Hello everyone! I Ishwari Basnet,I am a registered nurse and work in medical \surgical\mental health unit for more than ten years. I would like to welcome you all in this informal education session. Today, I would like to discuss about the changing role of nurses and concept of continuum of care and the new health care delivery models such as Accountable Care Organizations (ACOs), medical homes, and nurse -managed health clinics.
Evolving practice of Nursing and Health Care Models
Health care system is changing; it is not just focused on the curative care but the disease prevention and wellness. Also, the new healthcare reform has approach to the development of healthcare delivery models with the changes in nursing role from acute care settings to community health care. Patient Protection and Affordable Care Act (PPACA) formed at 2010 support the establishment of ACOs, medical homes and nurse-managed health clinics; also new payment policies. With the formation of Affordable Care Act (ACA), health has transformed from traditional treating approach to the preventive and wellness. Nurse’s role has also changed from front-line caregiver to the care integrators and coordinators and help in providing the health services to the underserved people. All the health models are focused on the concept of continuity of care which is defined as “a concept involving an integrated system of care that guides and tracks patient over time through a comprehensive array of health services spanning all levels of intensity of care” (Evashwick, 1989). It covers the delivery of healthcare over a period of time and for all levels and stages of care. Let’s talk about the three types of health care delivery model.
Accountable Care Organizations (ACOs)
In accordance to the Center for Medicare and Medicaid services, “Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients” (Cms.gov, n.d.). The ACO is the new unit built by the ACA; the health reform law 2010 which take care of the cost and quality of the care received by the patients. The main aim of ACO is to deliver flawless and high-quality care at the right time to the patients, especially the chronically ill patients. The ACO include the specific group of providers and suppliers assigned by the ACA and they encourage the active participation of nurse practitioners (NPs), clinical nurse specialists (CNSs) etc with physicians and physician assistants. Mayo clinic health system is a non-profit health organization that is based in Rochester, Minnesota and has a large number of employees ranging from 3900 physicians as well as more than 50,000 related health staff. This health care system has qualifications in scientific research and has been able to treat difficult health cases making it be one of the best organizations in the city. In terms of physical settings, Mayo clinic has presence in United States metropolitan areas of Arizona and Florida and also operates associated facilities right through Minnesota and Wisconsin. It has a very large college of medicine which also includes Mayo Medical School and Mayo School of Health Sciences (Ludwig, Viggiano, Mcgill & Oh, 1980).
Mayo clinic provides medical care for thousands of people in the U.S. and around the world; its client's bases are based on appointments as well as referrals by doctors from neighboring clinics. Most patients may return home after treatment while most patients who require inpatient care are always hospitalized in the clinic which is located on different campuses (Laurie, Moertel, Fleming, Wieand, Leigh, Rubin & Malliard, 1989).This medical facility has been able to provide best care to every patient and this is based on thorough clinical practice, research and education which are the secrets to success. Its model of care is defined by quality medical care delivery which includes meeting the needs of the patients. Delivery is based on care provided with compassion and trust among the physicians as well as respect.
According to the level of clinical activity, Mayo clinic is the world's largest multispecialty group medical practice which has been able to combine research, education and clinical practice to realize its activities of providing health care needs to patients and clients. It fosters team-oriented care and accountability hence has promoted absolute clinical excellence which has increased the level of clinical activities in this institution. It has specialized in information continuity as well as care coordination and transitions which have promoted easy access to appropriate care and information at all hours (Berry, 2004).

Transformational leadership styles positively influence the quality of nurses' working lives in Taiwan. A study examined the relationship between transformational leadership, supervisor support, job satisfaction, organizational commitment, and nurses' general health and well-being. Transformational leadership was found to significantly increase supervisor support and positively influence all variables. Supervisor support was an important mediator between transformational leadership and job satisfaction. Organizational commitment had the strongest relationship to nurses' general health. The findings demonstrate how work relationships and organizational factors impact nurses' quality of working lives.

