in surgical training by using

The expectations of modern healthcare call for
greater speed, precision, and timely delivery of
treatment in every aspect, and surgical processes
are no exception. Implementing emerging trends of
technology like robotics in the medical field enables a high
level of patient care, efficient processes in clinical settings,
and a safe environment for patients and healthcare workers.
Medical robots support minimally invasive procedures,
customized and frequent monitoring for patients with
chronic diseases, intelligent therapeutics, and social
engagement for elderly patients. In addition, as robots
alleviate workloads, nurses and other caregivers can offer
patients more empathy and human interaction, which can
promote long-term well-being.
Robotic-assisted surgery is becoming the standard of care in
many specialties where open and laparoscopic surgery was
once the standard. With more robots entering the market,
the increased competition is resulting in lower costs for
installation and greater uptake. Robotic surgery adoption
is increasing as surgical robots can reduce the physical
demands on surgeons in complex cases, and reduce patient
blood loss and adverse operating events, leading to shorter
hospital stays and quicker patient recovery.

The Tale of Inception
The genesis of IMRA began approximately five years ago
with Professor Tony Costello, the Founder's vision
that—robotic surgery is the future and that the education of
surgeons on this modern technology should be surgeon to
surgeon—independent from industry.
With support from the Victoria government and
philanthropist's help, he built a world-class training facility
in Melbourne. Using Professor Costello's training
pedagogy, Adam built an expert surgeon-led curriculum and
has incorporated synthetic organ models and extensive
simulation into IMRA's online and in-person courses.
The adult learning principles today depict that using
innovative ways to build skills can be an easier way to train
surgeons. Although Halsted's "See one, Do one, Teach one"
model of surgical learning has merit, it's not well suited to
teaching on robotic platforms for several reasons, not the
least being that it is focused on practicing on patients.
For this reason, Adam and his team have developed a
simulation-based training protocol akin to the aviation
industry, adopting a lot of their safety principles, such as
preparation for adverse events, emergencies, skill repetition
and human factors, and simulation. This training is based on
the new adult learning principles whereby—physically
completing tasks away from the patient and practicing
alongside expert faculty—will support better patient
outcomes and make surgeons proficient in their skills.
IMRA eliminates the ethical concern of causing harm to
animals and cadavers in surgical training by using synthetic
models. These models replicate human tissue and are cost-
effective compared to animals and cadavers. To create this
option, IMRA works in collaboration with the top
universities and faculties. Being an agile company, it can
easily adjust to the surgeons' needs and design products that
meet their unique training requirements.
Surgeons today want more opportunities to physically
practice their surgeries and a level of flexibility to choose

Current robotic surgical training is generally provided by
robot manufacturers. With new entrants to the market and
more uptake in the overall technology, surgeons must attend
multiple training sessions at various companies to support
their own hospital's equipment. Surgeons and staff at
hospitals are time-poor, so allowing complete training in
robotic surgery that is agnostic to the device supports these
goals.
IMRA meets the proficiency goals using data metrics. It
enables surgeons to seek training in the hospital operating
theatre (not in a remote training facility). The concern of
practicing on patients is also eliminated through IMRA's
linear pathway, which includes anatomically correct
synthetic human tissue models that deliver a complete
training solution.

I've had a long career in the medical device industry. Starting as a rep
for J&J many years ago and working at some of the major medical
device companies globally, I found my passion for Robotic Surgery at
Intuitive Surgical in the USA. I wanted to continue working in the field,
addressing the lack of innovation around alternative options for training
surgeons without conducting pigs and cadavers.
The medical industry has come so far with technology, yet our training
options are still very limited. But IMRA will continue to work alongside
surgeons, medical device companies, surgical societies and
governments—to create a truly integrated and product-agnostic
approach to training—that accelerates the uptake of robotic surgery
across all specialties and ultimately creates better patient outcomes.

