Home Visitation Form The MOV presented is a
sample of a Home Visitation
Form fully accomplished by
the student, student’s
parent and the class adviser
with the signature of
Approving Authorities of the
school.
It has been my practice to
home visit my learners’
homes in case there are
some issues that the
parents need to know
regarding their child. This
makes the conference more
private and personal.
Calling the parents to come
to school may make the
child feel ashamed so it may
only bring negative results.
In this home visit form, I
was able to assist my
learner who is at risk of
dropping out when I visited
his home. I also learned
about his family and some
of their issues so I was able
to give due solutions to the
problem of the child. With
home visitations, I was able
to solve conflicts in a subtle
way. Hence, the learners
feel more at ease in my
classroom.
