Dear Mr. Wes Larson,
Puget Sound Construction, Inc. (PSC) is pleased to present this proposal for the Spyglass Hill
Apartments located in Bremerton, WA. The Spyglass Hill Apartments, perched above Highland
Ave overlooking the Puget Sound, will be a great addition to the surrounding community. PSC
has worked with the project architect, Rice Fergus Miller on many successful projects and sees
the Spyglass Hill Apartment development as an excellent opportunity for the three parties to
collaborate for the first time. PSC has worked on numerous similar projects in the Puget Sound
region and understands the complexities of creating value with infill developments.
The attached proposal was prepared with the knowledge and experience of the PSC team, with
input from our preferred and trusted specialty subcontractors. PSC specializes in 5-over-2 type
construction projects; therefore we are able to provide a skilled and reliable workforce to self-
perform the majority of the structure of the building. In order to ensure the quality of work
meets the PSC standard, we plans to self-perform structural concrete, structural steel and wood
framing, sheathing, siding, as well as door and window installation. Our total estimated project
cost including all markups and a 5% fee is: Guaranteed Maximum Price (GMP): $10,745,427.00
(Excluding WSST). Given the anticipated start date of June 15th
, 2015, PSC’s scheduled
completion date for the Spyglass Hill Apartments is July 29th
, 2016. The total project duration is
295 working days, which is roughly 13.5 calendar months.

Standard Form of Agreement Between Owner and Contractor where the basis of payment is
the Cost of Work Plus a Fee with a Guaranteed Maximum Price
AGREEMENT made as of the twenty first day of May in the year two thousand fifteen.
BETWEEN the Owner:

and the Contractor:
Mr. Aegir Olsen, Puget Sound Construction, Inc., 4334 Waterfront Ave. Bremerton, WA 98337

Article 4.1 Date of commencement will be approximately June 15th

Permits, (b) Verification of financing, (c) Notice to proceed, (d) Signed and executed contract
Article 4.3 The contractor shall achieve Substantial Completion of the entire Work not later than
two hundred ninety five (295) work days from the date of commencement.

Article 5.1.1 The Contractor’s Fee: 5% of the estimated cost of the Work. Which is a lump sum
of $537,271.00 .
Article 5.1.2 The method of adjustment of the Contractor’s Fee for changes in the Work: In the
case of approved changes in the work, the contractor’s fee will be adjusted as follows: 5% for
increases in scope and 0% for decreases in scope.
Article 5.2.1 The Contract Sum is guaranteed by the Contractor not to exceed ten million seven
hundred forty five thousand four hundred and twenty seven Dolars ($10,745,427.00 ) If the
actual cost of Work plus the contractor’s fee is less than the estimated cost of the Work as
adjusted by approved change orders, the resultant savings will be distributed as follows: 80% to
the Owner and 20% to the Contractor.

Article 7.2.2 Wages or salaries of the Contractor’s supervisory and administrative personnel
when stationed at the site with the Owner’s approval. Including contractor’s project manager.
Article 12.1.3 Provided that an Application for Payment is received by the Architect not later
than the thirtieth (30th
) day of a month, the Owner shall make payment of the certified amount to
the Contractor not later than the tenth (10th
) day of the following month. If an Application for
Payment is received by the Architect after the application date fixed above, payment shall be
made by the Owner not later than thirty (30) days after the Architect receives the Application for
Payment.
Article 12.1.7.3 Add the Contractor’s Fee, less retention of five percent (5.0%).
Article 12.1.7.4 Subtract retention of five percent (5.0%).
Article 12.1.8 Except with the Owner’s prior approval, payments to Subcontractors shall be
subject to retention of not less than five percent (5.0%).
Article 12.2.4 The Owner’s final payment to the Contractor shall be made no later than 30 days
after the issuance of the Architect’s final Certificate of Payment, or as follows: Final release of
retention is predicated upon completion of all close-out activities including submission of full
and unconditional lien releases from the general contractor and all second tier subcontractors and
suppliers.
Article 13.2 Arbitration pursuant to Section 15.4 of AIA Document A201-2007
Article 15.2 Payments due and unpaid shall bear an interest rate of 1% monthly.