Hello everyone! I Ishwari Basnet,I am a registered nurse and work in medical \surgical\mental health unit for more than ten years. I would like to welcome you all in this informal education session. Today, I would like to discuss about the changing role of nurses and concept of continuum of care and the new health care delivery models such as Accountable Care Organizations (ACOs), medical homes, and nurse -managed health clinics.
Evolving practice of Nursing and Health Care Models
Health care system is changing; it is not just focused on the curative care but the disease prevention and wellness. Also, the new healthcare reform has approach to the development of healthcare delivery models with the changes in nursing role from acute care settings to community health care. Patient Protection and Affordable Care Act (PPACA) formed at 2010 support the establishment of ACOs, medical homes and nurse-managed health clinics; also new payment policies. With the formation of Affordable Care Act (ACA), health has transformed from traditional treating approach to the preventive and wellness. Nurse’s role has also changed from front-line caregiver to the care integrators and coordinators and help in providing the health services to the underserved people. All the health models are focused on the concept of continuity of care which is defined as “a concept involving an integrated system of care that guides and tracks patient over time through a comprehensive array of health services spanning all levels of intensity of care” (Evashwick, 1989). It covers the delivery of healthcare over a period of time and for all levels and stages of care. Let’s talk about the three types of health care delivery model.
Accountable Care Organizations (ACOs)
In accordance to the Center for Medicare and Medicaid services, “Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients” (Cms.gov, n.d.). The ACO is the new unit built by the ACA; the health reform law 2010 which take care of the cost and quality of the care received by the patients. The main aim of ACO is to deliver flawless and high-quality care at the right time to the patients, especially the chronically ill patients. The ACO include the specific group of providers and suppliers assigned by the ACA and they encourage the active participation of nurse practitioners (NPs), clinical nurse specialists (CNSs) etc with physicians and physician assistants. Mayo clinic health system is a non-profit health organization that is based in Rochester, Minnesota and has a large number of employees ranging from 3900 physicians as well as more than 50,000 related health staff. This health care system has qualifications in scientific research and has been able to treat difficult health cases making it be one of the best organizations in the city. In terms of physical settings, Mayo clinic has presence in United States metropolitan areas of Arizona and Florida and also operates associated facilities right through Minnesota and Wisconsin. Delivery is based on care provided with compassion and trust among the physicians as well as respect.
According to the level of clinical activity, Mayo clinic is the world's largest multispecialty group medical practice which has been able to combine research, education and clinical practice to realize its activities of providing health care needs to patients and clients. It fosters team-oriented care and accountability hence has promoted absolute clinical excellence which has increased the level of clinical activities in this institution. It has specialized in information continuity as well as care coordination and transitions which have promoted easy access to appropriate care and information at all hours (Berry, 2004).

Transformation Leadership
Transformational leadership styles positively influence the quality of nurses' working lives in Taiwan. A study examined the relationship between transformational leadership, supervisor support, job satisfaction, organizational commitment, and nurses' general health and well-being. Transformational leadership was found to significantly increase supervisor support and positively influence all variables. Supervisor support was an important mediator between transformational leadership and job satisfaction. Organizational commitment had the strongest relationship to nurses' general health. The findings demonstrate how work relationships and organizational factors impact nurses' quality of working lives.

