Community-based health fairs will be held every three months for one year in predominantly non-Hispanic black communities in Miami to reduce diabetes risk factors and improve disease management. The health fairs will provide free health screenings, education, and activities to motivate participants to make positive lifestyle changes through coaching and skills training. Various tents will address topics like screenings, diet, exercise, medication management, and more using a collaborative patient-provider relationship model. The goals are to reduce the number of residents with diabetes, increase self-care confidence and awareness of resources, and reduce complications of uncontrolled diabetes.