The company has signed a 5-year agreement with the Royal
Australasian College of Surgeons to provide educational
content regarding robotic-assisted surgery with basic,

Kindly brief us about the company and the ideas that
led to its inception.
I started my company gynecologist.pk two years back. The
basic idea for making this company is to transform
traditional open surgery into advanced techniques through
laparoscopic and robotic surgery. Since Pakistan is a
developing country, advanced surgery techniques are not
available here for the general public. I am fortunate enough
to get opportunities of learning advanced techniques during
my studies, and I was able to continue that journey of
learning advanced techniques. The idea behind this was to
treat the disease processes that could not be possible with
conventional techniques due to lack of expertise.

What is the key operations run by the company to serve
the changing needs of modern surgeons?
We are running complete solutions for gynecological issues
through
• Laparoscopic surgeries
• Robotic surgeries
• Hysteroscopic surgeries
This includes gynecological cancer surgeries, grade 4
endometriosis, myomectomies, congenital uterine problems
solutions, neovaginal formation, septum resection, duel
channel tubal cannulation and many others routine
gynecological surgeries.
Could you please shed some light on yourself and your
journey so far?
Before starting this journey, I did my medical graduation
from Allama Iqbal Medical College Lahore and Post
Graduation from the College of Physicians and Surgeons
Pakistan. I completed my Laparoscopic fellowship from
Endoscopic Training Centre Antwerp Belgium. I completed
my robotic surgery fellowship from The University of
Sharjah Clinical Centre UAE with CMR surgical team in
the Versius Robotic system. I am the first gynecological
robotic surgeon in Pakistan.
How is the company’s unique approach encouraging
innovative ideas among modern surgeons when
approaching healthcare?
We encourage surgeons to transform surgery through
modern techniques for better cure diseases. We are an
ambassador for the promotion of minimally invasive
surgery in Pakistan. With minimally invasive techniques,
surgeons can deal with gynecological issues with better
control and outcomes.
How do you think, will the emerging technological
solutions in surgery ease the process for modern
surgeons?
Robotic surgery provides better control to the surgeon of his
activities. With the help of a robot, surgeons can have better
depth of invasion in tissues that are impossible with hands
or naked eyes. Much more increased precision and accuracy
in surgery due to magnifying imaging and highly accurate
control over the surgical equipment leads to minimal blood
loss, less risk of injuries to surrounding organs, avoidance
of fatigue to the surgeon during long surgeries, freedom of
movement for the surgeon during surgery and many other
associated benefits. The surgeon can reanalyze the surgery
after the completion of surgery leading to improvement. It
helps to avoid legal complications as well.
What are the current challenges faced by the company.
How are you transforming them into opportunities for
growth in the market?
Since we are based in a developing country, the biggest
challenge is surgery cost. Equipment cost is very high, and
since minimal invasive surgery is not part of the national
health service, so the cost of the procedure is very high.
Since only high-income groups can avail our service, these
techniques are getting very popular in this segment of
society. The second challenge is that conventional surgeons
are always spreading negative words about modern
techniques, and because of this educated segment of society
is reading about these techniques more and more online.
As an experienced leader, what advice would you like to
give to robotic enthusiasts aiming to transform the
healthcare industry?
Any person who wants to run a robotic program in an
organization, he/she needs to develop a dedicated team of
health professionals. Along with health professionals, you
need to develop are very accurate financial plan for the
robotic program if its not government-funded.
Secondly, new surgeons should have patience while
learning robotic and minimally invasive surgery. The
learning curve of robotic surgery is very long.
Thirdly, develop a continuous awareness plan for robotic
surgery through which you can give awareness about its
benefits for patients and health professionals.
What are the future goals of the organization? How do
you envision upscaling its operations and reach in 2023
and beyond?
Our future plan is to give awareness to everyone about the
benefits of robotic surgery. We will try to break the false
myths about modern techniques so that more and more
people can avail benefits of modern surgery.

