve, such as problems you
might find in politics and
government or that involve religious/ethical/moral beliefs. The
type of proposal that
you’ll be writing needs to be similar to those written at
companies, which tend to focus
on small/medium-sized problems that have concrete causes and
effects that affect one
agency, company, or institution. The smaller the size of the
problem, the more
convincing and effective your proposal will be.
Also, please identify problems in agencies, companies, or other
institutions that you
know really well. For example, some students in this class are
parents and have written
strong proposals about a problem at their children’s daycare
centers or schools; some
students work part- or full-time and have experienced or
observed problems at those
jobs; and some students don’t have children, work experience,
or other such
connections in this area, but are able to identify problems here
at UWM or at a
community college they attended in this area.
Here are just a few examples of problems that have worked well
in past proposals

What are the main purposes of a Proposal?
Most business professionals write reports of various types. One
of the most common is
the proposal. A proposal can be in the form of an email, a
lengthy memo like the Ethics
Packet memo to the boss, or a formal report. For this
20. assignment, you will write a
proposal as a formal report with required sections in it.
A proposal typically has three central purposes:
x To convince the reader that a problem actually exists
x To convince the reader that this problem is serious
x To convince the reader that this problem has had a serious
negative impact on
the company, or might have fairly soon.
The proposal that you will write will need to fulfill all three of
these purposes in order to
receive an A-level grade.
To convince the reader that a serious problem actually exists,
you will write a fairly
lengthy Problem section that fulfills the first two purposes
listed above. There, you will
need to describe the problem in considerable detail, including
what gave rise to the
problem, what the problem looks like now (its scope, its
nature), and the problem’s
actual or potential negative impact on the company. As you
describe the causes, nature,
and negative effects of the problem, you will need to pull in
detailed description and
examples, but also some research evidence to support your
claims that the problem
exists and is serious. This research evidence might include
interview quotes, survey
statistics, data/statistical trends from company records, and
your own observations or
experiences with the situation. Note that there is no need to
make an emotional appeal
21. in a typical business proposal; instead, the combination of your
description, argument,
and evidence/examples will convince the reader sufficiently
well.
To convince the reader that your proposed plan is feasible and
has a strong chance to
either minimize or eradicate the problem, once again you will
need to describe all
aspects of your plan in some detail and to pull in evidence that
backs of your claims.
This research evidence can include everything listed in the
previous paragraph or
comparative data, such as evidence that the same plan has
worked really well at one or
more other companies, or a comparison of this plan and other
possible plans that
demonstrates that this plan is superior and ought to be approved
and implemented.
What if you can’t gather enough research evidence from actual
companies within the
tight time frame of this course? Please do your best to gather
some evidence from the
company that is experiencing the problem: even if all you can
do is conduct one
interview and look at annual company reports from the past
three years, that would be
a strong effort! However, for this particular assignment, I will
also accept the following
types of research:
x You might find descriptions of the same problem at another
company and the
successful implementation of a solution that resolved it—that
comparative data
22. would work well in the proposal.
x You can also do some memory searching and write down what
you remember
noticing or experiencing yourself when you worked at a
company or attended a
school, etc.
x Some magazines and newspapers publish descriptions of
problems/solutions and
you might be able to locate those types of articles with the help
of a reference
librarian at the UWM Golda Meir Library or at a community
library, or find them
on the Internet.
x And finally, given the tight time frame of this course, I will
give you permission to
fabricate (make up, invent) some research evidence to support
your claims; just
as an example, you can invent an adverse statistical trend over
the past five
years that give evidence of a problem hurting the company’s
profit margin.
Important: even though some evidence in your proposal can be
fictional, be sure
that most evidence that you use in your proposal is real. And
whether the
evidence is fictional or real, write citations at the end of your
report, including
imaginary citations for the fictional evidence and actual
citations for the actual
evidence that you found and used in your proposal.
23. Requirements
Audience: This proposal will be written for me – instead,
pretend that you are writing it
for an actual person or group who would make the final decision
whether to approve
the proposed plan. You will not actually submit this proposal to
that audience, unless
you want to do so, but aim the entire proposal to that individual
or group.
Length: This proposal should be between 5 and 10 pages,
single-spaced, not including
the Cover Page and Executive Summary.
Visuals: You are required to include at least two visuals in your
proposal, which can
include actual photographs, tables, charts, flow charts, or other
types of visual aids that
can help readers understand a problem and solution and can
back up your claims.
Sections: You are required to use the following sections:
1. Cover page. On this page, place a relevant photograph or
other professional
looking visual, in addition to the title of your report, the name
of the reader and
company (you could write, for example: “Submitted to:”
followed by the name
of your contact at the company and that person’s job title), your
own name and
job title (again, you could write “Submitted by” followed by
that information),
and the month/year of submission.
24. 2. Executive Summary. This is a half- or full-page section in
which you provide a
brief summary of the most important points of your report. Use
headings such
as these: Problem, Proposed Plan, Feasibility, and Benefits. In
each section, just
briefly summarize the main points in your proposal in one or a
few sentences. It
is also acceptable to include one or a few vertical lists in these
sections.
3. Problem. This is one of the main sections and needs to begin
on a new page. In
this section, provide a thorough, fairly detailed description of
the problem that
the organization is experiencing and give evidence that it exists
and is serious to
the organization. Describe its probable or actual causes, the
scope and nature of
the problem right now, and its actual or probable negative
effects (or impact) on
the organization. If you want, you can create subsections in
this major section,
25. such as Causes of the Problem, Nature of the Problem, and
Negative Impact of
the Problem – or How did the problem arise? What does the
problem look like
right now? How has the problem hurt our company?
4. Proposed Plan. In this section, describe your proposed plan
fully and in detail,
and objectively so that the reader will be able to see very
clearly it has the
potential to resolve the problem or fulfill the need. Within this
section, please
create subsections that you can call, for example, What is the
Proposed Plan?
How is the Proposed Plan Feasible? How will this Plan Benefit
the Company?
Some ideas for making this section especially persuasive and
convincing:
x Consider using comparative data anywhere in this main
section that
shows, convincingly, that your proposed plan has worked well
(was
feasible and had benefits) at another company or at a group of
companies.
x It’s effective to provide an overview of what the plan consists
of and then
propose that the company approach the plan as a pilot and then
decide,
later on, if it wants to make it permanent; you can then, for
example,
describe the pilot plan in terms of stages: planning,

evaluation. While describing those stages, provide specific
dates – for
approval of the plan, planning, implementation, evaluation, and
then
making a final decision regarding the plan. Also provide
specific steps
that will be taken for each stage.
x When discussing Feasibility, include subsections covering
personnel (who
will do what/their qualifications), resources (what equipment or
other
tangible items will be needed/whether those are available or
affordable),
and budget (the costs/whether the plan is cost-effective and

5. Closing Thoughts. You can also call this conclusion,

I will not be showing models of proposals, because each of you
needs to make all writing
decisions on your own while also meeting the assignment
requirements and I do not
want models to influence anyone to use the same decisions that
another student used
in the past. My best advice for you is to do the following:
x Structure and format this report by following the instructions
from this file VERY
carefully, and
28. x Apply to this proposal what you have learned in this course
about writing
effective business documents – for example, begin each major
section and each
subsection with a quick introduction or overview; use a set of
subsections if you
have quite a bit of information to present but use a list if you
don’t have that
much information; and so on.
