3. I. Giới thiệu
• As per World Health Organization, “a drug-drug
interaction (DDI) is a situation in which a substance
(usually another drug) affects the activity of a drug
when both are administered together.” It can also be
defined as “the modification of the effects of one drug
(i.e., the object drug) by the prior or concomitant
administration of another drug”

13. Ý nghĩa của tương tác thuốc có lợi
• Tăng tác dụng của thuốc.
Ví dụ: Phối hợp thuốc kháng sinh Trimethoprim + Sulfamethoxazol
(Bactrim) làm tăng tác dụng lên 4-100 lần của Sulfamethoxazol đơn
thuần.
• Làm giảm tác dụng không mong muốn của thuốc điều trị.
Ví dụ: Thuốc PPI sử dụng chung với thuốc kháng tiểu cầu giảm
tác dụng bất lợi của thuốc trên dạ dày.
• Để giải độc thuốc.
Ví dụ: Naloxone đối kháng tác dụng bất lợi của Morphin
14. Tương tác thuốc có hại
The American Food and Drug Administration define a serious adverse
event as one when the patient outcome is one of the following:
• Death
• Life-threatening
• Hospitalization (initial or prolonged)
• Disability— significant, persistent, or permanent change, impairment, damage or disruption in the patient’s body
function/structure, physical activities, or quality of life.
• Congenital anomaly.
• Requires intervention to prevent permanent impairment or damage

Aspirin đẩy các thuốc nhóm sulfonylurea khỏi protein liên kết
trong huyết tương

Alogliptin
Common adverse reactions (reported in ≥4% of patients
treated with alogliptin 25 mg and more frequently than in
patients who received placebo) are nasopharyngitis,
headache, and upper respiratory tract infection.

Rifampicin decreased linagliptin exposure suggesting that the efficacy of linagliptin may be
reduced when administered in combination with a strong P-glycoprotein or CYP3A4 inducer.
Therefore, use of alternative treatments is strongly recommended when linagliptin is to be
administered with a P-glycoprotein or CYP3A4 inducer.

Low potential for interactions with co-administered medicinal products. Not a CYP 450
enzyme substrate and does not inhibit or induce CYP 450 enzymes, hence not likely to
interact with active substances that are substrates, inhibitors or inducers of these enzymes.
Hypoglycemic effect may be reduced with thiazides, corticosteroids, thyroid products
and sympathomimetics
There was a slight increase in the area under the curve (AUC, 11%) and mean peak drug
concentration (Cmax, 18%) of digoxin with concomitant 100 mg sitagliptin for 10 days.
Patients receiving digoxin should be monitored appropriately. No dosage adjustment of
digoxin or sitagliptin is recommended.
Ketoconazole significantly increased saxagliptin exposure. Similar significant increases in
plasma concentrations of saxagliptin are anticipated with other strong CYP3A4/5 inhibitors.
The dose of saxagliptin should be limited to 2.5 mg when co-administered with a strong
CYP3A4/5 inhibitor. Using CYP3A4 inducers may reduce the glycemic lowering effect of
saxagliptin.

Background
The co-administration of dipeptidyl peptidase 4 (DPP-4) inhibitors
and angiotensin-converting enzyme (ACE) inhibitors may lead to
sympathetic tone stimulation and to increased cardiovascular (CV)
adverse events due to adverse haemodynamic effects [1-4].
Alogliptin is a selective DPP-4 inhibitor for the treatment of patients
with type 2 diabetes mellitus (T2DM) that was not inferior to placebo
in terms of CV safety in T2DM patients at high CV risk [5-7]. This
analysis of the EXAMINE study looks into the clinical outcome of the
combination of ACE inhibition and DPP-4 inhibition with alogliptin, by
comparing the CV outcomes in patients receiving ACE inhibitors at
baseline, between those receiving alogliptin vs. placebo.

(1). Nhóm thuốc DPP 4 là một nhóm thuốc an toàn và
hiệu quả trong điều trị bệnh lý ĐTĐ.
(2). Tương tác thuốc giữa nhóm thuốc DPP 4 và các
nhóm thuốc khác ít có ý nghĩa lâm sàng.
(3). Nhóm thuốc DPP 4 không ảnh hưởng đến kết cuộc
điều trị bệnh lý tim mạch đồng mắc trong phối hợp