Roles and Responsibilities
Project Manager – Aegir Olsen
As the Project Manager, Aegir Olsen will be the main liaison between Sound West Group and
Puget Sound Construction, Inc. Aegir will represent PSC in owner meetings and will be
responsible for generating payment applications for the owner and the payment of
subcontractors. Aegir will work closely with the Superintendent to make sure the project in
constructed according to the plans and specifications and PSC’s standards. Aegir will be in
charge of the buyout process to make sure work is packaged effectively and efficiently. Aegir
will work closely with subcontractors Project Managers from the time of contract issuance to
closeout documentation.
Superintendent – Jim Street
As the Superintendent, Jim Street will be in charge of the day-to-day operations of the
construction site. Jim will help Aegir modify the schedule to keep it up to date as the project
progresses. While PSC is self-performing structural concrete, structural steel, rough framing and
siding work, Jim will manage the crew foremen. Jim will also monitor production rates of the
direct labor and will make adjustments to the construction schedule as needed. Jim will conduct
weekly meetings with the craft workers to outline what needs to happen in that particular week.
Jim will conduct new personnel training every time a new craftsman enters the jobsite. Jim will
maintain a safety log and will report and injuries or near misses to the Project Manager so it can
be properly recorded. Jim will mange the subcontractors on site while they are performing the
work to make sure it conforms to the plans and specifications as well as PSC’s standards. Jim
will manage the equipment operators, material laydown areas, material deliveries and all hoisting
operations. Last but not least, Jim will be responsible for maintaining a clean and safe working
environment for all of the crews.
Project Engineer – Travis Davidson
As the Project Engineer, Travis Davidson will help Aegir make sure all necessary paperwork is
completed in a timely manner. Travis will be in charge of processing RFI’s, reviewing
submittals, maintaining the submittal log, helping coordinate show drawings, and helping
manage the self-perform packages of work. In the early stages of the construction schedule he
will also help buy out early work packages including the elevator, structural steel, doors frames
and hardware, and glazing. Travis will help with ordering material and tracking orders to make
sure they arrive on time without delaying the schedule. It will also be Travis’ responsibility to
help with MEP coordination.

Architect – Robert McNamara
During the construction phase, RFM will review and reply to RFI’s, review and approve
submittals, and will monitor the construction progress to make sure it is being completed to the
original design intent. If there are any scope changes, RFM will update the drawings and
specifications and review and approve change orders if they come up.
Owner – Mr. Wes Larson
The owner Wes Larson, or another member of Sound West Group will be representing the owner
during the construction of the Spyglass Hill Apartments. SWG will be responsibly for having
proper financing in place before construction begins. Sound West Group will be responsible for
reviewing and approving all payment requests. It will be SWG’s responsibility to then provide
payment for the completed work in accordance to the schedule of values with the architect’s
approval. The owner will be responsible for ordering, delivering and installation of all
furnishings, patio and outdoor equipment. The owner will be responsible for scheduling the
proper commissioning authority. It will be the responsibility of the owner to pay all utility bills,
parking lot rental, and provide aerial rights for the tower crane.

