7. Immunization Requirements
Starting Fall 2017, for all incoming students:
Submit via the myUOHealth Student Portal
Required Immunizations
Measles, Mumps and Rubella (MMR)
Varicella (chickenpox)
Tetanus, Diphtheria and Pertussis (Tdap)
Meningococcal Quadrivalent (MCV-4, MPSV-4)
Required Tuberculosis (TB) Screening
Must complete online questionnaire

9. Pharmacy
Includes prescriptions, consultations, low prices on over-
the-counter items (e.g. aspirin, cold and allergy medicines),
and direct-access birth control.

10. Dental
Includes check-ups, x-rays, cleanings, consultations, minor
fillings, and referrals. We do not do root canals or braces.

11. Physical Therapy / Sports Medicine
Includes physical therapy, athletic training, injury evaluation,
acupuncture, and massage therapy.

12. X-Ray
Includes general diagnostic and orthopedic x-rays.
No need to go to an outside radiology clinic.

13. Laboratory
Full range of on-site services including blood counts and
cultures.

14. Nurse Specialty Clinic
Includes immunizations, travel consultations, STI screening,
and allergy & asthma management.

15. Registered Dietitian
Includes nutrition education, healthy eating strategies,
diabetes management, body image, and sports nutrition.

16. Additional Services
Stress Reduction
Energy healing, massage, acupuncture, Relax & Renew
meditation class, sleep workshops, and referrals to the University
Counseling Center
Sexual Health Resources
Contraception, exams, pregnancy testing and counseling,
STI testing & treatment
Sexual Assault Nurse Examiners (SANE)
Exams for evidence collection after a sexual assault, support, and
resource referrals
Free Nicotine Replacement Therapy (NRT)

18. Sick Notes / Medical Excuses
UHC does not provide excuses for students who miss class
Commitment to confidentiality
Appropriate use of health care resources
Support dialogue between students and faculty
Students are advised to notify
faculty of injury/illness
Students with serious illness or
significant disability can request a
letter in their medical record for
access by Academic Advising

19. Health Service Fee
Mandatory for all enrolled students
Allows access to UHC services at much lower costs
compared to private clinics
Funds wellness activities and response to
campus health emergencies
The health service fee is not health insurance
Students are responsible for paying for UHC services

20. Courtesy Billing (Medical)
UHC is in-network with some insurance companies and will
courtesy-bill other plans
Currently accept: Oregon Health Plan, CCare, and
PacificSource UO Student Health Plan & GTF
Network providers are subject to change
Evaluate coverage before arriving on campus
Remaining balance after insurance payment is billed to the
student’s UO account (DuckWeb)
Must register insurance for courtesy billing

21. Pharmacy & Dental Billing
UHC Pharmacy and Dental clinics are in-network with most
insurance companies
Remaining balance after insurance payment (including co-pays)
is billed to the student’s UO account (DuckWeb)

All students are strongly encouraged to have coverage that
works well…

… anywhere their travels may take them (including international)

limited coverage when you leave your home area
