Serving the home care needs of the Southcoast Visiting Nurse Association (VNA) for both their hospice and infusion patients.
Working with a team of 16 professionals, delivered the highest possible standards of clinical care, education and support, within all USP
797 guidelines, with a particular focus on customized medication therapy management for patients with complex infusion needs;
outpatient/ambulatory adult TPN, vancomycin and aminoglycoside therapies, IVIG, SCIG, Remicade, Lumizyme, and catheter care.
Provided DURs/evaluations, dosing instructions and clinical support for the nursing staff for pain management medications and
compounded, sterile intravenous products, in addition to expert input regarding self-monitoring, lab monitoring where necessary, and
medication adherence.
Maintained all best practice process and workflow SOPs, attended bi-weekly Inter-Disciplinary Team hospice meetings, and
supported the training of nursing and pharmacy staff on the correct and safe use of home infusion products and machines, as
