Doğum yapmış kadınların %40'ında dereceden bağımsız pelvik organ prolapsusu (POP) görülmektedir. It describes a study of 21 patients treated with NPWT who had postoperative abdominal wound breakdown. NPWT resulted in decreased wound size and bowel edema in all patients. Definitive fascial closure was achieved in most patients. Complications occurred in 3 patients, including 2 enterocutaneous fistulas. The authors conclude that NPWT is an effective treatment for abdominal wound dehiscence and can be used for cases with exposed bowel.

2) Indwelling catheters are used when clean intermittent catheterization is not possible, for reasons like neurological conditions, incontinence, surgery, or intensive care.
3) Long term catheter use can lead to complications like infection, bladder and kidney damage, stones, and trauma due to bacterial biofilm buildup. Future research aims to develop catheters that better mimic natural physiology and prevent these issues.

Giriş
Günümüzde Tubularized Incised Urethrplasty
(TIPU) ameliyatı hipospadias onarımında tüm
dünyada oldukça yaygın olarak uygulanmaktadır.
Bu ameliyatın orjinalinde fistül oluşumunu
önlemeye yönelik prepüsyal Dartos flebi kullanımı
tarif edilmiştir. Yıllar içerisinde gerek üretroplasti,
gerekse fleb uygulamasına ait modifikasyonlar
bildirilmesine rağmen fistül gelişimi halen en
önemli komplikasyon olarak durmaktadır.
Hastalar ve Yöntem
Çalışmaya 2012-2016 tarihleri arasında
kliniğimizde TIPU tekniğiyle aynı cerrah tarafından
opere edilen distal tip hipospadias olguları dahil
edildi. Çalışmaya dahil edilen hiçbir hastada ek
penil anomali bulunmamaktaydı. Cerrahın tercihi
doğrultusunda hastalara üretroplastiyi takiben
Dartos flebi veya Spongioplasti uygulandı. Hastalar
postop dönemde serviste takip edildiler.
Tartışma
Literatürde TIPU prosedürü sonrası fistül gelişim riskini
azaltmaya yönelik ayrı ayrı Dartos fleb1 ve spongioplasti
uygulamalarının2 sadece TIPU prosedürü uygulanmasına göre
daha etkili olduğunu belirten çalışmalar mevcuttur. Ancak
prospektif bir çalışmada da3 Dartos fleb uygulanması ile
uygulanmaması arasında fistül gelişimi açısından istatistiksel
olarak anlamlı bir fark bulunamamıştır. Fakat literatürde
spongioplasti ve Dartos fleb uygulamalarını fistül gelişimi
açısından karşılaştıran bir yayın mevcut değildir. Bu çalışmanın
sonucunda spongioplasti ve Dartos fleb uygulamalrının
biribirine üstünlüğü olamadığı görülmektedir. Sonuç olarak
çalışmamızdaki hasta sayısının az olmasına rağmen, distal tip
hipospadias onarımında spongioplastinin postoperatif fistül
gelişimini engelleme üzerine Dartos fleb uygulamasına bir
alternatif olabileceğini düşünüyoruz.
Bulgular
Ortalama yaşı 42 Ay olan toplam 39 hastanın
19’una Dartos Flebi, 20’sine Spongioplasti
uygulandı. Dartos Flebi uygulanan hastalardan
1’inde fistül gelişirken Spongioplasti uygulanan
grupta herhangi bir komplikasyona rastlanmadı.
Hastalara ait diğer veriler karşılaştırıldığında her iki
grup arasında istatistiksel olarak anlamlı fark
olmadığı görüldü.