Construction Sequence Narrative
Phase 1: Preconstruction/ Initial Site work
The site will be handed over from the owner with demo, clearing and grading completed as well
as a fence around it. Once NTP is received the GC will mobilize to the site and set up TESC and
perform other preliminary site work including but not limited to a survey and layout. Once
utilities are located and removed, the structural excavation sub will mob to the site and begin the
excavation process for the apartment building. There is plenty of room for their equipment to be
left onsite overnight. The excavation will be sloped and or benched on all four sides. The dump
truck will be loaded with the haul off material via Highland Ave. The backfill material will be
left on site near Highland Ave until the PT is poured and the CIP walls can be back filled. During
excavation the retaining walls on the NE corner of the site can be formed, reinforced, Poured and
finished.
Phase 2: Substructure/Misc. Site work
Once excavation is finished and the building footprint is graded, excavation for the footings can
begin. Carpenters will build the forms, ironworkers will tie rebar and laborers will place the
footings. The same will then be done for the foundation walls and the CIP walls on the basement
level. Then the elevator pit will be backfilled and the MEP subs can begin under slab rough in.
Once the electricians and plumbers are finished roughing in under the slab, vapor barrier will be
placed and the SOG will be placed. The slab should be poured on a Friday so it can cure over the
weekend and formwork for the PT deck and begin to be formed by a subcontractor the following
Monday. MEP subcontractors will insert any sleeves or block-outs while ironworkers lay
reinforcing and a specialty subcontractor places PT tendons. The slab will be broken into two
sections North and South as indicated on the plans. Each section will be poured on separate days.
The slabs will then be tensioned and days later the pour strip will be poured. Once the PT Slab is
poured the exterior of the foundation walls can be applied with waterproofing, insulation and can
be backfilled. Once backfill is completed perimeter drains can be installed.
Phase 3: Superstructure
Once the PT Deck is poured, cured and stressed. Carpenters and ironworkers will begin wood
and steel framing, respectively, on level 1. Once all of the walls are framed, joists and floor
sheathing will be installed on level 2. This process will repeat up to the fifth level. The elevator
pent house, mechanical room, parapet walls will then be framed by carpenters. The Roof canopy
will then be erected by steel erectors and sheathed by carpenters. The roof will be ready for
insulation, waterproofing and roofing. MEP Subcontractors will begin installing initial MEP
rough-ins as the carpenters frame the level above them. Upon the completion of the framing on
the fifth floor the MEP subcontractors will then work their way back down through the floors as
the finish trades follow them. The elevator will begin installation once the roof is completely
finished, the elevator shaft walls are completed and inspected, main power to the building is
connected and the switchgear is operable. Once the elevator is installed the inside will be
covered in plywood and used as a man lift.

Once the roof construction starts the scaffolding subcontractor will begin erecting scaffolding
around the perimeter of the building. The scaffolding will then be used to wrap the building with
vapor barrier. Carpenters will then install Windows and a glazing subcontractor will install the
storefront. While the windows are being installed the building will be heated if necessary to dry
it out in preparation of loading it with GWB. Carpenters will then install the FRC siding per
elevation starting in the North West corner on level 1 of the building near the elevator and
working clockwise around the building.
Phase 4: Interior Finishes
Once MEP rough-ins are completed on the fifth floor and an inspector has approved the work,
the walls and ceiling will be insulated and carpenters will hang the GWB. The parade of trades
will begin in the North West portion of the building where the elevator is located and work
clockwise around the floor. Following the GWB hanging crew will be the GWB tape, finish and
texture crew. Following the GWB finish crew the walls and ceiling will get painted. Once the
walls and ceilings are painted, flooring in the kitchens and bathrooms will be installed. Casework
will then be installed in the kitchens and bathrooms. Once the casework is transported into each
of the units the doors can begin to be hung. Carpenters will then install door, window trim and
base. Once the door and window trim is installed, tiling in the kitchen and bathroom can begin.
The painter will then come back and paint the trim. Once painting is finished MEP trim can be
installed and carpet tile and sheet carpet can be installed. Following the completion of all finishes
in units, appliances will be installed.
Phase 5: Exterior Finishes
Once the siding is installed the exterior can be masked and painted by the painting subcontractor.
The prefabricated concrete patios can then be flow in with the help of the crane and bolted by
crews on the scaffolding. Once the decks are bolted on, the railing system and glazing panels can
be installed. Upon completion of the roof, the concrete pavers, planter boxes, and pedestal pavers
can be installed. The landscaping subcontractor can then finish all of the work associated with
the roof top deck planters.
Phase 6: Miscellaneous
The footings for the canopy at the entrance of the building will be excavated, formed, reinforced
and poured upon completion backfilling the CIP wall. Once the footings are complete the SOG
will be formed and poured. Then the steel columns will be erected and the roof will be framed
and sheathed by carpenters. Lightweight concrete roof decks will be poured on level 3 and 4
after topping out and while MEP rough in is occurring. Once the exterior enclosure of the
building in finished, the scaffolding will be dismantled. The site-work subcontractor will then
pour all curbs and sidewalks and pave the parking lots. The landscaping subcontractor will also
be landscaping at this time. Upon completion of all finishes on the interior of the building a
punch list will be performed and a final clean will occur.

