Valley Mountain Regional Center – Stockton, California 2007 to Present
A community-based, non-profit agency of the Department of Developmental Services serving Californians with developmental disabilities.
Intake Coordinator
Recruited to assess prospective consumers three years-old and older and coordinate the intake process.
Interview and screen prospective consumers (children and adults with developmental disabilities) in a five-county region which
have been referred to the Regional Center to determine eligibility for services.
Conduct and document intake assessments while observing the intake process to ensure compliance with statutory regulations.
Gather information, attend court and arrange diagnostic evaluations to determine eligibility for potential clients.
Assist consumers and their families in identifying service needs and developing service plans.
Ensure free diagnosis and assessment services are available to any person suspected of having a developmental disability, such
as intellectual disability, cerebral palsy, epilepsy, autism, or dual diagnoses.
Complete Client Development Evaluation Reports (CDER) and administer “Measures of Adaptive Behavior.”
Consistently achieve on-time completion of assignments related to the California Lanterman Act.
Utilize health care data analysis for descriptive, predictive, and prescriptive purposes.
Participate in ongoing clinical training on developmental disabilities, mental health issues, and interdisciplinary meetings.
Conduct intake assessment meetings with inmates at the County jail and attend court hearings for assigned cases.
Assigned to conduct Preadmission Screening and Resident Review (PASRR) of local nursing home residents.
Ensure thorough documentation of each resident in the SANDIS/ Interdisciplinary notes.
Fort Wayne State Developmental Center – Fort Wayne, Indiana 2000 to 2007
A state institution operated by a private company to provide services for people with severe disabilities and behavioral problems.
Shift Administrator (2005 to 2007)
Served as a Qualified Intellectual Development Professional to oversee the entire facility during the evening shift.
Provided leadership and direction to a team of 8 Supervisors and 50 direct care staff members working with individuals with
mental health issues and developmental disabilities.
Ensured smooth and orderly facility operations and delivery of quality care for 60 – 80 residents during the evening shift.
Ensured compliance with company policies and served as the designated facility representative with outside agencies.
Directed professional development training on emergency behavior restraint to over 100 direct care staff and supervisors.
