• 4 students per cabin
• Cabin groups student selected

Essentials for Parents
• There are 350 spots for students.
• Additional students placed on the
waiting list.
– A random number generator will be used
to determine wait list spots.
• Thursday lunch meetings are
mandatory for students.
– Please pack a lunch since the
doors close at 12:40.
– Attendance is taken, and students with
unexcused absences are moved to the
waiting list
All children will be able to participate on
this field trip with or without a donation.
Children will not be singled out in any way
and donations received will not be
assigned to the child of the adult making
the donation. All donations will go to
support the total costs of the field trip and
will not be applied to costs associated
with your child.

participate; yet insufficient funding will result in a
cancellation of the trip.
More Essentials
• You are responsible for picking up your child
in Yosemite if they are too ill to hike, injured
or they need to be sent home because of a
behavior violation.
• Behavior is all important
– Students are expected to follow all rules outlined
in the Behavior Contract.
• No Electronics - watches & cameras okay
– No cell phone (use calling card if necessary)
– No games, iPods, MP3 or CD players.

• Purple Cover = Checklist
• Entire Packet Due Dec. 13
• White = General Health (Nature Bridge)
– ALL Dietary/Medical issues MUST BE included on this form.
• YELLOW OTC MEDICAL AUTHORIZATION (CUSD)
– Choices – NEEDS PHYSICIAN SIGNATURE/STAMP
– CMS supplies and administers these OTC meds
• BUFF – MEDICAL AUTHORIZATION
– ANY OTHER MEDS NEED to be listed on this form.

Forms and Donations • Parent Essentials Form and any donations will be collected tonight at the end of the meeting, but not before. • If you need to leave early, please drop form off in the office tomorrow. • You can review the narrated meeting slides on the web site. • We will be done by 8:30
  • 4. Support of the Program • To Sustain The Program at Cupertino Middle School we will need to raise through donations, $ 236,250.00 350 students @ $675
  • 5. Donations Cover • These costs are calculated on a per student basis: • • Transportation: $ 67.00 • NatureBridge Program Cost: $ 540.00 • Food (Included in NB cost): $ 0.00 • Admin/Supervision $ 60.00 • Other***: $ 8.00 • ======= • Total Donation: $ 675.00
  • 6. Housing – Curry Village • Curry Village/Boys Town • New Tent Cabins • 4 students per cabin • Cabin groups student selected
  • 7. Housing – Crane Flat • Group dorm-style housing • Boys/Girls • Stay in cabin groups

Essentials for Parents • There are 350 spots for students. • Additional students placed on the waiting list. – A random number generator will be used to determine wait list spots. • Thursday lunch meetings are mandatory for students. – Please pack a lunch since the doors close at 12:40. – Attendance is taken, and students with unexcused absences are moved to the waiting list
  • 9. All children will be able to participate on this field trip with or without a donation. Children will not be singled out in any way and donations received will not be assigned to the child of the adult making the donation.   • 10. Parent Essentials Form Please know that this donation is not required to participate; yet insufficient funding will result in a cancellation of the trip.
  • 11. More Essentials • You are responsible for picking up your child in Yosemite if they are too ill to hike, injured or they need to be sent home because of a behavior violation. • Behavior is all important – Students are expected to follow all rules outlined in the Behavior Contract. • No Electronics - watches & cameras okay – No cell phone (use calling card if necessary) – No games, iPods, MP3 or CD players.

Medical/Dietary Packet • Purple Cover = Checklist • Entire Packet Due Dec. 13 • White = General Health (Nature Bridge) – ALL Dietary/Medical issues MUST BE included on this form. • YELLOW OTC MEDICAL AUTHORIZATION (CUSD) – Choices – NEEDS PHYSICIAN SIGNATURE/STAMP – CMS supplies and administers these OTC meds • BUFF – MEDICAL AUTHORIZATION – ANY OTHER MEDS NEED to be listed on this form.

  1. Do Not use the word Fee. Always use donation. The only item that has to be turned in is the parent essentials form. It is best to talk about turning in a form when discussing “what needs to be done”.
  2. The Yosemite program is only sustainable with the support of voluntary donations. Our program goal for 2014 is $236, 250.00 This is based on the estimated attendance of 350 students and a cost estimate of $675.00 per student. (Adapt for your site’s numbers)As a district we plan to raise $983,475 to support 1457 students.Therefore, we are asking families for a donation of $675.00 to support our trip; however, all children will be able to participate on this field trip with or without a donation. Children will not be singled out in any way and donations received will not be assigned to the child of the adult making the donation. All donations will go to support the total costs of the field trip and will not be applied to costs associated with your child.The Yosemite program is only sustainable with the support of voluntary donations. Please know that this donation is not required to participate; yet insufficient funding will result in a cancellation of the trip. Any donation amount greater than $675 is tax deductible, and we would ask that you are as generous as possible to support this trip for our entire community.

  7. Please take a moment to locate your white parent essentials form. We will be asked you to record your anticipated donation on this form and initial various items through this presentation and turn in this form either at this meeting or by Friday October 4 in the office.On the white Yosemite Parent Essentials Form, we are asking you to record your desired anticipated donation to the program on the first line.Because we have to make initial deposits and payments to NatureBridge and the charter bus company, we ask families to consider making part or all of their voluntary donation in October. Please record any amount that you may be donating in October on the second line.On the third line you can record the remainder of your desired donation that is due by November 8, 2013.A $675 donation covers the full cost, any amount greater is tax deductible and greatly appreciated so that every child in our community that would like to go, can go.Based on the anticipated donations, the district will determine if we have sufficient funds to sustain the program. The decision will be made by October 11, 2013. Donations will only be deposited if it is determined that the program is sustainable. Since this is a donation to the program, there are no refunds . If for some reason your child does not attend then the FULL amount of your donation is tax deductible.All questions about donations or refunds should be sent to the District Yosemite Coordinator: Mary Alarid-Enright or Hans Barber, Director or Science and Math Instruction.Because you will be turning in this form, we have provided a celery version of the form for your records. You should take the celery form home, and turn in the white form if your student plans to attend.
