4. Introduction
Virus is a small infectious agent , unicellular , that replicate only inside
the living cells of other organism.
The free independent particle form is known virion.
It consist of three parts : a) Genetic material i.e. DNA or RNA.
b)protein coat called capsid.
c)lipid envelope.
6. Signs &symptoms
After an incubation period of about 5 to 12 days, conjunctival
hyperemia, watery discharge, and ocular irritation usually
begin in one eye and spread rapidly to the other. Follicles may
be present on the palpebral conjunctiva. A preauricular lymph
node is often enlarged and painful. Many patients have had
contact with someone with conjunctivitis, a recent URI, or
both.
8. Who is at risk for herpes infections of
the eyes?
Although a very large percentage of the population (85% or more) carries the
HSV-1 virus, not everyone who carries the virus gets an eye infection.
When a person carrying the virus becomes immunocompromised for
example their immune system becomes weakened due to HIV, medications
(steroids, chemotherapy), age, and perhaps stress, the virus is more likely to
become "active" and incite an outbreak that may include an eye infection.
9. What is the incubation period for
ocular herpes?
The incubation period (the time between acquiring the
virus and the appearance of ocular symptoms) can
range from a few days to several decades.
10. Diagnosis
• Tzank technique:
Slide is prepared from the fluid of fresh bullae shows multinucleated giant epithelial
cells.
• Immune detection: ELISA & PCR.
11. Lab investigation:
Specific tests :
viral culture(Gold std)
Ag detection by ELISA or immunofluorescence
PCR
Serology
Non specific tests:
-cytology
-Electron microscopy
Gimsa stain
Viral culture
12. Treatment
By topical drops or oral drugs:
1. Anti viral medications: works by suppressing the reproduction of
the virus , reducing the no. of active viruses may shorten the
duration and severity of infection.
2. Anti bacterial drops to reduce the chance of secondary
infection.
3. NSAIDS: to relief allergy and itching.
4. Intra ocular pressure can rise as result of inflammation so
pressure lowering medication also will be described.