dequate protein intake among elderly is a growing
Aconcern in healthcare. At the beginning of this
century scientific studies signaled malnutrition in
affluent countries up to 25% with hospital patients. Disease
related protein deficiency due to acute or chronic diseases
or decreased appetite and frailty were identified as the main
causes for malnutrition.
General dietary advice focused on adding high protein
snacks and oral nutritional supplements to the daily menu.
The disappointing result was that still less than 50% of
hospital patients would not meet the recommended protein
intake.
A consortium, lead by Wageningen University and
Research, started in 2012 to develop a new nutritional
strategy and products to solve malnutrition. Scientific
studies revealed that the standardized care focused both on
supplementation of energy and protein. As satiation was the
barrier for higher protein intake the current practice of both
energy and protein supplementation did not work.
To solve this problem, new products were developed,
focusing on protein enrichment instead of energy density.
These products were familiar basic foods, like bread and
pastry, fruit juice and soups, and ice-cream and small meals.
Al these products were tested in the foodhospital Gelderse
Valley, in residential care and among patients at home. RCT
studies concluded that 79% of patients reached the
recommended protein intake.
This study shows that the new nutritional strategy works:
substitution instead of supplementation. Substitute regular
basic nutrition with protein enriched basic nutrition.
The essence of the substitution strategy is the range of
familiar products people like to eat, replaced by the same
products containing 2 to 4 times the amount of protein,
tasting equally as good as their reference products.
The range of protein enriched products fits in all eating
moments of the day. Spreading the intake in 3 main meals
and 3 in-between moments results in an optimal protein
intake per moment and the highest intake per day. This was
all confirmed by RCT studies.
In 2015 Carezzo Nutrition, a start-up partner of the
consortium introduced this range of products in the Dutch
market. The success of the strategy was confirmed by
winning the Accenture Innovation Award for Fair Food, and
scientific proof of the effectiveness of the new nutritional
strategy of substitution was presented.
We first entered the Dutch hospital market, which
welcomed the good taste of these familiar products and the
high protein density versus regular protein-rich food. It also
adds to the quality of life as these products fit in regular
eating habits and do not look like medicine.

Since hospital length of stay gets shorter, 'better in is better
out' gets more important. Improving patients' nutritional
condition before hospitalization and after discharge, called
prehabilitation and rehabilitation, is seen as a direct
contribution to efficacy of medical treatment. Less
complications, shorter length of stay and successful
recovery potentially leads to increased benefits at decreased
costs. A RCT study on pre-operative nutritional support on
senior patients scheduled for heart surgery was conducted
by Nina Zipfel PhD in collaboration with Carezzo.
Participants received Carezzo products at home during 2
weeks. Their protein intake increased by 50-100%, due to
the protein enriched products and the high motivation to
self-manage nutritional condition improvement.
The group of people with a protein deficiency is much
larger than discussed so far. Frail elderly people with low
appetite tend to eat less. Since they are less physically
active their portions get smaller and so does the energy
intake. However, their protein intake recommendation is not
related to their activity, but to their body composition.
Knowing that their protein recommendation is 25% higher
than that of young adults, there is a high risk of protein
malnutrition. In practice we see that it is hard to finish a full
hot meal, and that 10-20% is left behind on the plate, or
worse one meal is consumed in two days, shared with a
partner or thrown in the trash. Protein intake then drops
under 20 g, as regular meals (500 g) contain 20-25 g
protein. Carezzo developed small meals of 350 g,
containing 30 g of protein.

There is a challenging goal, also for Carezzo, to improve
the protein quality of plant-based products also for the older
aged groups. We will develop new products combining
fruit, vegetables and hybride proteins.
We now have 80% of all Dutch hospitals as clients, which
is a fast-track record in institutional healthcare. It shows
that the new nutritional strategy is well accepted.
Unfortunately, due to healthcare insurance and
governmental regulations, there is no level playing field
with oral nutritional supplements. There is no
reimbursement on Carezzo products by health care
insurances, so people do not have an equal choice.
The 'Food as Medicine' theme acknowledges the
importance of the functionality of food in the prevention
and treatment of lifestyle related diseases. There is a huge
opportunity to change towards 'the food solution' and
reduce healthcare costs while improving the quality of life.

healthcare:

