Tuberopterygoid implants are dental implants placed in the upper jaw in the second molar area that anchor in the dense cortical bone of the pterygoid plates, avoiding the need for invasive sinus lift surgery. They allow for immediate loading and replacement of teeth up to the second molars. Key advantages include eliminating sinus grafting, replacing all teeth without cantilevers, using strong single-piece implants in rigid cortical bone, and enabling immediate loading with permanent crowns in just 3 days with a flapless technique.
Pterygoid implant insertion is an alternative to avoid sinus-lifting or other grafting procedures to treat the posterior maxilla. They are especially used in partial edentulism in order to avoid distal cantilevers.
The placement of a pterygoid implant requires surgical experience and expertise in the field of implantology.
Pterygoid implants have high success rates, to those of conventional implants, minimal complications and a good patient acceptance.

Maxillary ridge augmentation is a common procedure nowadays, This presentation is about the direct and indirect procedures for maxillary sinus lift for implant placement. with recent advancement in the procedures.
Many patients have been told that they are not candidates for dental implants due to lack of bone in the upper jaw. These patients can benefit from an implant developed by Professor Per Ingvar Brånemark. This implant is called the zygomatic implant ,which provides an excellent alternative to bone grafting procedures for the severely resorbed jaw bone with minimal surgical trauma and maximum oral function.
The Zygomatic Implants are attached to the area of the jawbone close to the zygoma bone.
The extra-long implant is placed from inside the mouth near the location of the bicuspid teeth, it goes through or right next to the sinus and anchors into the thick and solid zygomatic bone.
The common recovery time after this procedure is about four months in length. After this time, a final restoration can be applied to completely finish the operation. By the end of this period, patients can enjoy their new set of teeth.
The placement of zygomatic implants requires surgical experience and expertise in the field of implantology.

Sinus Lift Technique
While placing Dental Implants on posterior region of upper jaw, due to either expansion of Maxillary Sinus as age advances or ridge resorption occurs because of various reason, dental implants cannot be placed on inadequate bone.
Sinus Lift procedure, to elevate Sinus Membrane through bone graft from socket, and Implant installation these indirectly augment the ridge and is called Indirect Technique.
And if it is done through Cald-well-luc procedure, it is called Direct Sinus Technique.

How Singapore is Paving the Way for Global Smart CitiesSolace
Singapore is at the leading edge of global smart city initiatives and is a good deal of the way along the path to truly transforming the “city nation” for the benefit of its citizens and the redefinition of its government. Everything is on the table for reinvention in Singapore from smart transportation to smart energy to smart buildings to how its citizens receive smarter healthcare.
But what is a “smart city”? A smart city uses technology to deliver services, uses technology to reduce costs for service providers and uses technology to allow citizens to improve the running of the city. With sensors planted measuring everything from traffic to street lights, Singapore has deep insights into the lessons in IoT deployment and delivery on a truly large scale.
On October 13th, 2016 at the Internet of Things Developer meetup, Sumeet Puri presented a social and technical perspective on the Singapore smart cities initiative from two viewpoints: his role as an architect of several Singapore Smart City projects and as a resident of Singapore living through the transformation of his surroundings. Sumeet will share stories about Singapore’s efforts, and give us insights into the technical decisions needed to make this a reality. Sumeet will talk about architecting for IoT at scale, lessons learned from the Singapore Smart City Initiatives and the response of the public to these changes.

Basal Implant is an advanced technology to replace missing tooth. It is specially designed for immediate implant loading so you will get permanent teeth just within 3 days and patients will get permanent natural looking ceramic teeth. Basal Implants have a high success rate for those patients have gum disease.
A photographic illustration of the dental implant options. the presentation talks about implants from the standard implant with the associated ridge augmentation and another modifying tech. followed by other options including short implants, all on four concepts, zygomatic and pterygoid implants, and the 3D subperiosteal implants.
Clinical Replacement Therapy and the Immediate Post-extraction Dental ImplantAbu-Hussein Muhamad
Immediate dental implants have greatly reduced the treatment time and the number of surgical intervene tions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. The aim of this article is to describe a clinical case in which a fractured maxillary canine was replaced by an osseointegrated implant using a simplified technique in a patient who was a smoker and presented poor oral hygiene. The technique adopted permits a reduction of the number of implant components and consequently a lower cost of treatment, while at the same time maintaining acceptable aesthetic and functional outcomes.
Immediate Implant Placement And Restoration With Natural Tooth In The Maxilla...Abu-Hussein Muhamad
Anterior tooth loss and restoration in the esthetic zone is a common challenge in dentistry today. The prominent visibility of the area can be especially distressing to the patient and requires a timely and esthetically pleasing solution. Immediate single-tooth implantation followed by immediate provisionalization is becoming an increasingly desirable treatment that offers numerous benefits over conventional delayed loading. Provisionalization for immediately-placed implants using the patient’s existing tooth can enhance the final aesthetic outcome if certain steps are
followed. If the natural tooth is intact and can be used as a provisional, the emergence profile can be very similar to the preoperative condition. This article outlines a technique to use the patient’s natural tooth after extraction to provisionalize an implant.

Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration
HEMORRHOIDS
Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)
FECAL INCONTINENCE
Fecal incontinence is the inability to control passage of feces and gas from the anus. Incontinence harms a patient’s body image

Antibiotic Stewardship by Anushri Srivastava.pptxAnushriSrivastav
Stewardship is the act of taking good care of something.
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
WHO launched the Global Antimicrobial Resistance and Use Surveillance System (GLASS) in 2015 to fill knowledge gaps and inform strategies at all levels.
ACCORDING TO apic.org,
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
ACCORDING TO pewtrusts.org,
Antibiotic stewardship refers to efforts in doctors’ offices, hospitals, long term care facilities, and other health care settings to ensure that antibiotics are used only when necessary and appropriate
According to WHO,
Antimicrobial stewardship is a systematic approach to educate and support health care professionals to follow evidence-based guidelines for prescribing and administering antimicrobials
In 1996, John McGowan and Dale Gerding first applied the term antimicrobial stewardship, where they suggested a causal association between antimicrobial agent use and resistance. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis.
Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance.
According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. In addition to this, it also mentioned that 223,900 cases of Clostridoides difficile occurred in 2017, of which 12800 people died. The report did not include viruses or parasites

Medical Technology Tackles New Health Care Demand - Research Report - March 2...pchutichetpong
M Capital Group (“MCG”) predicts that with, against, despite, and even without the global pandemic, the medical technology (MedTech) industry shows signs of continuous healthy growth, driven by smaller, faster, and cheaper devices, growing demand for home-based applications, technological innovation, strategic acquisitions, investments, and SPAC listings. MCG predicts that this should reflects itself in annual growth of over 6%, well beyond 2028.
According to Chris Mouchabhani, Managing Partner at M Capital Group, “Despite all economic scenarios that one may consider, beyond overall economic shocks, medical technology should remain one of the most promising and robust sectors over the short to medium term and well beyond 2028.”
There is a movement towards home-based care for the elderly, next generation scanning and MRI devices, wearable technology, artificial intelligence incorporation, and online connectivity. Experts also see a focus on predictive, preventive, personalized, participatory, and precision medicine, with rising levels of integration of home care and technological innovation.
The average cost of treatment has been rising across the board, creating additional financial burdens to governments, healthcare providers and insurance companies. According to MCG, cost-per-inpatient-stay in the United States alone rose on average annually by over 13% between 2014 to 2021, leading MedTech to focus research efforts on optimized medical equipment at lower price points, whilst emphasizing portability and ease of use. Namely, 46% of the 1,008 medical technology companies in the 2021 MedTech Innovator (“MTI”) database are focusing on prevention, wellness, detection, or diagnosis, signaling a clear push for preventive care to also tackle costs.
In addition, there has also been a lasting impact on consumer and medical demand for home care, supported by the pandemic. Lockdowns, closure of care facilities, and healthcare systems subjected to capacity pressure, accelerated demand away from traditional inpatient care. Now, outpatient care solutions are driving industry production, with nearly 70% of recent diagnostics start-up companies producing products in areas such as ambulatory clinics, at-home care, and self-administered diagnostics.

