The REHAB@HOME project is an EU-funded collaboration between 9 partners across 4 countries developing an innovative home rehabilitation solution using games and sensors. The project aims to enable people with physical or cognitive impairments to receive high-quality rehabilitation at home for longer than traditional healthcare allows. By integrating software, hardware, medical data and social networks, the system transforms the home into an engaging rehabilitation space with remote support from specialists. Initial achievements include a low-cost user-friendly system to support long-term programs and monitor patients while involving family members.

4. The Consortium
We are an international EC funded R&D project, comprised of a consortium of 9
partners from 4 different countries - Germany, Italy, Austria & Israel, which aims
to provide innovative & effective Home-Rehabilitation solution

5. ✚High quality Home-rehabilitation is essential at regaining many,
if not all, capabilities to lead a meaningful, fulfilling and
productive life.
✚The aim is to enable both physically & cognitively challenged
people to enjoy high quality home-rehabilitation for a longer
period than the health systems can currently provide.
Objectives of the project

6. ✚The idea is to transform the patient's home into a place where
physical and cognitive rehabilitation processes can be
performed in an intensive, engaging and professionally guided
way, while promoting social inclusion and quality of life.
✚By integrating state-of-the art software and hardware
components and devices along with professional medical data
into a personalized serious-game based connected with
Web2.0 social networks (Health 2.0).
