2.  Admitting patients on to your ward or clinical
area is your opportunity to get to know a
patient and should always be conducted in a
warm and friendly manner. It should never
be forgotten that you are representing your
organisation and your conduct should be
professional at all times.

3.  This is a term used in health care to refer to
the self-care activities that people are able
or unable to perform; ADLs are our
functional status in key areas of ‘living’
 The ADLs also incorporate biological,
psychological, sociocultural, environmental
and politico-economic factors.

 The nursing process is a
dynamic and on-going
cyclical process (never
static) that enables the
nurse to plan
individualised nursing
care for the patient. This
planning is initiated from
the information gathered
from the nursing
admission.

6. INFORMATION
While completing this documentation, you may
have come across some things you may not
be familiar with, such as the meaning of
some of the abbreviations.
 DNACPR this means do not resuscitate
(specifi cally, do not attempt cardiopulmonary
resuscitation)
 NOK this means next of kin

7.  AVPU this means Alert, Verbal, Pain or
Unresponsive and is a neurological
assessment (this will be explained in
Chapter 10)
 GCS this is the Glasgow Coma Scale
(or Score), a neurological assessment
tool
 EWS this is the Early Warning Score
(this will be explained in Chapter 10)

8. You may also have noticed that it is not
just the admission documentation that
needs to be completed, but also other
paperwork, such as:
• infection control,
• venous thrombolic embolism,
• falls,
• pressure ulcer (skin bundles),
• malnutrition screening tool.
