OBJECTIVES:
Concept paper
1.
• Write a clear and succinct purpose
statement.
• Your purpose statement should be direct,
clear, and detailed.
• Show your knowledge of the organization
you belong in the introduction.
2.
• Let the administration know that you understand the
types of projects and the vision-mission of the
institution.
• Provide the administration that illustrates alignment
between your goals and the school’s ideals.
• Describe exactly your procedures for
accomplishing the task.
3.
• Explain what you need from the organization and
why.
• Including anticipated budgets, estimated costs of
operation and upstart, equipment, and supplies
for the implementation.
• Time line- indicating your anticipated date of
completion. (E.g. Programme)
4.
• Relate to the administration the connections
between your ideas, your plans and your
expected outcomes.
• By linking thoughts to actions to results, you
illustrate to the organization that you have
thought out the project from start to finish and
have not overlooked any important details.
5.
• Emphasize the benefits of the
seminar/training/workshop.
• End your proposal by revisiting the benefits to the
participants or to the school.
• Giving a more in-depth explanation of reasons the
project will be beneficial to the organization, will help
to reinforce the positive outcomes mentioned in the
introduction.
1. The heading shall bear the school logo
where the proponent belongs.
2. If the proponent belongs to the two schools, there
should be a separate heading/concept paper for MCNP and ISAP.
3. Along with the heading, the proponent must
indicate the office, department or club he/she
represents.

4. The date of the implementation must be
reflected.
5. Submission of the concept paper must be a
month prior to the date of implementation to
give enough time for the administrators to
review and for the proponent/s/organizers to
prepare for the activity.
6.The following shall be the specifications of
the Concept Paper:
 Title of the Activity - All Caps and Bold
 Font - Tahoma
 Font size - 12
 Margins - 0.5” (Top, Bottom, Left, Right)
 Spacing - Single Space
 Bond Paper Size - 8.5” x 13” (Long)
 Page Numbers - Bottom right of the paper
7. The Rationale must compose of at
least two paragraphs and must not exceed 200
words. It should reflect an overview of the
activity and provide a justification for its conduct.
8. The objectives must be based on the school’s
VISION, MISSION, and CORE VALUES.
9. Activity guidelines shall compose of the
following stages and shall be stated in narrative form:
Pre-planning stage shall include the dates and procedure of pre-
conferences with the core group, working committees and
consultation with focal persons and administration.
Implementation stage shall contain the guidelines, steps,
procedures as to the actual implementation of the activity.
Evaluation stage shall include the dates and procedure of post
conferences with the core group, working committees, focal
persons and administration.
10. The following shall be the
attachments:
Logistics- this shall be presented in matrix form
Transportation- this shall be presented in matrix form
The logistics shall contain the signature of the head, budget and
finance and VP for Admin and Finance and date it was signed.
Upon approval, a copy of the logistics shall be forwarded to the
office and personnel concerned (Accounting office, Registrar,
Supply officer and General Services).
10. 2. The sample programme and the lay out of the invitation shall
be attached. Once approved, the proponent has to validate the availability of
the administrators involved in the program from the Admin secretary.
10.2.a Colored invitation shall only be given to the President,
Executive Vice President and the Guest/s.
10.2.b Black and White invitation printed in corona bond
paper shall be distributed to the different offices and other
members of the administration.
10.3 Necessary forms which will be utilized in the activity
must be attached
along with the concept paper.
10.3.a Registration Form shall be known as FORM R
10.3.b Registration Form shall contain the page number which should be
reflected in the bottom right and the form number indicating the month,
year, and the number of the form in the upper right corner of the paper.
The adviser must affix his/her signature just below the name of the last
person signed in the registration form.
10.3.c Workshop hand outs must also be attached
10.4 Letters must be attached
10.4.a Excused letter for the student participants must be
attached along with the concept paper with the note or remarks
of the Vice President for Academic Affairs.
10.4.b Letter of Invitation to the speaker/guest must be attached
with the signature of the proponent/s and the VPAA.
10.5 The draft of the layout of the Certificates both
for the speaker and participants must be attached
10.5.a The signatories in the certificates shall include the
proponent/organizer/adviser and the President
10.5.b Once approved, the proponent/organizer/adviser must
sign at the back of the certificate for control and monitoring
purposes during the signing of clearances.
10.5.c The Registration form shall be attached with the
Certificates to be signed by the President for validation purposes.
10.6 The Working Committees shall be identified with
corresponding job description and must be presented in Matrix format.
The following shall be the Standing Working Committee:
10.6.a Program and Invitation
10.6.b Physical Preparation and After Care
10.6.c Welcome, Reception, and Nourishment
10.6.d Security and Discipline
10.6.e Audio/Sounds
10.6.f Awards and Tokens if necessary
10.6.g Eucharistic if necessary
10.6 The Working Committees shall be identified with
corresponding job description and must be presented in Matrix format.
The following shall be the Standing Working Committee:
10.6.a Program and Invitation
10.6.b Physical Preparation and After Care
10.6.c Welcome, Reception, and Nourishment
10.6.d Security and Discipline
10.6.e Audio/Sounds
10.6.f Awards and Tokens if necessary

10.7 The Evaluation Tools shall be known
as:
10.7.a Evaluation Form for Seminars/Lectures/Workshops
– FORM E1 12-001
10.7.b Evaluation Form for School Wide Activities
– FORM E2 12-001
11. The approved original copy shall be filed in the
proponent’s office, while a photocopy shall be filed with the admin.
Secretary.
12. After completion of the activity, the narrative report (1 page) plus
summary of Evaluation Tool no later than 5 days following the activity.
The narrative report shall contain a summary of the activity including
the positive and negative points. Said report shall be attached to the
concept paper for future reference.
13. Non compliance with items no. 11 and 12 would result to non-
issuance (or non- signing) of the proponent’s clearance for the
semester.

The narrative report shall contain a summary of the activity including the positive and negative points. Said report shall be attached to the concept paper for future reference. 13. Non compliance with items no. 11 and 12 would result to non- issuance (or non- signing) of the proponent’s clearance for the semester.
