2. Thanks for showing your interest in Uber! We have put this guide to help you and make your
TNVS application easier.
In the following slides, we will discuss each document required, and some tips on how to
procure them, fill them up.
Please always remember that regardless of who will be managing the vehicle/s, name on
OR/CR = TNVS Operator Applicant and should be the name indicated on all forms.

*SPA between vehicle owner and Uberpartner is needed IF AND ONLY IF the Uber
partner submits the application in behalf of the vehicle owner. However, vehicle
owner = TNVS applicant and should still be the signatory and document owner for
all requirements.
**All documents submitted for PA submission must be resubmitted online for CPC.

PA.

• Chassis Number = Frame Number = Serial Number
• Do not use MODEL numberof the vehicle because this is not unique and may be
