Medical Benefits
Effective January 1, 2015 your medical benefits will be provided through Blue Cross/Blue Shield. You will be offered a $2,600/$5,200 plan and a
$5,000/$10,000 plan. A summary of each of these plans is included here for your review.
Basic Life/AD&D
Meadows & Ohly provides eligible employees with a Basic Life and Accidental Death and Dismemberment benefit through Principal at no cost to you.
This benefit is equal to $30,000.
Enrollment Procedures
The following steps will guide you through the enrollment process:
1Carefully review the plan information in this benefit enrollment guide
and all other plan materials included in your enrollment packet. The
insurance carriers’ web sites also provide important information and
tools that can help you make enrollment decisions.
2Consider the needs of any dependents you may have. If you are
married, review the coverage currently offered through your
spouse’s employer to avoid costly duplicate coverage.
3All enrollment paperwork must be returned to Human Resources
on the designated due date.
EligibilityBenefits are available to full-time employees working a minimum of
30 hours per week. Benefits are effective after 60 days of employment.
Also eligible for coverage are your legal spouse and/or children to the
age of 26.
Changing your Coverage
Make your benefit elections carefully, including the choice to waive
coverage. Your pretax elections will remain in effect until the next
annual open enrollment period unless you experience an IRS approved
qualifying change in status.Qualifying change in status events include, but are not limited to:
• Marriage, divorce, or legal separation
• Death of spouse or other dependent
• Birth or adoption of a child
• A spouse loses coverage under another employer group medical health plan
• A spouse becomes eligible for coverage under another employer
group medical health plan
• A dependent’s eligibility status changes due to age
• You or your spouse are covered under a group health plan and
experience a change in work hours
• Relocation into or outside of your plan’s service area

Dental Benefits
You and your family have the opportunity to enroll in one of the two
dental insurance plans provided by Principal.
The Principal dental plans are like PPO plans, which include a national
network of preferred providers. Unlike a medical PPO
plan, dental
PPO
plans have the same benefit coverage both in and out of network.
You can maximize your benefits by using an in network dentist since
these providers charge a discounted rate for services and do not charge
you for amounts in excess of UCR. In addition, in network providers
will file claims on behalf of members.

vision plan through EyeMed. Vision care offers you and your family
a benefit that covers all routine eye care, including eye exams and

Short Term Disability
Bennett Thrasher provides eligible employees with a Short TermDisability benefit through UNUM and pays the full cost. This planwill provide you with a weekly income benefit equal to 60% of yourweekly earnings up to a maximum of $1,000 per week in the eventof a non work related sickness or injury. There is a 7 day waitingperiod following an accident or illness before benefits are payable.
Long Term Disability
Bennett Thrasher also provides eligible employees with a Long TermDisability benefit through UNUM. This benefit is employee paid. Inthe event that you become disabled from a non-work related injuryor sickness, long term disability income benefits are provided as asource of income. After 90 days of being disabled, employees whoenroll are eligible for this benefit. This benefit pays 60%of monthly earnings to a maximum of $8,500 per month.
Voluntary Life
Bennett Thrasher provides eligible employees with the option ofpurchasing additional life insurance on themselves and/or theirdependents. This program is provided through UNUM.
An employee who wants to supplement their basic life benefit maypurchase additional coverage. The schedule of benefits is below:
• Employee: $10,000 increments up to 5x annual salary to amaximum of $500,000, whichever is less• Spouse: $5,000 increments up to 100% of employee amount orto a maximum of $250,000, whichever is less• Child(ren): 6 months and older - $2,000 increments to $10,000not to exceed 100% of employee amount; 0-6 months old - $1,000• Guarantee Issues: Employees$100,000;Spouse$25,000;Child$10,000
The cost of the Voluntary Life and ADD coverage for you and yourspouse will be based on current age, tobacco use, and the amount ofcoverage you elect.
Employee Assistance Program (EAP)This plan provides all employees with a work-life balance program,which includes free confidential, short-term counseling throughtelephone consultations, in addition to web-based tools on-line. Youmay contact The EAP at www.lifebalance.net or 1.800.854.1446.
Flexible Spending Account (FSA)
This plan offers you a way to reduce your taxable income while at thesame time pay for medical and dependent daycare expenses. You canuse pre-tax dollars toward eligible expenses. The annual maximumallowed for Medical is $2,500 and the maximum for dependentcare is $5,000. Please remember that you can now carry over upto $500 of your unused Medical FSA elected funds to help pay forqualified expenses that occur in 2014.
Health Savings Account (HSA)
The HSA allows you to save pre-tax dollars for eligible healthexpenses. A tax deferred savings account helps you save toward med-ical expenses at retirement. In 2014 you can save $3,300 individual,$6,550 family, and $1,000 catch-up if over age 55 ($2,000 if bothspouses are age 55 or older). In addition to what you save, the Firmmay contribute to your HSA.

