OVERVIEW
What 
Where 
When
Community-Based Health Fairs designed to reduce diabetes risk factors and improve
disease self-management within non-Hispanic black communities in Miami Florida.
Attendees will receive free health screenings, coaching, educational materials, and
have access to activities and demonstrations pertaining to lifestyle modifications. We
aim to motivate participants to make positive health behavior changes through
education and skills training related to self-care practices.
The Health fairs will be held at public parks in predominantly non-Hispanic black
communities in Miami including Little Haiti, Liberty City & Overtown.
The health fairs will be held every three months for 1 year.

What can attendees to
the health fair expect
to find there?
Know Your Health Numbers Tent:
You can’t manage what you don’t measure, which is why we want health fair attendees to know how their personal health numbers
correlate with their risk for developing Type 2 Diabetes. Each attendee will progress through stations wherein we will provide on-site
measurements of Height, Weight, BMI, Waist Circumference, Blood Pressure, and Cholesterol. The results of these measurements will
then be used where applicable to answer questions on a 7 item diabetes risk assessment survey developed by the Centers for Disease
Control and Prevention (CDC; http://www.cdc.gov/diabetes/prevention/pdf/prediabetestest.pdf). Individuals with elevated risk
factors on evaluation will be ushered to the HgA1C screening tent for further assessment.
HgA1c Screening Tent:
Evaluation at this tent will be restricted to those individuals found to be at higher risk for prediabetes and diabetes on the paper-
based survey. Nurse and APRN facilitators will run stations performing fingers sticks to assess the glycosylated hemoglobin (A1c) using
point-of-care A1c monitoring kits. The use of A1c is preferred over fasting or random glucose as it requires only a drop of capillary
blood (finger prick) and can be completed at any time of the day. Hence, it is appropriate for community settings and reflects the
average blood glucose levels in the past 3 months. A1c levels of 5.7 and higher with no prior diagnosis will allow us to identify
individuals with prediabetes or possibly undiagnosed diabetes. Facilitators will provide individuals who are identified as being at risk
with a written report of the screening results and contact information for identified providers.
Goal Setting & Evaluation Tent:
Our nurse facilitators will provide stations for 15 minute one-on-one consultations with attendees that are interested in reviewing
their personal health numbers and setting specific blood pressure, activity, and weight loss goals. Participants will be encouraged to
attend each future Health Fair for follow-up, review and reevaluation of goals.
Healthy Eating Education Tent: 
Our nutrition facilitators will provide educational handouts and stations for Q30 min group teaching on the diabetes diet, label
reading, meal planning, carbohydrate counting, controlling dietary fats, portion sizes and keeping food logs to identify barriers to
healthy eating.
Healthy Eating Demo Tent:
Tasting healthy food converts skeptics and creates teachable moments. Our experts will demo how culture specific diabetic
meals and snacks can be prepared using recipes from the Diabetes Soul Food Cookbook. Attendees will receive recipe packets
that use simple, low-cost, low-fat, low- cholesterol ingredients.
Fitness Education Tent: 
Our exercise physiology facilitators will provide educational handouts and stations for Q30 min group teaching on aerobic exercises,
overall fitness, and weight management. Topics covered will include barriers to being active, types of physical activity (walking, work-
related exercise, leisure time physical activities, group exercises and community-based exercise programs) and cultural preferences
for physical activity.
Fun Fitness Demo Tent:
Our exercise instructor will lead group exercises demonstrating easy ways to be active without the gym. Participants will learn to use
exercise as their medicine!
Health Coping Tent:
Social workers will provide educational handouts and stations for Q30 min group teaching on psychological distress and ways in
which it directly affects health and influences motivation. Participants will receive instruction on stress reduction strategies and
behavioral skills for emotional management including deep breathing exercises, and mental imagery.
Meditation Demo Tent:
A licensed clinical psychologist will lead participants in a meditation exercise involving progressive muscle relaxation (PMR).
Attendees will learn to consecutively tense and relax muscle groups in the body, starting with the feet and progressing to the
head. PMR may also be combined with techniques such as deep breathing and mental imagery specific to African-American
culture.
Risk Reduction Tent: 
Our nurse facilitators will provide educational handouts and stations for Q30 min group teaching on ways to reduce risk behaviors
such as smoking, and the importance of regular eye, foot and dental examinations. Participants will be taught how to identify the
need for a referral and how to make a list of their questions or concerns in order to improve communication with their providers.
Medication Management Tent: 
Facilitators will provide educational handouts and a Q30 min group teaching forum that targets diabetes specific medications,
their actions, side effects, and toxicity. Facilitators will review the effects of missed or delayed doses, and proper storage of
medications. 15-minute one-on-one medication education consultations will be offered for attendees that participated in the
educational forum. These participants will have the opportunity to review their prescribed medications, and ask questions
about their personalized regimens.
Loving Someone with Diabetes Tent:
Facilitators will provide handouts and stations for Q30 min group teaching providing education, advice and help for those who
care about someone with diabetes. Through interactive learning and scenario role-playing, participants will gain a better
understanding of how to recognize signs of a problem, become involved in healthy lifestyle changes, and offer non-judgmental
support, comfort, and kindness to loved ones with T2DM. Stations within this tent will strive to promote a sense of
interdependence among family members, which is a major strength when managing and preventing a chronic illness such as T2D.
Blood Glucose Monitoring Tent: 
Nurses and APRNs will provide education about equipment choice and selection, the timing and frequency of testing, target
values, and interpretation and use of results. Participants will be able to check glucose levels under the guidance of a registered
nurse.

