Introduction Community of Practice – PARTNERS Core objective – to achieve integrated records of care across and between health service providers in the acute primary and continuing care sector for persons aged over 65 Develop a summary discharge assessment tool for persons over 65 (for specific use when patients care is transferred between differing health care teams) – Prototype assessment tool created Create a short documentary focusing on the challenges that members of the COP experience in their differing roles which have a direct impact on patient safety and to identify how ICT may help ✔ 4
6. Objective To describe how this COP engaged with the authors to produce a documentary. Documentary Aim: to promote the benefits of developing shared health care records addressing ‘real life’ patient health issues. 6

Challenges Numerous practitioners in different health care environments what is core what is unique to each of their roles and practice environments (delicate balance maintaining respect without attributing blame to one service group) Participatory approach all inclusive however, stakeholders have no background in media production – practitioners not actors! Sensitive issues, sensitive context, focusing on a vulnerable client group 11

Reflections Decisions on selection of particular footage was sometimes difficult – real life context was important Fostering continued empowerment of practitioners whilst being sensitive to the topic was critical to the programme Ensuring that the final product is a useful resource for the intended audience was also key. 12
17. Conclusion Case studies are powerful mediums to deliver learning particularly in a visual representation such as a documentary (Barab & Duffy, 2000) Witnessing the ‘real thing, in the real context, with the real people’ (Beetham & Sharpe, 2007) 15
18. BNS 2011 – Student Reflection 16 ‘…..The film highlighted the needs of nurses in different care agencies looking after older adults in order to ensure optimal care and a good quality of life. Through the use of a common assessment form that is transferrable through these agencies the communication of pertinent information is achieved i.e. medication management – changes of medication identified and initiated so care is seamless’.
