Ivabrad (Generic Ivabradine Hydrochloride Tablets)

Ivabrad (Generic Ivabradine Hydrochloride
Tablets) is used to treat adults who have chronic
heart failure to reduce their risk of hospitalization
for worsening heart failure.
It is also used to treat heart failure in children 6
months of age and older who have stable heart
failure, with symptoms, caused by an enlarged
heart (dilated cardiomyopathy).

Active Ingredient
The active ingredient contained in Ivabrad
tablets is Ivabradine Hydrochloride.
Each film-coated Ivabrad tablet contains
Ivabradine Hydrochloride equivalent to 5 mg
or 7.5 mg of Ivabradine.

Ivabradine Hydrochloride is available
under various Drug trade names that
include Corlanor, Procoralan, Coralan,
Corlentor, Ivabrad, Ivabeat, Inapure,
Bradia, Ivanode and Ivazine.

Uses of Ivabrad Tablets
Ivabrad (Generic Ivabradine Hydrochloride Tablets) is used for the
treatment of the following indications:
Coronary Artery Disease: Ivabrad (Generic Ivabradine
Hydrochloride tablets) is used for the treatment of chronic stable
angina pectoris in patients with normal sinus rhythm who have a
contraindication or intolerance for Beta-blockers.
Ivabradine Hydrochloride tablets are also used for the treatment of
coronary artery disease patients with normal sinus rhythm,
indicated in combination with beta-blockers in patients inadequately
controlled with an optimal beta-blocker dose and whose heart rate
is equal to or more than 70 bpm.

Uses of Ivabrad Tablets
Ivabrad (Generic Ivabradine Hydrochloride tablets) is used for
the treatment of the following indications:
Treatment of Chronic Heart Failure: Ivabradine Hydrochloride
tablets are also indicated in patients with CHF New York Heart
Association (NYHA) II to IV class with systolic dysfunction, in
patients in sinus rhythm and whose heart rate is greater than 75
bpm, in combination with standard therapy including beta-blocker
therapy.

Ivabrad Tablets Dosage
The dosage of Ivabrad tablets (Generic Ivabradine
Hydrochloride Tablets) is given below:
Tablets must be taken orally twice daily, i.e. once in the morning
and once in the evening during meals.
The starting dose is 5 mg twice daily. After 2 weeks of treatment,
adjust dose based on heart rate. The maximum dose is 7.5 mg
twice daily.
In patients with conduction defects or in whom bradycardia
could lead to hemodynamic compromise, initiate dosing at 2.5
mg (i.e. one half 5 mg tablet) twice daily.

Ivabrad Tablets Contraindications
Ivabrad tablets are contraindicated in patients with a
hypersensitivity to Ivabradine Hydrochloride or any other
component of this medication.
The other contraindications are Resting heart rate less than 70
bpm prior to treatment, Cardiogenic shock, Acute myocardial
infarction, Severe hypotension (less than 90/50 mmHg),
Severe hepatic insufficiency, Sick sinus syndrome, Sino-atrial
block, Unstable or acute heart failure, Pacemaker dependent
(heart rate imposed exclusively by the pacemaker), Unstable
angina and AV-block of third degree.

Ivabrad Tablets Contraindications
The other contraindications of Ivabrad (Generic Ivabradine
Hydrochloride Tablets) are: Combination with strong cytochrome
P450 3A4 inhibitors such as azole antifungals (Ketoconazole,
Itraconazole), macrolide antibiotics (Clarithromycin, Erythromycin
Per Os, Josamycin, Telithromycin), HIV protease inhibitors
(Nelfinavir, Ritonavir) and Nefazodone is contraindicated.
Combination with Verapamil or Diltiazem which are moderate
CYP3A4 inhibitors with heart rate reducing properties is
contraindicated.
Pregnancy and lactation and women of child-bearing potential not
using appropriate contraceptive measures.

