New appointments to be made, new doctor(s) to trust, new processes to navigate. Most 18 year olds thrusted into adult care have no clue how to navigate their transition from pediatric to adult care, nor are parents prepared to entrust their child with the management of care they have perfected over the years. As winners of the Fall 2016 Medicine X Youth Changemakers Workshop at Stanford University, we present our mobile platform: Maia. Maia is a new pediatric centered platform to help young adults slowly acquire the necessary skills to be the best stewards of their health. With the ongoing mentorship of 5 pediatric physicians, the creation of Maia will engage teens with an enticing technological platform, breaking the invisible barrier that prevents a healthy dialogue between patients and providers and ease a parents’s minds. Every child will be ready for the responsibility their own care.