Tuberopterygoid implants
1. Tuberopterygoid Implants
What are Tuberopterygoid Implants?
Tuberopterygoid implants are implants located in 2nd molar area of
upper jaw. They allow the prosthetic rehabilitation on dental implants in
upper jaw. The applied forces go to the pterygomaxillary buttress area
allowing immediate loading of the implants. The white arrows show
tuberopterygoid implant, this avoided invasive Sinus lift Surgery and
saved time period of 6-8 months post sinus lift surgery. We were in
position to deliver fixed teeeth in period of just 3 days even in such an
atrophied situation.
2. Introduction
If you have been told that there is less bone in your upper jaw for dental
implants, do not worry. The best solution for cases with severe bone
resorption in upper jaw is pterygoid implant. Tubero pterygoid
implants are the best solution for patients requiring dental implants
for upper jaw.
Such patients were going for sinus lift or bone grafts as per old
techniques. In a single procedure we place pterygoid implant and place
the permanent crown in 3 days. As we use single piece implants the
procedure is flapless and done under local anesthesia.
3. Current definitions for pterygoid implants
Throughout literature, several terms are being used to describe
pterygoid implants. The terms "pterygoid implants", "pterygomaxillary
implants" and "tuberosity implants" are used interchangeably.
"Pterygoid implants" have been defined by the Glossary of Oral and
Maxillofacial Implants (GOMI) as "implant placement through the
maxillary tuberosity and into the pterygoid plate".
• There are significant differences between pterygoid and tuberosity
implants including the following:
• pterygoid implants originate in the tuberosity and engage in the
dense bone of the pterygoid plates and palatine bone
• tuberosity implants originate in the maxillary alveolar process and
can occasionally anchor in the pyramidal process. The bone quality
in this region is mainly composed of type III or IV cancellous bone
Indications
• In the edentulous upper jaw the placement of implants can be
challenging due to limited bone quantity and the presence of the
maxillary sinus. Pterygoid implants have high success rates, similar
bone loss levels to those of conventional implants, minimal
complications and good acceptance by patients, being an
alternative to treat patients with highly atrophic maxillae and
avoiding the need for extensive augmentation procedures.
• However, at present, pterygoid implants have mainly been studied
in partial edentulism as a very attractive treatment alternative to
sinus lift procedures.
4. Procedure
When the upper grinding teeth are lost and are not replaced on time, it
leads to drooping of the sinus there by resulting in bone resorption
making it difficult for tooth replacement, Some conventional
implantologists suggest Sinus lift procedure, which is a complex
traumatic surgical approach, that is totally unpredictable and takes
approximately 6 to 8 months to heal. All the patients interested in
getting the upper full arch replaced on implants are suitable candidates
for this as pterygoid bone is present in all the patients. Pterygoid bone
is present distal to the second molar region in the upper jaw and is the
only option available if you want to avoid cantilevers, but it requires a
very experienced hand with an in depth knowledge of Anatomy of the
surrounding areas.
The option of fixed teeth on dental implants in upper jaw
• Cantilever bridges: It devised to avoid the expensive and aggressive
bone grafting by placing just 4 or 6 implants in the front part of the
jaw. Then placing a set of just 10/12 teeth. As 1st molar crowns are
placed as cantilevers but there is no need to replace 2nd molars at
all.
5. • Occasionally, only 10 teeth are placed. The permanent ceramic
bridges are fixed after 3-6 months which is another disadvantage of
cantilevers.
Clinical recommendations
The pterygoid implant is placed in the region of the former first or
second maxillary molars and follows a diagonal direction posteriorly
towards the pyramidal process. The implant ultimately anchors in the
pterygoid fossa of the sphenoid bone. The angulation of pterygoid
implants ranges from 45°-50° towards the maxillary plane.
Before and After of Tubero-pterygoid implants
• In clinical reality, whenever pterygoid implants are described those
are in reality tuberosity implants with an anchorage in the
pterygoid plate and thus the treatment difficulties are very limited.
As with every implant surgery the preparation of the implant bed is
of high importance and treatment planning should be performed
with great attention to the anatomical structures and bone
availability/quality.
6. • Due to the presence of type III or IV cancellous bone in this region,
a preferred choice of implant type is the NobelActive implant given
its high primary stability in soft bone
Advantages of Tubero pterygoid Implants
• Eliminates the need of sinus grafting.
• No cantilevers from front teeth or implants.
• Efficient chewing is possible as teeth upto second molars are
replaced. In cantilevers implant cases or in all-on -4 and all – on – 6
techniques 2nd molars are not replaced. If replaced then they are
made as cantilevers which make the last implant weak and the
bridge susceptible to fracture in the long run.
• Bone in pterygoid area is cortical in nature, and hence very strong
and rigid allowing Immediate loading with permanent crown in 3
days.
• Single piece implants are used which are strong in comparison to
conventional dental implants.
• All patients who want dental implants in upper jaw are good
candidates as bone is always available in pterygoid area.
• Flapless technique is used, as such patients have no swelling or
bleeding and patients need not stop blood thinners, if they are
taking any.
• With Tuberopterygoid Implant - No More Sinus Lift No More Bone
graft.
• Strong single piece implants are being used in strong cortical bone.
• 100% success rate.