Management Commitment
Committed to providing a safe workplace for all employees, PSC has developed a program for
injury prevention that encourages managers, supervisors, and employees to identify and
eliminate hazards that may develop during construction.
General Overview
The Spyglass Hill Apartment building is comprised of 73 residential units atop one level of
below grade parking on a 3.2 acre plot of land in Bremerton Washington. Each of the 6 levels is
approximately 15,000 sf. The residential portion of the project is comprised of wood framing
with some structural steel for support. The below grade parking portion of the project is all cast
in place concrete with an elevated post-tensioned deck. NTP and Substantial Completion are
expected in June 2015 and August 2016, respectively, for a total duration of 14 months.

Adverse Weather Conditions
Bremerton is located in the Pacific Northwest of the United States where temperatures tend to
fluctuate substantially between seasons. During the duration of the project both hot and cold
conditions are to be expected. Adequate dress, appropriate preparation, and access to conditioned
spaces are recommended precautions to help reduce weather related accidents.
Working in Cold Conditions
Potential injuries:
Hypothermia- when a workers body looses heat faster than it can produce heat
causing a dangerously lower body temperature. Wearing the proper PPE such as a warm hat, rain
jacket, and rain pants can prevent injury by keeping the worker dry and protected from the
elements.
Muscle strain or tear- when a worker strains or tears a muscle because their
muscles are not warm and flexible. Wearing the proper PPE such as a warm hat, rain jacket, and
rain pants can prevent injury by keeping the worker warm and flexible.
Working in Hot Conditions
Potential injuries:
Heat stroke- is a life threatening condition often associated with symptoms of
high body temperature, rapid pulse, difficulty breathing, and confusion. It is the most severe
heat-related injury, after heat cramps and heat exhaustion. Wearing the proper PPE, taking
adequate breaks, eating and drinking adequate amounts can prevent such an injury.
Emergency Procedures
1) Any injured worker in need of immediate medical attention should not be left unattended.
Stay with the injured worker while dialing 911. A primary contact individual should tend
to the injured worker until an ambulance arrives. When possible, a trained and qualified
person should assist the injured. A trained and qualified individual will be certified in
CPR, First Aid, Hazmat (in case of exposure to bodily fluids) and the use of External
Defibrillators.
2) All injuries, regardless of severity, should be reported to one of the primary contacts on
the first page.
3) Witnesses of an accident should debrief with the site supervisor and create a
comprehensive report of the incident. This report will be submitted to the necessary
authorities including but not limited to OSHA and Washington State’s Department of
Labor and Industries.
If an injured worker does not need immediate medical attention, a member from the primary
contact list has the authority to drive him/her to the nearest hospital.

Evacuation Procedure & Meeting Point
In the event of an evacuation of the premises due to an earthquake, flooding, gas leak, fire or
other major incident refer to the evacuation diagram shown below. As shown, meet at the
intersection of Highland Ave and the main jobsite entrance.
At the meeting point, a competent person from the list of primary contacts will take attendance.
To ensure all workers are accounted for, compare the attendance list administered at the meeting
point with crew lists made earlier in the day. No one is allowed to re-enter the building, even in
the case of missing personnel. Everyone must wait for the local fire department or qualified
officials to arrive and determine whether or not it is safe to return to work. Under no
circumstances can someone return to work without the consent of a competent person from the
primary contact list.
Subcontractor Requirements
All subcontractors are required to have a site-specific accident prevention plan approved by the
superintendent. Subcontractors are required to have an orientation with all of their new workers
about their APP before they can start working.
Pre-work Safety Orientation
All new workers to the job site will review and perform the following before being allowed to
work on the Spyglass Hill Apartments:
• Review the PPE checklist
• Review the Evacuation Plan
• Review known jobsite hazards
• Review fall protection plan
• Review confined space plan
• Discuss site specific electrical safety concerns
• Make each worker aware of the emergency contacts

