The Transition Nurse Specialist's (TNS) is a blended advance practice nurse role aimed at
improving transitions of care for high risk patients and eliminate gaps in the continuum of
care. Responsible for coordinating a comprehensive program of social work and case
management services in the department of care coordination for high risk patients. TNS
integrates concepts derived from case management, communication, care coordination
theory, and nurse educator spheres of influence to ensure a network of providers and
community resource to assist patients and/or family to optimize their level of self-efficacy.
Utilize proven quality improvement frameworks to identify and redesign care transition
models to improve patient and family outcomes and organizational goals such as reduce
readmission rates. Collaborate and cultivates collegial relationships with intrer-professional
team on designated units and/or within specific disease populations. TNS provides
clinically informative variables to identify high risk patients who have a modifiable risk by
conducting:
• In depth Interview for modifiable risk & intervention planning:
o Inter professional collaboration for patient centered discharge plan
o Patient/family/caregiver education – ‘Teach Back’
o Schedule follow-up appointments prior to discharge with primary care provider and
specialists within 7 day post discharge
o Identify appropriate additional interventions
o Review discharge instructions with patient – time out for accuracy & teach back
o Connect with PCP/community clinic
o Warm hand off/post discharge availability to providers
• Follow-up calls post discharge within 48 -72 hours & as needed
• Provide feedback to PCP, Specialists, and other care providers
• Accessible to patient, Home Health, Skilled Nursing Facility, and other inter
professional team members
• Data collection & Analysis
Participate in the development and implementation of guidelines, evidence based practice,
pathways and other process and performance improvement activities. Assist in the
development/writing of protocols; designing assessment tools; preparing abstracts,
presentations, and reports. TNS also participate in interdepartmental and inter-facility
committees, community agencies, and professional associations. Additionally, currently
managing automated post-discharge call system for UCSD Health, monitoring responses
and addressing identified issues.
11/09 – 01/13 Post-Liver Transplant and Hepatology Coordinator, Division of Organ Transplant,
Scripps Green Hospital, La Jolla, CA
Responsible for the planning, coordination, and case management of all aspects of patient
care for the throughout the continuum of service—from initial referral to follow-up care for
hepatology and immediate post-operative through long term post-liver transplant outpatient
follow-up. Secondarily responsible for the same roles in nephrology and post-kidney and/or
pancreas patient populations. Serve as the primary case management coordinator for the
interdisciplinary team, including: physicians, nurses, social workers, dieticians,
pharmacists, financial analysts, case managers and support staff. Develop, coordinate,
implement and evaluate educational programs and projects for patients, personnel and the
community. Collect and document data to meet clinical, financial and regulatory
requirements. Participated in the development of guidelines, protocols, pathways and
other performance improvement activities. Participate in research activities. Serve as a
patient advocate throughout the patient care process. An additional component of this
position includes providing direct patient care as the clinic nurse for the unit procedure
room, including activities such as medication administration (PO, IM, SQ, and IV), prepping
and assisting with minor procedures and biopsies, and post-procedure direct patient
evaluation and monitoring until discharge from procedure room.

ostomy or wound care, nutritional supplementation, IV administration, and specimen
collection). Provide education to promote self or caregiver management of long-term
chronic or new medical diagnosis; including, hypertension, diabetes, congestive heart
failure, and chronic obstructive pulmonary disease. Maximize patient’s rehab-ability and
make referrals for necessary durable medical equipment and appropriate evaluation by

patients throughout the continuum of service—from initial referral to follow-up care for a
large pancreatic hepatobiliary and surgical oncology practice, as well as live liver donor
program patients. Served as the primary coordinator for the interdisciplinary team,
including: physicians, nurses, social workers, dieticians, pharmacists, financial analysts,
case managers and support staff. Developed, coordinated, implemented and evaluated