Dental Benefits
You and your family have the opportunity to enroll in one of the two
dental insurance plans provided by Guardian.
The Guardian dental plans are like PPO plans, which include a nation-
al network of preferred providers. Unlike a medical PPO plan, dental
PPO plans have the same benefit coverage both in and out of network.
You can maximize your benefits by using an in network dentist since
these providers charge a discounted rate for services and do not charge
you for amounts in excess of UCR. In addition, in network providers
will file claims on behalf of members.
Enrollment Procedures
The following steps will guide you through the enrollment process:
1Carefully review the plan information in this benefit enrollment guide
and all other plan materials included in your enrollment packet. The
insurance carriers’ web sites also provide important information and
tools that can help you make enrollment decisions.
2Consider the needs of any dependents you may have. If you are
married, review the coverage currently offered through your
spouse’s employer to avoid costly duplicate coverage.
3You must register for your ADP account and complete online
enrollment or waiver of benefits within 30 days of hire for new
benefits eligible employees or 30 days of a Qualifying Event. Any
applicable hard copy forms must be returned to Human Resources
on designated due dates.
Eligibility
Benefits are available to full-time employees working a minimum of 30
hours per week. Benefits are effective on the date of hire. Also eligible
for coverage are your legal spouse and / or children to age
26 or disabled dependents.
Changing your Coverage
Make your benefit elections carefully, including the choice to waive
coverage. Your pretax elections will remain in effect until the next
annual open enrollment period unless you experience an IRS approved
qualifying change in status.
Qualifying change in status events include, but are not limited to:
• Marriage, divorce, or legal separation
• Death of spouse or other dependent
• Birth or adoption of a child
• A spouse loses coverage under another employer group medical health plan
• A spouse becomes eligible for coverage under another employer
group medical health plan
• A dependent’s eligibility status changes due to age
• You or your spouse are covered under a group health plan and
experience a change in work hours
• Relocation into or outside of your plan’s service area

Bennett Thrasher provides employees with the option of enrolling in a
vision plan through EyeMed. Vision care offers you and your fam-
ily a benefit that covers all routine eye care, including eye exams and
eyeglasses (lenses  frames) or contacts.

Short Term Disability
Meadows  Ohly provides eligible employees with a Short Term
Disability benefit through Principal and pays the full cost. This plan
will provide you with a weekly income benefit equal to 60% of your
weekly earnings up to a maximum of $1,000 per week in the event of
a non-work related sickness or injury. There is a 14 day waiting
period following an accident or illness before benefits are payable.
Long Term Disability
Meadows  Ohly also provides eligible employees with a Long Term
Disability benefit through Principal. This benefit is employee paid.
In the event that you become disabled from a non-work related injury
or sickness, long term disability income benefits are provided as a
source of income. After 90 days of being disabled, employees who
enroll are eligible for this benefit. This benefit pays 60% of monthly
earnings to a maximum of $6,000 per month.
Voluntary Life
Meadows  Ohly provides eligible employees with the option of
purchasing additional life insurance on themselves and/or their
dependents. This program is provided through Principal.
An employee who wants to supplement their basic life benefit may
purchase additional coverage. The schedule of benefits is below:
• Employee: $10,000 increments up to 5x annual salary to a
maximum of $500,000, whichever is less
• Spouse: $5,000 increments up to 100% of employee amount or
to a maximum of $250,000, whichever is less
• Child(ren): 15 days and older - $5,000 increments to $10,000
not to exceed 100% of employee amount; birth to 14 days - $1,000
• Guarantee Issues: Employees$100,000;Spouse$25,000;Child$10,000
The cost of the Voluntary Life and ADD coverage for you and your
spouse will be based on current age, tobacco use, and the amount of
coverage you elect.
Employee Assistance Program (EAP)
This plan provides all employees with a work-life balance program,
which includes free confidential, short-term counseling through
telephone consultations, in addition to web-based tools online. You
may contact The EAP at www.principal.com or 1.800.986.3343 .
Flexible Spending Account (FSA)
This plan offers you a way to reduce your taxable income while at the
same time pay for medical and dependent daycare expenses. You can
use pre-tax dollars toward eligible expenses. The annual maximum
allowed for Medical is $2,400 and the maximum for dependent care
is $5,000.
Health Savings Account (HSA)
The HSA allows you to save pre-tax dollars for eligible health
expenses. A tax deferred savings account helps you save toward
medical expenses at retirement. In 2015 you can save $3,350
individual, $6,650 family, and $1,000 catch-up if over age 55
($2,000 if both spouses are age 55 or older). In addition to what
you save, the Firm may contribute to your HSA.

You will be offered a $2,600/$5,200 plan and a $5,000/$10,000 plan. A summary of each of these plans is included here for your review. Basic Life/AD&D Meadows & Ohly provides eligible employees with a Basic Life and Accidental Death and Dismemberment benefit through Principal at no cost to you. This benefit is equal to $30,000. Enrollment Procedures The following steps will guide you through the enrollment process: 1Carefully review the plan information in this benefit enrollment guide and all other plan materials included in your enrollment packet. The insurance carriers’ web sites also provide important information and tools that can help you make enrollment decisions. 2Consider the needs of any dependents you may have. If you are married, review the coverage currently offered through your spouse’s employer to avoid costly duplicate coverage. 3All enrollment paperwork must be returned to Human Resources on the designated due date. EligibilityBenefits are available to full-time employees working a minimum of 30 hours per week. Benefits are effective after 60 days of employment.
