In a hospital setting with various departments, a
number of data can be accessed and retrieved within the different departments. Clinical data is a staple resource for most health and
medical research. Clinical data is either collected during the course of
ongoing patient care or as part of a formal clinical trial program. Clinical data
falls into six major types; Electronic health records;Administrative data; Claims data; Disease registries; Health surveys; Clinical trials data. Clinical research data may be available through national or
discipline-specific organizations. Level of access is likely restricted but
available through proper channels. Electronic
health record is the purest type of electronic clinical data which is
obtained at the point of care at a medical facility, hospital, clinic or
practice. Often referred to as the electronic medical record (EMR), the EMR is
generally not available to outside researchers. The data collected includes
administrative and demographic information, diagnosis, treatment, prescription
drugs, laboratory tests, hospitalization, patient insurance, etc.
Administrative date is often associated with electronic health records; these
are primarily hospital discharge data reported to a government agency like AHRQ. Claims data describe the billable interactions
(insurance claims) between insured patients and the healthcare delivery system.
Claims data falls into four general categories: inpatient, outpatient,
pharmacy, and enrollment. The sources of claims data can be obtained from the
government (e.g., Medicare) and/or commercial health firms (e.g., United
HealthCare).

Patient-Centered Care
When electronic health records (EHRs) first entered the market, their primary focus was to collect and analyze patient information within health care settings. As technological capabilities grew, so did the interest in making these records available to patients. In addition, many health care professionals saw benefits in allowing the patient to enter his or her own health data into EHR platforms. Though many patients are already utilizing personal health records (PHRs) to manage and track their own health, some believe that an integrated system would provide a better, more comprehensive picture of a patient’s health history.
As a result, many EHR platforms are now equipped with a PHR tool. This PHR tool allows patients to enter health information as they would in a stand-alone PHR system. In addition, web-based portals within the EHR allow patients to access information entered by their physicians and health care providers.
Like many emerging trends and technologies, there is much discussion about the potential benefits and challenges of this type of integrated system. While many health care professionals are excited about the empowerment provided to patients, others express significant concerns about access, security, ethics, and other implications.

DE-IDENTIFICATION OF PROTECTED HEALTH INFORMATION PHI FROM FREE TEXT IN MEDIC...
Medical health records often contain clinical investigations results and critical information regarding patient health conditions. In these medical records, along with patient health information, patient Protected Health Information (PHI) such as names, locations and date information can co-exist. As per Health Insurance Portability and Accountability Act (HIPAA), before sharing the medical records with researchers and others, all types of PHI information needs to be de-identified. Manual de-identification through human annotators is laborious and error prone, hence, a reliable automated de-identification system is need of the hour.
In this work, various state of the art techniques for de-identification of patient notes in electronic health records were analyzed for their performance, based on the performance quoted in the literature, NeuroNER was selected to de-identify Indian Radiology reports. NeuroNER is a named-entity recognition text de-identification tool developed by Massachusetts Institute of Technology (MIT). This tool is based on the Artificial Neural Networks written in Python and uses Tensorflow machine-learning framework and it comes with five pre-trained models.
To test the NeuroNER models on Indian context data such as name of the person and place, 3300 medical records were simulated. Medical records were simulated by extracting clinical findings, remarks from MIMIC-III data set. For collection of all the relevant Indian data, various websites were scraped to include Indian names, Indian locations (all towns and cities), and Indian Hospital and unit names. During the testing of NeuroNER system, we observed that some of the Indian data such as name, location, etc. were not de-identified satisfactorily. To improve the performance of NeuroNER on Indian context data, along with the existing NeuroNER pre-trained model, a new pre-trained model was added to handle Indian medical reports. Medical dictionary lookup was used to reduce number of misclassifications. Results from all four pre-trained models and the model trained on Indian simulated data were concatenated and final PHI token list was generated to anonymize the medical records to obtain de-identified records. Using this approach, we improved the applicability of the NeuroNER system to Indian data and improved its efficiency and reliability. 2000 simulated reports were used for transfer learning as training set, 1000 reports were used for test set and 300 reports were used for validation (unseen) set.

