ACC Outreach Specialist
 Conductsclient outreach to schedule assessment appointments. Facilitatesclient engagement via
phone calls, letters and arranging transportation as needed.
 Responsible for maintaining Care Coordinator schedules including assessment and follow up
appointments for all Care Coordinators in the regional area.
 Acts as a conduit for coordination and communication with welfare and other social services
agencies including DYFS
 Monitors clients with eligibility problems and implements procedures regarding same.
 Participates in special projects assigned by the supervisors.
 Performs other clerical functions including but not limited to answering phones and filing.
 Contacts treatment providers to inquire about slots, beds or space availability as needed.
 Advocates with welfare and DYFS as needed to meet client needs.
 Contacts treatment providers to obtain BHI/SAI client attendance and enters the date in the data
base.
 Processes referrals
 Verifies eligibility and employability status, enters the Med1 dates in Atlantis documenting all
information.
 Generates CWRRF’s for clients who are referred but not assessed and closes cases as
appropriate.
 Enters urine drug screen results for clients in Extended Evaluations and creates a PA for these
services.
 Assists CC’s in the administrative management of client files. Including CSR attempts.
1/2005-9/2010 Hackensack University Medical Center, Hackensack, NJ
Patient Placement Bedcontrol Rep.
 Executes bed management functions on multiple hospital computer systems to assign beds to
incoming admission in accordance with the policies, procedures, protocols, medical board
rulings, philosophy of department and the medical center.
 Prioritizes admission sources (cath lab, ETD, PACU) to minimize delays and enhance patient
flow.
 Prepares and distributes bed status report to executive staff, senior and medical leadership,
department directors, etc... Identifying bed availability.
 Performs ongoing assessment in anticipation of patient needs based on volume or specialty
requirements, isolation, organ transplants, etc…, to minimize the need for blocked beds and
allowing for the proper use of private rooms.
 Reviews and expedites all in house transferrequests in accordance with established policies in
following priority: medical (includes level of care ICU, intermediate, isolation, observation
management, etc.), bed status, patient and family request.
 Is the medical center’s initial contact for transfers from other facilities. Obtains all required
information and communicates with physicians, families, the manager and/or vice president for
capacity management in a timely manner.
 In the absence of the vice president for capacity management,is the medical centers contact for
families of patients awaiting transfer. Communicates as needed with the vice president of
capacity management.
 On a daily basis, interacts with members of the medical staff as well as physician offices in
processing patients for direct admission. Obtains all required information and communicates
with the manager/vice president of capacity management in a timely manner.
 Identifies criteria for medical examiners cases in accordance with state law, verifies reporting
and coordinates autopsy scheduling with Bergen County Medical Examiner’s office.
 Update and revise accommodation codes based on changes in level of care for billing purposes.
 Verifying and pre-certification for all transfers from other facilities and incoming pre-
admissions.