Protein enriched products fit in maintaining the dietary
habits. Just change products to address the different protein
recommendations due to different circumstances.
To serve patients at home the Carezzo webshop was opened
which facilitates the substitution strategy in the home
setting and patients could continue the use of the products
they received in the hospital during the total period of
medical treatment; pre and post included. The webshop also
serves people with chronic diseases, such as age related
malnutrition, sarcopenia and COPD, which already used
Carezzo products for multiple years.
The combination of global trends 'better for the
environment = better for your health' boosts the shift
towards plant-based products, reducing meat and dairy
consumption. The protein quality for golden-aged people is,
however, a major concern. The advice for them is to not go
Vegan. Protein quality, defined by the amount and
completeness of essential amino-acid profile, from plant-
proteins is lower than the quality of animal-based proteins.

that's able to diagnose lung cancer sooner and with greater

The Midwest Institute for Robotic Surgery at Silver Cross
Hospital was the only facility in Will County to offer
Mako™ robotic-arm assisted surgery for partial knee and
total hip replacement for adults living with early to mid-
stage osteoarthritis. This new technology helps provide
more accurate placement and alignment of a patient's
implant.
And in 2022, we introduced the Ion robotic bronchoscopy

point. We seek engagement from all of our surgeons, listen

How do you think, will the emerging technological
solutions in surgery, ease its process for modern
surgeons?
Any technology that will contribute to improved efficiency
will be impactful as time is an important commodity for a
surgeon. Continued refinement in the DaVinci platform is
also important.
What are the current challenges faced by the company.
How are you transforming them into opportunities for
growth in the market?
Hospitals are still recovering from the effects of COVID-
19, and Silver Cross is no exception. Most notable is the
"early" retirement of tenured, experienced staff and the loss
of talent to agency work. Surgeons who use the DaVinci are
also seeing sicker patients who delayed care; surgeries are
more abundant and complex. Our teams are collectively
working on access.
As an experienced leader, what advice would you like to
give to robotic enthusiasts aiming to transform the
healthcare industry?
Be bold!
What are the future goals of the organization? How do
you envision upscaling its operations and reach in 2023
and beyond?

