Integrated Plan
• Working together, the HPG, RWPC,AACO & OHP
developed and submitted the Philadelphia EMA
Integrated HIV Prevention and Care Plan 2017-
2021 to guide the planning of HIV Prevention, Care &
Treatment activities in the EMA.
• Although the plan is written by the OHP, it is with the
knowledge and expertise that the HPG and RWPC
membersAND stakeholders/community experts bring
to the table

Council, with the HD, will
monitor the plan over the next
5 year(s) making updates as
necessary
• Monitoring Activities are
continuous (see Section III:
Monitoring and
Improvement for specifics).
• Plan is updated when/as
needed

• Philadelphia EMA Integrated Plan was
submitted in 2016
– Currently awaiting feedback from CDC/HRSA
• It is a 5-year plan (2017-2021)
• There have been no changes to the Plan since it
was submitted
Concurrence
Refers to the planning group’s agreement that the we:
Have REVIEWED (monitored) the Philadelphia EMA Integrated
HIV Prevention and Care Plan 2017-2021 (submitted in 2016)
AND
CONCUR (agree) that the Plan describes how programmatic
activities & resources are being allocated to the most
disproportionately affected populations and geographic areas that
bear the greatest burden of HIV disease.