Storage Instructions
Ivabrad (Generic Ivabradine Hydrochloride
Tablets) have to be stored at controlled room
temperature i.e. from 20°C to 25°C (68°F to
77°F) with excursions permitted between 15°
to 30°C (59° to 86°F).
Keep this as well as all other medicines away
from children and pets.

Warnings
Monitor heart rate decreases and bradycardia symptoms during treatment.
Cardiac Arrhythmias: Not recommended in patients with atrial fibrillation or
other cardiac arrhythmias that interfere with sinus node function.
2nd degree AV block: Not recommended in patients with 2nd degree AV
block.
Low Heart Rate: Must not be used in patients with pre-treatment resting
heart rate below 60 bpm.
Combination with Calcium Channel Blockers: Concomitant use of
Ivabradine with heart rate reducing calcium channel blockers is not
recommended.
Chronic Heart Failure: Caution in heart failure patients with NYHA
functional classification IV due to limited data.
Stroke: Not recommended immediately after stroke.

Warnings
Visual Function: Discontinuation should be considered if
deterioration in visual function occurs unexpectedly. Use with caution
in patients with retinitis pigmentosa.
Excipients: Contains lactose, should not be used in patients with
galactose intolerance, Lapp lactase deficiency or glucose-
galactose malabsorption.
Mild to Moderate Hypotension: Use with caution.
Atrial Fibrillation/ Cardiac arrhythmias: Monitor patients for atrial
fibrillation. If non urgent DC-cardioversion is required, leave 24 hours
between the last dose of Ivabradine and the procedure.
Patients with Congenital QT Syndrome or Treated with QT Prolonging
Medicinal Products: Avoid.

Warnings
Hypertensive Patients Requiring Changes to Antihypertensive
Treatment: Monitor blood pressure at an appropriate
interval after modifications.
Moderate Hepatic Insufficiency: Use with caution.
Severe Renal Insufficiency: Use with caution.
Fetal Toxicity: Females should use effective contraception.

Effects on Ability to Drive and Use Machines
A specific study to assess the possible influence of Ivabradine Hydrochloride
(Ivabrad Tablets) on driving performance has been performed in healthy
volunteers where no alteration of the driving performance was evidenced.
However, in post-marketing experience, cases of impaired driving ability due
to visual symptoms have been reported. Ivabradine may cause transient
luminous phenomena consisting mainly of phosphenes.
The possible occurrence of such luminous phenomena should be taken into
account when driving or using machines in situations where sudden
variations in light intensity may occur, especially when driving at night.
Ivabradine has no influence on the ability to use machines

Ivabrad Tablets Side Effects
Most common Ivabradine Hydrochloride (Ivabrad tablets)
side effects occurring in 1% of patients or greater are
bradycardia, hypertension, blood pressure increased ,
atrial fibrillation and luminous phenomena (phosphenes,
visual brightness).
The following side effects have been identified during
post-approval use of Ivabradine Hydrochloride tablets:
syncope, hypotension, angioedema, erythema, rash,
pruritus, urticaria, vertigo, diplopia, and visual
impairment.

Ivabrad Tablets Overdose
Ivabradine Hydrochloride (Ivabrad tablets) overdose may lead to
severe and prolonged bradycardia. In the event of bradycardia with
poor hemodynamic tolerance, temporary cardiac pacing may be
required.
Supportive treatment, including intravenous (IV) fluids, atropine,
and intravenous beta-stimulating agents such as isoproterenol, may
be considered.

Ivabrad Tablets During Pregnancy
Ivabradine Hydrochloride (Ivabrad tablets) has been classified by
the US FDA as Pregnancy Category D.
Ivabrad may cause fetal toxicity when administered to a pregnant
woman, based on findings in controlled animal studies. Doctors must
advise female patients of childbearing potential to use effective
contraception.
Embryo-fetal toxicity and cardiac teratogenic effects were observed
in fetuses of pregnant rats treated during organogenesis at exposures
1 to 3 times the human exposures at the maximum recommended
human dose. The potential risk for humans is unknown. Therefore,
Ivabradine is contraindicated during pregnancy.