DE-IDENTIFICATION OF PROTECTED HEALTH INFORMATION PHI FROM FREE TEXT IN MEDIC...
Medical health records often contain clinical investigations results and critical information regarding patient health conditions. In these medical records, along with patient health information, patient Protected Health Information (PHI) such as names, locations and date information can co-exist. As per Health Insurance Portability and Accountability Act (HIPAA), before sharing the medical records with researchers and others, all types of PHI information needs to be de-identified. Manual de-identification through human annotators is laborious and error prone, hence, a reliable automated de-identification system is need of the hour.
In this work, various state of the art techniques for de-identification of patient notes in electronic health records were analyzed for their performance, based on the performance quoted in the literature, NeuroNER was selected to de-identify Indian Radiology reports. NeuroNER is a named-entity recognition text de-identification tool developed by Massachusetts Institute of Technology (MIT). This tool is based on the Artificial Neural Networks written in Python and uses Tensorflow machine-learning framework and it comes with five pre-trained models.
To test the NeuroNER models on Indian context data such as name of the person and place, 3300 medical records were simulated. Medical records were simulated by extracting clinical findings, remarks from MIMIC-III data set. For collection of all the relevant Indian data, various websites were scraped to include Indian names, Indian locations (all towns and cities), and Indian Hospital and unit names. During the testing of NeuroNER system, we observed that some of the Indian data such as name, location, etc. were not de-identified satisfactorily. To improve the performance of NeuroNER on Indian context data, along with the existing NeuroNER pre-trained model, a new pre-trained model was added to handle Indian medical reports. Medical dictionary lookup was used to reduce number of misclassifications. Results from all four pre-trained models and the model trained on Indian simulated data were concatenated and final PHI token list was generated to anonymize the medical records to obtain de-identified records. Using this approach, we improved the applicability of the NeuroNER system to Indian data and improved its efficiency and reliability. 2000 simulated reports were used for transfer learning as training set, 1000 reports were used for test set and 300 reports were used for validation (unseen) set.

Although the original purpose of the HL7’s
Continuity of Care Documents (CCD) was to deliver clinical
summaries between healthcare organizations, nowadays they
are increasingly used for collecting patients’ health
documentation from various healthcare providers. Usually the
collected CCD documents are organized into hierarchical
structures that simplify the search of documents, e.g., grouping
together the documents by episode, clinical specialty or time
period. Yet each clinical document is stored as a stand-alone
artifact, meaning that each document is complete and whole

a whole in itself also has its drawback: the efficient usage of
patients’ health documentation often is data centric, meaning
that data should be extracted from various documents and

Objective: The purpose of this study was to identify core journals and other types of literature cited
in four major pediatric nursing journals and to characterize coverage of these resources in major
bibliographic databases. The study was part of the ‘‘Mapping the Literature of Nursing Project’’ of
the Medical Library Association’s Nursing and Allied Health Resource Section. It updates a similar
analysis published in 2006 and determines whether citation patterns have changed over time.
Methods: Cited references from articles published in 4 pediatric nursing journals between 2011 and
2013 were collected. Cited journal titles were ranked according to number of times cited and
analyzed according to Bradford’s Law of Scattering and the 80/20 rule to identify the most frequently
cited journals. Five databases were surveyed to assess the coverage of the most-often-cited journals.