Utilize proven quality improvement frameworks to identify and redesign care transition models to improve patient and family outcomes and organizational goals such as reduce readmission rates. Collaborate and cultivates collegial relationships with intrer-professional team on designated units and/or within specific disease populations. TNS provides clinically informative variables to identify high risk patients who have a modifiable risk by conducting: • In depth Interview for modifiable risk & intervention planning: o Inter professional collaboration for patient centered discharge plan o Patient/family/caregiver education – ‘Teach Back’ o Schedule follow-up appointments prior to discharge with primary care provider and specialists within 7 day post discharge o Identify appropriate additional interventions o Review discharge instructions with patient – time out for accuracy & teach back o Connect with PCP/community clinic o Warm hand off/post discharge availability to providers • Follow-up calls post discharge within 48 -72 hours & as needed • Provide feedback to PCP, Specialists, and other care providers • Accessible to patient, Home Health, Skilled Nursing Facility, and other inter professional team members • Data collection & Analysis Participate in the development and implementation of guidelines, evidence based practice, pathways and other process and performance improvement activities. Assist in the development/writing of protocols; designing assessment tools; preparing abstracts, presentations, and reports. TNS also participate in interdepartmental and inter-facility committees, community agencies, and professional associations. Additionally, currently managing automated post-discharge call system for UCSD Health, monitoring responses and addressing identified issues. 11/09 – 01/13 Post-Liver Transplant and Hepatology Coordinator, Division of Organ Transplant, Scripps Green Hospital, La Jolla, CA Responsible for the planning, coordination, and case management of all aspects of patient care for the throughout the continuum of service—from initial referral to follow-up care for hepatology and immediate post-operative through long term post-liver transplant outpatient follow-up. Secondarily responsible for the same roles in nephrology and post-kidney and/or pancreas patient populations. Serve as the primary case management coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers and support staff. Develop, coordinate, implement and evaluate educational programs and projects for patients, personnel and the community. Collect and document data to meet clinical, financial and regulatory requirements. Participated in the development of guidelines, protocols, pathways and other performance improvement activities. Participate in research activities. Serve as a patient advocate throughout the patient care process. An additional component of this position includes providing direct patient care as the clinic nurse for the unit procedure room, including activities such as medication administration (PO, IM, SQ, and IV), prepping and assisting with minor procedures and biopsies, and post-procedure direct patient evaluation and monitoring until discharge from procedure room.

Facilitate direct patient care and clinical interventions as deemed necessary (such as, ostomy or wound care, nutritional supplementation, IV administration, and specimen collection). Provide education to promote self or caregiver management of long-term chronic or new medical diagnosis; including, hypertension, diabetes, congestive heart failure, and chronic obstructive pulmonary disease. Maximize patient’s rehab-ability and make referrals for necessary durable medical equipment and appropriate evaluation by supportive services. Serve as a patient advocate throughout the process and communication liaison with physicians overseeing and ordering patient’s direct home care. 09/04 – 02/09 Clincal HBS Program and Live-Transplant Coordinator, Hepatobiliary Surgery and Live Liver Donation, Center for Liver Disease and Transplantation (CLDT), Cedars-Sinai Medical Center, Los Angeles, CA Responsible for the planning, coordination, and case manager of all aspects of care for the patients throughout the continuum of service—from initial referral to follow-up care for a large pancreatic hepatobiliary and surgical oncology practice, as well as live liver donor program patients. Served as the primary coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers and support staff. Developed, coordinated, implemented and evaluated educational programs and projects for patients, personnel and the community. Component of position also included taking 24-hour Liver Transplant Call for organ procurement, transplant surgeries, and first line patient issues for all patients under the care of the CLDT (including Liver Transplant, Hepatology, and Hepatobiliary Surgery). 10/01 – 02/09 Clinical Program Coordinator, Neuroendocrine Tumor Center, Cedars-Sinai Medical Center, Los Angeles, CA Coordinated the development of a new program focused on providing comprehensive, cutting edge, multi-disciplinary care for individuals suffering from neuroendocrine tumors. Responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of service—from initial referral to life long follow-up care. Served as the primary coordinator and case manager for the interdisciplinary team, including: endocrinologist, oncologist, surgeons, pathologist, radiologist, nurses, and support staff.