I completed my robotic surgery fellowship from The University of Sharjah Clinical Centre UAE with CMR surgical team in the Versius Robotic system. With the help of a robot, surgeons can have better depth of invasion in tissues that are impossible with hands or naked eyes. Much more increased precision and accuracy in surgery due to magnifying imaging and highly accurate control over the surgical equipment leads to minimal blood loss, less risk of injuries to surrounding organs, avoidance of fatigue to the surgeon during long surgeries, freedom of movement for the surgeon during surgery and many other associated benefits. The surgeon can reanalyze the surgery after the completion of surgery leading to improvement. It helps to avoid legal complications as well. What are the current challenges faced by the company. How are you transforming them into opportunities for growth in the market? Since we are based in a developing country, the biggest challenge is surgery cost. Equipment cost is very high, and since minimal invasive surgery is not part of the national health service, so the cost of the procedure is very high. Since only high-income groups can avail our service, these techniques are getting very popular in this segment of society. The second challenge is that conventional surgeons are always spreading negative words about modern techniques, and because of this educated segment of society is reading about these techniques more and more online. As an experienced leader, what advice would you like to give to robotic enthusiasts aiming to transform the healthcare industry? Any person who wants to run a robotic program in an organization, he/she needs to develop a dedicated team of health professionals. Along with health professionals, you need to develop are very accurate financial plan for the robotic program if its not government-funded. Secondly, new surgeons should have patience while learning robotic and minimally invasive surgery. The learning curve of robotic surgery is very long. Thirdly, develop a continuous awareness plan for robotic surgery through which you can give awareness about its benefits for patients and health professionals. General dietary advice focused on adding high protein snacks and oral nutritional supplements to the daily menu. The disappointing result was that still less than 50% of hospital patients would not meet the recommended protein intake. A consortium, lead by Wageningen University and Research, started in 2012 to develop a new nutritional strategy and products to solve malnutrition. Scientific studies revealed that the standardized care focused both on supplementation of energy and protein. As satiation was the barrier for higher protein intake the current practice of both energy and protein supplementation did not work. To solve this problem, new products were developed, focusing on protein enrichment instead of energy density. These products were familiar basic foods, like bread and pastry, fruit juice and soups, and ice-cream and small meals. Al these products were tested in the foodhospital Gelderse Valley, in residential care and among patients at home. RCT studies concluded that 79% of patients reached the recommended protein intake. This study shows that the new nutritional strategy works: substitution instead of supplementation. Substitute regular basic nutrition with protein enriched basic nutrition. The essence of the substitution strategy is the range of familiar products people like to eat, replaced by the same products containing 2 to 4 times the amount of protein, tasting equally as good as their reference products. The range of protein enriched products fits in all eating moments of the day. Spreading the intake in 3 main meals and 3 in-between moments results in an optimal protein intake per moment and the highest intake per day. This was all confirmed by RCT studies. In 2015 Carezzo Nutrition, a start-up partner of the consortium introduced this range of products in the Dutch market. The success of the strategy was confirmed by winning the Accenture Innovation Award for Fair Food, and scientific proof of the effectiveness of the new nutritional strategy of substitution was presented. We first entered the Dutch hospital market, which welcomed the good taste of these familiar products and the high protein density versus regular protein-rich food.   • 23. 21 Since hospital length of stay gets shorter, 'better in is better out' gets more important. Improving patients' nutritional condition before hospitalization and after discharge, called prehabilitation and rehabilitation, is seen as a direct contribution to efficacy of medical treatment. Less complications, shorter length of stay and successful recovery potentially leads to increased benefits at decreased costs. A RCT study on pre-operative nutritional support on senior patients scheduled for heart surgery was conducted by Nina Zipfel PhD in collaboration with Carezzo. Participants received Carezzo products at home during 2 weeks. Their protein intake increased by 50-100%, due to the protein enriched products and the high motivation to self-manage nutritional condition improvement. The group of people with a protein deficiency is much larger than discussed so far. Frail elderly people with low appetite tend to eat less. Since they are less physically active their portions get smaller and so does the energy intake. However, their protein intake recommendation is not related to their activity, but to their body composition. Knowing that their protein recommendation is 25% higher than that of young adults, there is a high risk of protein malnutrition. In practice we see that it is hard to finish a full hot meal, and that 10-20% is left behind on the plate, or worse one meal is consumed in two days, shared with a partner or thrown in the trash. Protein intake then drops under 20 g, as regular meals (500 g) contain 20-25 g protein. Carezzo developed small meals of 350 g, containing 30 g of protein.   • 24. 22 There is a challenging goal, also for Carezzo, to improve the protein quality of plant-based products also for the older aged groups. We will develop new products combining fruit, vegetables and hybride proteins. We now have 80% of all Dutch hospitals as clients, which is a fast-track record in institutional healthcare. It shows that the new nutritional strategy is well accepted. Unfortunately, due to healthcare insurance and governmental regulations, there is no level playing field with oral nutritional supplements. There is no reimbursement on Carezzo products by health care insurances, so people do not have an equal choice. The 'Food as Medicine' theme acknowledges the importance of the functionality of food in the prevention and treatment of lifestyle related diseases. There is a huge opportunity to change towards 'the food solution' and reduce healthcare costs while improving the quality of life. As a society we should embrace innovation to improve healthcare: Ÿ The technological innovation for better food products Ÿ The social innovation for faster implementation Ÿ The governance innovation to reduce barriers in finance and regulation We are committed to transfer academic insights into good tasting functional food products, that is our practice. We are deeply touched by the gratitude of patients that finally reach their desired protein intake, because that is our purpose. Protein enriched products fit in maintaining the dietary habits. Just change products to address the different protein recommendations due to different circumstances. To serve patients at home the Carezzo webshop was opened which facilitates the substitution strategy in the home setting and patients could continue the use of the products they received in the hospital during the total period of medical treatment; pre and post included. The webshop also serves people with chronic diseases, such as age related malnutrition, sarcopenia and COPD, which already used Carezzo products for multiple years. The combination of global trends 'better for the environment = better for your health' boosts the shift towards plant-based products, reducing meat and dairy consumption. The protein quality for golden-aged people is, however, a major concern. The advice for them is to not go Vegan. Protein quality, defined by the amount and completeness of essential amino-acid profile, from plant- proteins is lower than the quality of animal-based proteins.
