This lesson explains the process of creating the appropriate case file to document your professional counseling case.<br /> This lesson is created as refresher training for the BC3 course offered by the LWBCC.<br /> The content of this lesson is provided to support student education purposes and does not reflect legal or counseling advice. <br />Introduction<br />

  4. 4. In order to meet the LWBCC requirement to create a consistent and professional counseling experience for the counselee and the counselor, LWBCC has adopted standards for developing case files.<br />Why is this lesson important?<br />
  5. 5. Case files can be considered legal documents for several reasons, and it is our goal to prepare you to document your counseling sessions and counseling process as thoroughly and completely as possible.<br />Why is this lesson important?<br />
  6. 6. There are several principles that govern case file development including:<br />Principle One:<br /> Case files are arranged to focus on the forms.<br />The case file is divided into sections which contain all similar forms in session order.<br />Case File Essentials: Principles<br />

  9. 9. Counseling Agendas
  10. 10. Homework Assignments
  11. 11. Weekly Roundtable review forms

  13. 13. All release forms
  14. 14. Counselor Notes and Observations

Principle Four:<br /> If you are sharing information out of a case file for any reason, you must have a signed release form and/or expressed written permission from the counselee. <br /> You may:<br /><ul><li>Provide the inquirer with a copy of the document however, you must keep the original LWBCC documents in the case file
  19. 19. All file content sharing must be approved by the LWBCC Lead Counselor prior to disclosure (including Emergency Session and/or referral situations). Please see HIPAA documentation on information sharing directly with the counselee.</li></ul>Case File Essentials: Principles<br />

There are several types of case files we use in Biblical Counseling. <br /> In Becoming an Effective Biblical Counselor, we describe the following case file types:<br />Single case file<br />Dual case file<br />Emergency Session Case file<br />Pre-Marital Case file<br />Types of Case Files<br />
