The Hospice Friendly Hospitals programme aims to make end of life care a priority in healthcare facilities. In 2013, one hospital signed an agreement to join the program for three years. Hospice Friendly Hospitals Programme 2013
The Hospice Friendly Hospitals programme is an initiative of the Irish Hospice
Foundation (in partnership with the HSE) with the purpose of ensuring that
end of life care is central to the mission and every day business of healthcare
sites.
The achievements of the Hospice Friendly Hospitals programme in insert
Hospital during 21013 include:
• The signing of a Memorandum of Understanding between the Irish
Hospice Foundation and insert Hospital committing the Hospital to
membership of the Hospice Friendly Hospitals Programme for the period
2013-2015.
• The appointment of an End of Life Care Coordinator (insert WTE) to
support the implementation of the Quality Standards for End of Life Care
in Acute Hospitals.
• Reconvening of a multidisciplinary, End of Life Care Steering Committee
to oversee the implementation of the Quality Standards for End of Life
Care in Acute Hospitals.
• Completion of a insert [renovation] [capital] project. This project was
funded by insert support of insert local fundraising partner.
• The ongoing delivery of in-service awareness, education and training
sessions on end of life issues, including the Final Journeys Programme
which was attended by insert staff members in 2013.
• Updating of various End of Life Care leaflets and other documents.
These achievements were made possible by the continuing commitment of a
large number of staff, from many disciplines and departments through out the
Hospital, to continually improve the quality of End of life Care for patients and
their families.
