Envas Tablets (Generic Enalapril Maleate Tablets)

Envas tablets (Generic Enalapril Maleate tablets) are used
alone or in combination with other antihypertensive agents,
for the treatment of hypertension and for the treatment of
symptomatic congestive heart failure, usually in combination
with diuretics and digitalis.
This medicine is also used for the treatment of symptomatic
congestive heart failure, usually in combination with diuretics
and digitalis. In these patients Enalapril Maleate tablets
improve symptoms, increase survival, and decrease the
frequency of hospitalization.

Active Ingredient
The active ingredient present in Envas tablets is Enalapril
Maleate. Each Envas tablet contains 2.5 mg, 5 mg or 10 mg of
Enalapril Maleate.

include Vasotec, Envas, Canapril, Dilvas,
Enam, Enamate, Enapril, Enpril, Envas,
Hytrol, Invoril, Lupinace, Normace,
Nuril, Oren and Viviril.

Uses of Envas Tablets
Envas tablets (Generic Enalapril Maleate tablets) manufactured by
Cadila Pharmaceuticals Limited, India is used alone or together with
other medicines to treat high blood pressure (hypertension), to treat
heart failure (weakening of heart function) and to prevent signs of heart
failure.
High blood pressure adds to the workload of the heart and arteries. If it
continues for a long time, the heart and arteries may not function
properly. This can damage the blood vessels of the brain, heart, and
kidneys, resulting in a stroke, heart failure, or kidney failure. Lowering
blood pressure can reduce the risk of strokes and heart attacks.

Uses of Envas Tablets
Enalapril Maleate (Envas Tablets) is an angiotensin converting enzyme
(ACE) inhibitor. It works by blocking a substance in the body that causes
the blood vessels to tighten. As a result, Enalapril relaxes the blood vessels.
This lowers blood pressure and increases the supply of blood and oxygen
to the heart.
Enalapril is also used in combination with other medicines to treat
congestive heart failure and cut down on the number of hospital visits for
heart problems.

Envas Tablets Dosage
The dosage of Envas tablets (Generic Enalapril Maleate
Tablets) is given below:
Hypertension In Adults: The recommended starting dose in adults
is 5 mg once a day. Titrate upward to maximum of 40 mg daily as
required to help achieve blood pressure goals. The dose can be
divided and administered twice daily in case the antihypertensive
effect reduces at the end of the dosing interval.
Heart Failure: The recommended starting adult dose is 2.5 mg
twice a day titrated up to a maximum of 20 mg twice a day, as
tolerated. Doses are normally given in combination with diuretics
and digitalis.

Envas Tablets Dosage
The dosage of Envas tablets (Generic Enalapril
Maleate Tablets) is given below:
Asymptomatic Left Ventricular Dysfunction: The
recommended starting dose is 2.5 mg twice a day titrated
up to a maximum of 10 mg twice a day, as tolerated.
Diuretic dose may be required to be adjusted.

Envas Tablets Contraindications
Envas tablets are contraindicated in persons with a
hypersensitivity (allergy) to Enalapril Maleate or any of the
other ingredients of this medicine.
It is also contraindcated in in patients with a history of
angioedema related to previous treatment with an angiotensin
converting enzyme inhibitor and in patients with hereditary or
idiopathic angioedema. Do not co-administer Aliskiren along
with this medication in patients with diabetes.

Storage Instructions
Envas Tablets (Generic Enalapril Maleate
Tablets) have to be stored at controlled room
temperature i.e. from 20°C to 25°C (68°F to
77°F) with excursions permitted between 15°
to 30°C (59° to 86°F).
Keep this as well as all other medicines away
from children and pets.

Warnings
Aortic Stenosis/Hypertrophic Cardiomyopathy: As with all vasodilators, Envas
tablets should be given with caution to patients with obstruction in the outflow tract
of the left ventricle.
Impaired Renal Function: Assess renal function: Monitor renal function in patients
treated with Enalapril Maleate tablets. Changes in renal function including acute
renal failure can be caused by drugs that inhibit the renin-angiotensin system.
Hyperkalemia: Elevated serum potassium (greater than 5.7 mEq/L) was observed in
approximately one percent patients in clinical trials. In most cases these were
isolated values which resolved despite continued therapy.
Risk factors for the development of hyperkalemia include renal insufficiency,
diabetes mellitus, and the concomitant use of potassiumsparing diuretics, potassium
supplements and/or potassium-containing salt substitutes, which should be used
cautiously, if at all.

