3.  Business case currently being drafted for the
2019 National Service Plan submission.
 This will be brought to Liam Woods on the
26th June when members of the Oversight
Group meet with him.
 Quality Improvement Initiative.

4.  There were 843 deaths in ED recorded in
2016 and this figure is likely to be
underrepresented.
 HFH collaboration with the existing work of
the HSE on unscheduled care to enhance end
of life care.
 In England:
◦ 86% of all admissions in the last year of life are
emergency admissions.
◦ People average around 2.1hospital admissions in
the last year of life – ALOS 27 days

5. Follow up meeting planned with Liam Woods to
update him on the progress of the HSE HFH