My name is Taimi (pronounced ty-mee). I have worked as a registered nurse with an associate of science degree for over 24 years. Across these years, I have practiced nursing in the states of New Jersey, Pennsylvania, and my home state of Massachusetts, to which I returned two years ago. When I graduated from a hospital-based nursing program, I took a job as a Post-Acute/Rehab Care staff nurse. After two years as a staff nurse, I was promoted to a clinical management position in a Post-Acute Care (PAC) facility. Since then, I have worked in a variety of milieus, including both free-standing and hospital-based facilities and served as a corporate nurse, a traveling nurse, and a private consultant. My nursing specialty has grown exponentially over the years. My role now encompasses reporting to many governmental agencies, on both a state and federal level, and most recently, I am involved in mandated Quarterly Reporting Program (SNF QRP). I am currently working as a Regional Clinical Reimbursement Specialist for a major Post-Acute Care Company.
I have been in the healthcare field since I was a junior in high school, working as a candy striper, certified nursing assistant, and home health aide before becoming an RN. So, I have been both exposed to and used critical thinking for a lot of years. Nevertheless, I am excited to broaden my understanding, experience, and critical thinking skills with new and fresh information on the best practices approach of nurse leaders in today's healthcare environment.
How do you see current trends and changes in healthcare models impacting the delivery of healthcare? In the recent past, the fee for service healthcare trend generated money through the volume of patients or census levels and viewed the patient as a source of revenue. Whereas, the new trend of value-based healthcare has changed healthcare, particularly in my industry of post-acute care. Our patients have now become an expense versus a source of revenue. This change has made it necessary for different types of healthcare providers to stop working in silos and instead communicate and collaborate with care providers across the continuum. Each provider will need to leverage each other’s strengths to provide the best possible outcomes for the patient. Also, with this new value-based payment model system, providers, and doctor’s need to begin to see cradle to grave care within a fixed premium payment.

Physical Environment: How I experience the work environment at any given time, how it feels to work in the organization and to perform in the organization’s culture influences my mood. Within the facility, families visiting their patients have a well-furnished waiting bay with television in which they first rest before being allowed to see their patients. Those with cars can access marked parking lots that are enough. Since the organization is Christian-based, it has a chapel in which patients’ families can gather and pray for their loved ones undergoing medical treatment.
Environmental Safety: As workers, we share values and beliefs that influence our behavior within the organization. It is through these share aspects in which the organization has identified to provide safety measures since the organization believes that healthy workers are essential assets in performance. As a result, environmental safety measures such as safety stickers on elevators, stairwells, passageways, and hallways are maintained. These stickers remind workers and other people that safety at the workplace is critical and that they should observe safety measures.
Social Environment: The organization has maintained its annual event at the end of the year in which all workers and other senior employees converge to celebrate the achievements of that year. These internal social events have helped in strengthening interdepartmental socialization and making the organization one big family. As a result, the shared assumptions, values, and beliefs of the organization get enhanced to propel the achievement of goals and realization of the vision.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

- 3. “I am thrilled to welcome Ken Moore to our leadership team,” said Stacie McEntyre, founder, president, and CEO of Veritas Collaborative. “His extraordinary leadership skills, unique experience in inpatient eating disorders hospital operations, and mission-driven focus will ensure that Veritas Collaborative continues to provide premier care as we expand to new locations.”
- 4. Moore is a veteran hospital executive with a 30-year track record of excellence in the behavioral health field. He joined Veritas July 5 after serving as the director of operations at one of the most respected inpatient eating disorders hospitals in the country – The Laureate Eating Disorders Program within the Laureate Psychiatric Clinic and Hospital in Tulsa, Oklahoma. He has experience effectively managing multiple treatment cohorts and multiple levels of care – maximizing efficiencies, maintaining consistency, and infusing the company’s culture across all programs.
- 5. Moore is responsible for overseeing the operations of each facility within the Veritas hospital system, including two inpatient hospitals in Durham, a facility in Richmond, Virginia, and a new 50-bed pediatric hospital to open in Atlanta next year. “What drew me to Veritas Collaborative was the opportunity to be a part of an organization that has developed an impeccable reputation for providing excellent eating disorders care,” said Moore. “I’m excited to be part of a team dedicated to driving a new standard of care in eating disorders treatment.”
- 6. In addition to his credentials in health care operations and senior leadership, Moore’s career also includes credentials as a licensed clinical social worker and psychotherapist. He was named the 2011 Adjunct Instructor of the Year and 2010 MSW Educator of the Year from the University of Oklahoma Zarrow School of Social Work. Moore holds both a bachelor’s degree and master’s degree in social work from the University of Oklahoma.
