Employment
° Paystubs for the 30 days with year to date earnings
° Contact information for Verification of employment.
° W-2’s for the past two years.
° Tax returns for the last two years. Confirm page 2 is signed.
° If you own 25% or more of any business, please provide last 2 years business
tax returns and K1
Assets
° Checking, Savings, and investment statements (ALL pages) for the last two
months
° 401K statement from the last quarter ALL pages
Personal information
° Copy of a picture ID and Social Security Card
° Green Card or US visa if not US citizen
Additional Items (if relevant to your situation)
° Divorce decree
° Bankruptcy and Bankruptcy discharge papers
