This document is an information return for income tax credits on business machinery and equipment or working interests in an oil lease in Kansas. It provides instructions for operators to give working interest owners information to claim tax credits, including either providing copies of tax receipts and a breakdown of property taxes paid, or completing this K-64O form with schedules of taxes paid, owner names and identification numbers, and each owner's tax share. The form collects the total property taxes paid, number of attached schedules and receipts, and signatures of the operator and preparer.

Operators must provide each working interest owner with the information necessary to complete a Schedule K-64. To do this, operators
have two options:
1. Provide each working interest owner with a copy of the paid tax receipt(s) along with a breakdown of their share of the property
tax paid this tax year on the working interest of an oil lease or on the qualified business machinery and equipment, OR
2. Complete this information return, K-64O, and provide each working interest owner with a copy of the joint interest billing,
detailing their share of the property tax paid this tax year on the working interest of an oil lease or on the qualified business
machinery and equipment.
If option 2 is chosen, the operator must prepare a schedule of the following for each well and enclose it with this information return:
• the amount of tax paid this tax year on the working interest of an oil lease or on the qualified business machinery and equipment;
• the name and identification number (SSN or EIN) of each working interest owner; and
• each working interest owner’s share of the qualifying taxes.