Turning a New Leaf: Health Fairs for Diabetes Prevention & Management

  • 1. DIABETES PREVENTION & MANAGEMENT TURNING A NEW LEAF QUARTERLY HEATLH FAIRS
  • 2. OVERVIEW What  Where  When Community-Based Health Fairs designed to reduce diabetes risk factors and improve disease self-management within non-Hispanic black communities in Miami Florida. Attendees will receive free health screenings, coaching, educational materials, and have access to activities and demonstrations pertaining to lifestyle modifications. We aim to motivate participants to make positive health behavior changes through education and skills training related to self-care practices. The Health fairs will be held at public parks in predominantly non-Hispanic black communities in Miami including Little Haiti, Liberty City & Overtown. The health fairs will be held every three months for 1 year.

General Self-Care for Diabetes Prevention: Specific Self-Care for Diabetes Management: All Health Fair facilitators will use The Theory of Collaborative Patient-Provider Relationships to guide their interaction, education, and skills training with attendees.

Provide skills training on diabetes management Provide positive feedback, acknowledgement, support and motivation to attendees  who are showing up for change!
  • 6. What can attendees to the health fair expect to find there?
  • 7. Know Your Health Numbers Tent: You can’t manage what you don’t measure, which is why we want health fair attendees to know how their personal health numbers correlate with their risk for developing Type 2 Diabetes. Each attendee will progress through stations wherein we will provide on-site measurements of Height, Weight, BMI, Waist Circumference, Blood Pressure, and Cholesterol. The results of these measurements will then be used where applicable to answer questions on a 7 item diabetes risk assessment survey developed by the Centers for Disease Control and Prevention (CDC; http://www.cdc.gov/diabetes/prevention/pdf/prediabetestest.pdf). Individuals with elevated risk factors on evaluation will be ushered to the HgA1C screening tent for further assessment. HgA1c Screening Tent: Evaluation at this tent will be restricted to those individuals found to be at higher risk for prediabetes and diabetes on the paper- based survey. Nurse and APRN facilitators will run stations performing fingers sticks to assess the glycosylated hemoglobin (A1c) using point-of-care A1c monitoring kits. The use of A1c is preferred over fasting or random glucose as it requires only a drop of capillary blood (finger prick) and can be completed at any time of the day. Hence, it is appropriate for community settings and reflects the average blood glucose levels in the past 3 months. A1c levels of 5.7 and higher with no prior diagnosis will allow us to identify individuals with prediabetes or possibly undiagnosed diabetes. Facilitators will provide individuals who are identified as being at risk with a written report of the screening results and contact information for identified providers. Goal Setting & Evaluation Tent: Our nurse facilitators will provide stations for 15 minute one-on-one consultations with attendees that are interested in reviewing their personal health numbers and setting specific blood pressure, activity, and weight loss goals. Participants will be encouraged to attend each future Health Fair for follow-up, review and reevaluation of goals.
