The Optimum Pharmacy system improves the level of patient care by providing up-to-date

The Optimum Clinicals Electronic Medication Administration Record (eMAR) with barcode scanning is
available for assisting the user during the medication administration process and provides electronic
documentation of the administration. eMAR provides a safer, more streamlined medication administration
process and includes a series of automated patient safety checkpoints that provide system validation of
the five rights of medication administration and supports the user to ensure that the right patient receives
the right medication, at the right dose by the right route at the right time.

Optimum Assessments provide a quick and easy way for the clinical staff to obtain an assessment unique to the patient's needs.
Assessments can be built in a range from simple to complex. One assessment might simply record basic data and
be very brief, whereas another may encompass several pages and also trigger off other assessments or flow sheets,
care plans, or orders (if one or more data points a re answered in a specified fashion).

OptimumCareplans are created to outline the care to be provided to individual patients and to
guide in the ongoing delivery of care to assist in the evaluationof thatcare.OptimumCareplans
provide information to all health care providers involved in the care of the patient. The ultimate
goal of the Optimumcareplan is to provide consistency of care across time to ensure an optimal
outcome for the individual patient.
Care Plan - The care plan focuses on patient-specific nursing outcomes that are realistic for the
care recipient that support the medical diagnosis and the patient's condition. This may include an
elaborate description with defining characteristics (subjective and objective data) that support the
diagnosis/condition or simply the name of the care plan.
Problems - Problems are attached to care plans and include a statement of the nursing
diagnosis or patient problems. Problems may also include patient strengths as well as individual,
family, or community experiences/responses to actual or potential health problems/life processes.
Outcomes - Outcomes include the goal that the patient is striving to meet. Outcomes are
attached to problems to indicate the goal of the specific interventions that were put into place to
solve the problems within the care plan.
Interventions - Interventions are activities performed by the health care providers that assist the
patient in meeting their goal (outcome).