Warnings
Angioedema: Angioedema, including laryngeal edema, may occur at any
time during treatment with angiotensin converting enzyme inhibitors,
including Enalapril tablets. Patients must be cautioned to to report
immediately any signs or symptoms suggesting angioedema (swelling of
face, extremities, eyes, lips, tongue, difficulty in swallowing or breathing).
Hypotension: Patients must be cautioned to report lightheadedness,
especially during the first few days of therapy. In case actual syncope
occurs, the patients should be told to discontinue the drug until they have
consulted with the prescribing physician.
Hyperkalemia: Patients should be informed not to use salt substitutes
containing potassium without consulting their doctor.

Warnings
Neutropenia: Patients should be told to report promptly any indication of
infection (e.g., sore throat, fever) which may be a sign of neutropenia.
Hepatic Failure: Rarely, ACE inhibitors have been associated with a
syndrome that starts with cholestatic jaundice and progresses to fulminant
hepatic necrosis, and (sometimes) death.
Fetal Toxicity: Use of medicines that act on the renin-angiotensin system
during the second and third trimesters of pregnancy reduces fetal renal
function and increases fetal and neonatal morbidity and death. Resulting
oligohydramnios can be associated with fetal lung hypoplasia and skeletal
deformations. Potential neonatal adverse effects include skull hypoplasia,
anuria, hypotension, renal failure, and death. When pregnancy is detected,
discontinue Enalapril Maleate tablets as soon as possible.

Side Effects
The most common Enalapril Maleate (Envas Tablets) side effects for
patients treated for hypertension with an incidence rate of 3% or more
was fatigue. The other common side effects were Orthostatic Effects,
Asthenia, Diarrhea, Nausea, Headache, Dizziness, Cough, Rash.
The most common side effects of Enalapril Maleate Tablets for patients
treated for heart failure with an incidence rate of more than 6% were
hypotension and dizziness. Other common side effects were Orthostatic
Effects, Syncope, Chest Pain, Fatigue, Abdominal Pain, Asthenia,
Orthostatic Hypotension, Angina Pectoris, Myocardial Infarction, Diarrhea,
Nausea, Vomiting, Headache, Vertigo, Cough, Bronchitis, Dyspnea,
Pneumonia and Rash.

Envas Tablets Overdose
The most prominent Enalapril Maleate overdose symptoms reported to
date are marked hypotension, beginning some six hours after ingestion of
tablets, concomitant with blockade of the renin-angiotensin-aldosterone
system, and stupor. Symptoms associated with overdosage of ACE
inhibitors may include circulatory shock, electrolyte disturbances, renal
failure, hyperventilation, tachycardia, palpitations, bradycardia, dizziness,
anxiety and cough.

Treatment of Envas Tablets Overdose
The recommended treatment of Enalapril Maleate (Envas Tablets)
overdosage is intravenous infusion of normal saline solution. If
hypotension occurs, the patient should be placed in the shock position. If
available, treatment with angiotensin II infusion and/or intravenous
catecholamines may also be considered. If ingestion is recent, take
measures aimed at eliminating Enalapril maleate (e.g., emesis, gastric
lavage, administration of absorbents, and sodium sulphate). Enalapril
may be removed from the general circulation by haemodialysis.
Pacemaker therapy is indicated for therapy-resistant bradycardia. Vital
signs, serum electrolytes and creatinine concentrations should be
monitored continuously.

Envas Tablets During Pregnancy
Enalapril Maleate (Envas Tablets) has been classified by the US
FDA as Pregnancy Category D.
Use of medicines that act on the renin-angiotensin system during the
second and third trimesters of pregnancy reduces fetal renal function and
increases fetal and neonatal morbidity and death.
Resulting oligohydramnios can be associated with fetal lung hypoplasia
and skeletal deformations. Potential neonatal adverse effects include skull
hypoplasia, anuria, hypotension, renal failure, and death. When
pregnancy is detected, discontinue Enalapril Maleate as soon as possible.
Medicines that act directly on the renin-angiotensin system can cause
injury and death to the developing fetus.