  • 8. Healthy Eating Education Tent:  Our nutrition facilitators will provide educational handouts and stations for Q30 min group teaching on the diabetes diet, label reading, meal planning, carbohydrate counting, controlling dietary fats, portion sizes and keeping food logs to identify barriers to healthy eating. Healthy Eating Demo Tent: Tasting healthy food converts skeptics and creates teachable moments. Our experts will demo how culture specific diabetic meals and snacks can be prepared using recipes from the Diabetes Soul Food Cookbook. Attendees will receive recipe packets that use simple, low-cost, low-fat, low- cholesterol ingredients. Fitness Education Tent:  Our exercise physiology facilitators will provide educational handouts and stations for Q30 min group teaching on aerobic exercises, overall fitness, and weight management. Topics covered will include barriers to being active, types of physical activity (walking, work- related exercise, leisure time physical activities, group exercises and community-based exercise programs) and cultural preferences for physical activity. Fun Fitness Demo Tent: Our exercise instructor will lead group exercises demonstrating easy ways to be active without the gym. Participants will learn to use exercise as their medicine!
  • 9. Health Coping Tent: Social workers will provide educational handouts and stations for Q30 min group teaching on psychological distress and ways in which it directly affects health and influences motivation. Participants will receive instruction on stress reduction strategies and behavioral skills for emotional management including deep breathing exercises, and mental imagery. Meditation Demo Tent: A licensed clinical psychologist will lead participants in a meditation exercise involving progressive muscle relaxation (PMR). Attendees will learn to consecutively tense and relax muscle groups in the body, starting with the feet and progressing to the head. PMR may also be combined with techniques such as deep breathing and mental imagery specific to African-American culture. Risk Reduction Tent:  Our nurse facilitators will provide educational handouts and stations for Q30 min group teaching on ways to reduce risk behaviors such as smoking, and the importance of regular eye, foot and dental examinations. Participants will be taught how to identify the need for a referral and how to make a list of their questions or concerns in order to improve communication with their providers.
  • 10. Medication Management Tent:  Facilitators will provide educational handouts and a Q30 min group teaching forum that targets diabetes specific medications, their actions, side effects, and toxicity. Facilitators will review the effects of missed or delayed doses, and proper storage of medications. 15-minute one-on-one medication education consultations will be offered for attendees that participated in the educational forum. These participants will have the opportunity to review their prescribed medications, and ask questions about their personalized regimens. Loving Someone with Diabetes Tent: Facilitators will provide handouts and stations for Q30 min group teaching providing education, advice and help for those who care about someone with diabetes. Through interactive learning and scenario role-playing, participants will gain a better understanding of how to recognize signs of a problem, become involved in healthy lifestyle changes, and offer non-judgmental support, comfort, and kindness to loved ones with T2DM. Stations within this tent will strive to promote a sense of interdependence among family members, which is a major strength when managing and preventing a chronic illness such as T2D. Blood Glucose Monitoring Tent:  Nurses and APRNs will provide education about equipment choice and selection, the timing and frequency of testing, target values, and interpretation and use of results. Participants will be able to check glucose levels under the guidance of a registered nurse.

Reduce the # of Non- Hispanic Black Residents with T2DM in the city of Miami using secondary prevention interventions like screenings Increase the confidence of Non-Hispanic Black Residents in the city of Miami to properly care for themselves Increase awareness of local and national diabetes health services and resources Reduce the debilitating consequences of poorly controlled T2DM using primary prevention interventions like lifestyle modifacation & disease management education