Medical care institutions have provided care for their patients since old times before the digital technology era that we are in today. Medical institutions like clinics and hospitals which existed during those previous times, used paper based methods to get most of their basic operations done within the institutions. Operations like obtaining, saving and updating customer details, keeping appointment schedules, and sharing customer data with other institutions. Now in the modern era of technology, the same operations are needed but are simpler now than back then, thanks to digital technology.
The sharing of data between institutions is necessity in the medical profession in for various reasons. The institutions that require such data have different reasons for that as well. For these reasons, data sharing between institutions needs to be properly set and streamlined process for maximum efficiency.B. Need to Share Data
Of the many institutions that exist in the medical industry, two institutions are very crucial to the process of administering medical help to patients; Laboratories and Insurance companies.
Laboratories are essential to the process of diagnosing and treating an illness in a patient for various reasons. First of all, a patient’s diagnosis process can be a difficult problem and a rather complicated one too. When a doctor listens to a patient describe the symptoms of an illness, he/she gets a general idea of what a patient is suffering from and may need further information from a laboratory to confirm his findings. In such a scenario, the doctor sends the patient to a laboratory either within the institution or outside the institution. The laboratory will most definitely require accurate information about the patient to understand the basic nature of the condition of which the patient is required to be tested on, background information like allergies and any other relevant information. This information is usually given by the doctor or retrieved from data storage facilities like a file or a digital database.
Insurance companies are also essential in the process of treating a patient for various reasons. The major reason however is for the purposes of billing of patients expenses. These companies need information about the expenses incurred by a patient during treatment. Such information may include: laboratory test costs, drugs and medicine costs and doctor consultation fees. Proper communication and data sharing frameworks need to be put in place for this purpose as well.

Although the original purpose of the HL7’s
Continuity of Care Documents (CCD) was to deliver clinical
summaries between healthcare organizations, nowadays they
are increasingly used for collecting patients’ health
documentation from various healthcare providers. Usually the
collected CCD documents are organized into hierarchical
structures that simplify the search of documents, e.g., grouping
together the documents by episode, clinical specialty or time
period. Yet each clinical document is stored as a stand-alone
artifact, meaning that each document is complete and whole
in itself. Considering each document only as a complete and
a whole in itself also has its drawback: the efficient usage of
patients’ health documentation often is data centric, meaning
that data should be extracted from various documents and
then integrated according to specific criteria. Processing such
queries requires the integration of the data of the CCD
documents. In this paper, we present two ontology-based

appropriate depends on whether the header or the whole CCD

Objective: The purpose of this study was to identify core journals and other types of literature cited
in four major pediatric nursing journals and to characterize coverage of these resources in major
bibliographic databases. The study was part of the ‘‘Mapping the Literature of Nursing Project’’ of
the Medical Library Association’s Nursing and Allied Health Resource Section. It updates a similar
analysis published in 2006 and determines whether citation patterns have changed over time.
Methods: Cited references from articles published in 4 pediatric nursing journals between 2011 and
2013 were collected. Cited journal titles were ranked according to number of times cited and
analyzed according to Bradford’s Law of Scattering and the 80/20 rule to identify the most frequently
cited journals. Five databases were surveyed to assess the coverage of the most-often-cited journals.
The most frequently cited non-journal sources were also identified.

Laboratories are essential to the process of diagnosing and treating an illness in a patient for various reasons. First of all, a patient’s diagnosis process can be a difficult problem and a rather complicated one too. When a doctor listens to a patient describe the symptoms of an illness, he/she gets a general idea of what a patient is suffering from and may need further information from a laboratory to confirm his findings. In such a scenario, the doctor sends the patient to a laboratory either within the institution or outside the institution. The laboratory will most definitely require accurate information about the patient to understand the basic nature of the condition of which the patient is required to be tested on, background information like allergies and any other relevant information. This information is usually given by the doctor or retrieved from data storage facilities like a file or a digital database.
Insurance companies are also essential in the process of treating a patient for various reasons. The major reason however is for the purposes of billing of patients expenses. These companies need information about the expenses incurred by a patient during treatment. Such information may include: laboratory test costs, drugs and medicine costs and doctor consultation fees. Proper communication and data sharing frameworks need to be put in place for this purpose as well.
