This document provides a template for a Notarized Special Power of Attorney (SPA) between a vehicle owner and Uber partner. The SPA is only needed if the Uber partner is submitting the application on behalf of the vehicle owner. However, the vehicle owner must still be the signatory and owner of all application requirements. The two page template includes fields for the vehicle owner and partner's names, addresses, signatures, and notarization. Instructions are given to submit the completed SPA and find more information.