Required Personal Protective Equipment
In order to enter the job site, every single person must satisfy PPE requirements including but not
limited to:
• Hardhat
• Steel-toed boots
• Safety Vest
• Ear protection
• Safety goggles
• Safety gloves
• Long pants
Fall Protection Plan
The Spyglass Hill Apartment building after top out will be 6 stories above grade. During
construction there will be many workers exposed to falling hazards and will be required to wear
fall protection. When possible, all areas with fall hazard potential will have guard railing.
During rough framing all window openings will need a 2x4 railing installed. Fall protection in
the form of a harness and tie off rope will be required for the following situations:
o When working on scaffolding greater than 10 feet off the ground
o When working within ten feet of the edge of the roof
o When working within ten feet of a stair shaft, elevator shaft, or vent shaft hole
before the Shaft Wall is installed.
Before any worker can start working, a competent person must walk the site to determine if there
are any new fall hazards. If any new fall hazards are identified, a Competent Person will mark
them and determine appropriate fall protection is for each instance. The most common forms of
fall protection that will be used on the Spyglass Hill Apartments are tie-offs, body harness, with
fall limiting lanyard, and guard railings where necessary. At a minimum, workers must wear fall
protection during the following tasks:
• Erecting the structural steel
• Placing the roof decking
• Sheathing the floors
• All work performed on the roof
• All work performed near the edge of the floors before the exterior enclosure is
complete

Internal Company Risk and Mitigation
Construction is inherently risky. Puget Sound Construction, Inc. excels at identifying potential
risks and takes the necessary precautions to mitigate them before they prove to be a problem.
Existing Retaining Wall
An existing concrete retaining wall approximately 10 feet in height is located on the eastern
portion of the property and will remain in place with the proposed development. The original site
had one and two story single-family residences substantially set back from the retaining wall.
According to the calculations of the geotechnical engineer the proposed apartment building will
only be set back 12’ in order for the angle of repose to not put lateral pressure on to the existing
retaining wall. Structural excavation near the wall and the layout of the foundations pose a risk if
done incorrectly. PSC plans to subcontract the structural excavation, site survey and layout
scopes of work in order to transfer the risk on to the appropriate subcontractors.
Labor Productivity
Puget Sound Construction, Inc. plans to self-perform all structural concrete, reinforcement,
structural steel, rough framing, siding, and window and door installation. The value of this self
perform work including labor burden is $2,00,342 which is 17.95% of the total GMP. This
percentage is in the normal range for percentage of self-perform work, however because it is a
significant portion of the GMP, labor productivity must be tracked to make sure we do not go
over budget. If PSC is more productive than estimated this can be another source of profit.
Site Location
The project site is located in a small residential neighborhood in Bremerton, WA. Many of the
residents in the neighborhood are young children. The road is narrow and has limited access. All
of these pose a risk for possible third party injury. Purchasing general liability insurance as well
as bringing the delivery trucks onto the site while they are being unloaded will mitigate this risk.
Injury to the public is also a risk once the scaffolding goes up on the east side of the building.
The highest platform will be roughly 70’ above the sidewalk on Highland Ave. In order to
protect the general public from tools and materials falling onto the sidewalk, the scaffolding will
be enclosed with a protective screen.
Design Build MEP
Plumbing, HVAC, Fire Protection and Electrical scopes of work are to be contracted in a design
build fashion. This is a risk because PSC will be carrying the cost of design for those systems.
This risk can be mitigated by working with qualified subcontractors that PSC has worked with
before, and that have proven their competency in realizing the total cost of their scope before the
GMP contract is finalized. Any unexpected cost overruns could potentially come out of PSC’s
profit unless they could be justified by a change that came from the owner.
