  1. Each organization will submit one nominee to be considered for the Outstanding Volunteer of the Year. The nomination will include a cover sheet with the person’s name, organization and telephone number. On a separate page, cite duties and why the nominee should be selected. DA Form 4713 must be attached showing hours volunteered and must be signed by the volunteer’s supervisor. Nominations submitted without DA Form 4713 will not be accepted. The nomination for Volunteer of Year must be turned in no later than 15 March 2011. The nomination can be brought in hard copy form to Yolanda Hingel at Army Community Service (ACS) or sent by email to yolanda.f.hingel.civ@mail.mil. A panel will rate nominees by a point system criterion. The three highest scores will be awarded the Outstanding Volunteer of the Year (1st), Honored Volunteer of the Year (2nd), and Merited Volunteer of the Year (3rd). Points will be given for the length of time the individual has been volunteering, the number of hours volunteered during 2011, the list of volunteer hours provided to other services or agencies, the individual’s impact on the U.S. Army Garrison, Japan, and the enhancement of bilateral relationship with our host nation.
