SUBJECTIVE Patient attends with complaints of leg pain for a period of about 1–1/2 weeks. Pain is
so severe that he has to keep active, the more active he is the less the discomfort bothers him. He
finds that sleeping at night really is uncomfortable because of the cramping in his legs, the cramps
start in the base of his feet and run up into the calves, but again, if he gets up and is active it seems

to the appointment due to his physical condition and distance. Phone conversation with the patient
was noted. He described his depression s mild. His sleep has been decreased. He uses CPAP
machine. He enjoys being outside. He has occasional feelings of hopelessness. His energy and
concentration have been fair. His appetite has decreased ad he has lost weight. He now weights 207
pounds. He denies any suicidal or homicidal ideation. Plan was to continue Pristiq 100 mg daily for
depression. He also gets Temazepam, methocarbamol muscle relaxants, and Buspar. Patient

12/03/15 UDT Report showed that the patient is consistent with the opioids.
10/29/15 Psychological QME indicated that the patient had an injury in December 2005. He noticed

recurrent, severe, non–psychotic and Generalized Anxiety Disorder was treated with 600 mg/day of
CYP2D6 inhibitor bupropion XL (a common antidepressant drug) and a 60 mg BID oral dose of
Dextromethorphan. The patient showed improvement in major depressive disorder without side
effects. This case study was the first to show the fast acting effect of Dextromethorphan in major
depressive disorder in humans. This adds to the building literature on the antidepressant effects
observed in Dextromethorphan. The report suggested the administration of other common
antidepressants with Dextromethorphan, such as lithium might prolong the duration of the rapid–
antidepressant effect [19].
Although this case report illustrates that Dextromethorphan has rapid antidepressant effects in
humans, there are very few clinical trial on the effect of Dextromethorphan on major depressive
disorder. This clinical trial will be one of very few controlled evaluating the safety and efficacy of

least one side effect than reboxetine or venlafaxine (OR 0.64, 95% CI 0.42 to 0.97 and OR 0.46,
