Hackley Community Care Center (HCCC) provides integrated behavioral health services at their primary care center located in Muskegon Heights, Michigan. HCCC uses the 5 levels of behavioral health integration model, currently implementing level 4 where behavioral health providers are on-site and share some systems with primary care providers. They provide services like therapy, psychiatric consultations, and care management for depression. Barriers to further integration include reimbursement issues and lack of coordination with community mental health services.
1 in 7 say they have trouble with health bills in U.S.
The percentage of persons in families having problems paying medical bills in the past 12 months decreased from 19.7% in 2011 to 14.2% in 2018 according to a National Health Interview Survey. In 2018, females, children aged 0-17, and non-Hispanic black persons were more likely than other groups to have problems paying medical bills. Among those under age 65, the uninsured had the highest rates of problems paying bills, followed by those with Medicaid or private insurance. For those over 65, those with Medicare and Medicaid or Medicare only had higher rates than those with Medicare Advantage or private coverage.

HCCC uses the 5 levels of behavioral health integration model, currently implementing level 4 where behavioral health providers are on-site and share some systems with primary care providers. The percentage of persons in families having problems paying medical bills in the past 12 months decreased from 19.7% in 2011 to 14.2% in 2018 according to a National Health Interview Survey. In 2018, females, children aged 0-17, and non-Hispanic black persons were more likely than other groups to have problems paying medical bills. Among those under age 65, the uninsured had the highest rates of problems paying bills, followed by those with Medicaid or private insurance. For those over 65, those with Medicare and Medicaid or Medicare only had higher rates than those with Medicare Advantage or private coverage.

Mental Health, we hear about it all the time. Mental health is a driving force in the media, when law enforcement officers in America, engage in a critical incident, involving a subject with “Mental health problems”. Mental health continues to take countless lives, from our Military Veterans, as each day passes. Children in school experienced an extreme disrupt in their daily lives, when they were forced to stay indoors, and attend school through a screen, in the early stages of the Covid-19 Pandemic. Homelessness in America is rising, and the amount of homeless Americans with untreated mental health issues is staggering. We don’t need to see a fact sheet, read a medical journal, or test subjects in a clinical trial, to be cognizant of the amount of persons with unattended mental health disorders, in America. American children’s health should be a priority, they are our future professor’s, philosophers, engineers, doctors, free thinkers; they are the future “Us”.
The Mental Health Services for Students Act of 2021, was introduced by Congresswoman Grace Napolitano. HR 721 passed in the house on May 12th, 2021. HR 721 has 86 cosponsors (82(D) & 4(R)). Since 2001, Congresswoman Napolitano has had this program implemented into 35 schools, which has shown to be extremely helpful (Facts on the Mental Health Services for Students Act, n.d.). With low funds nationally, for on site mental health care professionals in schools, HR 721 would provide additional funding. HR 721 will provide $130,000,000 in competitive grants. The Substance Abuse and Mental Health Services Adminsitration (SAMHSA), would be delegated with distribution of funds. HR 721 would expand on

Personally, I support HR 721, from what I have researched thus far. The problem is, 49.4% of children in the United States did not receive treatment or counseling for a mental health disorder. (Whitley, G., 2019). According to the CDC, the third leading cause of death for adolescents aged 15-19 was suicide (CDC, 2021). Those two statistics alone, show the deprivation of resources and funding in the American school system. My husband, being a police officer, noticed a significant spike in “suicidal juvenile” calls, over the past two years. I myself, work on a occasion, at the juvenile detention facility. I have watched these children, over the past couple of years, destroy their lives due to untreated mental health illnesse.

Objectives. To compare access to care between US-born and foreign-born US adults
by mental health status.
Methods. We analyzed data on nonelderly adults (n = 100 428) from the 2013–2016
National Health Interview Survey. We used prevalence estimates and multivariable lo-
gistic regression models to compare issues of affordability and accessibility between
US-born and foreign-born individuals.
Results. Approximately 22.2% of US-born adults and 18.1% of foreign-born adults
had symptoms of moderate to severe psychological distress. Compared with US-born
adults with no psychological distress, and after adjustment for sociodemographic
characteristics, US-born and foreign-born adults with psychological distress were much
more likely to report multiple emergency room visits and unmet medical care, mental
health care, and prescription medications because of cost.
Conclusions. Our study found that adults with moderate to severe psychological
distress, regardless of their immigration status, were at greater risk for reporting issues
of affordability when accessing health care compared with US-born adults with no
psychological distress.
Public Health Implications. Health care and mental health reforms should focus
on reducing health care costs and establishing innovative efforts to broaden access to

Are mental health problems increasing? The answer does seem to be yes, especially among adolescents and young adults. According to a study from the American Psychological Association, rates of mood disorders and events that relate to suicide have gone up significantly in the past decade within these younger age groups.

Introduction
How well a person is able to live a full life, build and maintain relationships and pursue their education, profession or other pursuits requires them to maintain their well-being ranging from physical to mental health. When assisting others to achieve good and self-motivated changes, social workers draw on their relationship-based abilities and emphasize personalization and rehabilitation. A key issue is, "What components and obstacles of an assessment are there in order to reach these outcomes?" When it comes to health promotion and public involvement, social workers are well-suited for this role since primary care is all about these things. Social workers who deal with mental health have unique challenges in assessment, which necessitates them learning and using psychiatric principles. Identifying the need for mental health care requires an understanding of assessment principles.
For this research, the focus is on urban youth's increase in mental health cases which is more influenced by lack of access to mental health care as a result of poverty that affects many children and families in the US. Young people are the most impacted by poverty, accounting for 33% of the total number of individuals in poverty. Children who live in "high risk neighborhoods" are more likely to lack access to the mental health supports they need to manage their symptoms. Stabilization, individual treatment, and symptom management are all important aspects of aftercare for children who have been hospitalized for behavioral difficulties by the time they are six years old (Hodgkinson, 2017).

This is a brief overview of the evolving field of prophylactic and therapeutic cancer vaccines.
Cancer vaccines are active immunotherapies. As seen in the accompanying figure, the distinction from passive immunotherapies is based on different mechanisms of action. Passive immunotherapies and adoptive T-cell transfer, for example, are made/modified outside of the body.
Once inside the body they can compensate for missing or deficient functions. Active immunotherapies, on the other hand, stimulate effector functions in vivo. What this means, is that the patient’s immune system can respond to the challenge and be stimulated to mediate effector cells that defend the body in an immune response. Examples of active immunotherapies include peptide, dendritic cell, and allogeneic whole-cell vaccines.

Stents are small mesh tubes used to keep arteries, blood vessels, and ureters open. During coronary artery disease, arteries in the heart become narrowed or blocked by plaque, reducing blood flow and oxygen to the heart. Doctors may place a stent in an artery to prevent collapse or improve blood flow. Stents are placed using imaging and a method called percutaneous coronary intervention (PCI) or coronary angioplasty. When deciding between open heart surgery or PCI to treat coronary artery disease, doctors consider the complexity of the blockages and patient preferences, with more complex cases sometimes better suited for surgery and simpler cases often having equivalent outcomes with either approach.

Gastroesophageal reflux disease (GERD) affects at least 20% of Westerners and is characterized by heartburn and acid regurgitation. For cases that do not respond well to medication, surgical procedures may be considered to strengthen the lower esophageal sphincter. Could CDK12 mutations guide immune checkpoint inhibitor use?
Certain mutations in genes like CDK12 may cause prostate cancers to be more amenable to immune checkpoint inhibitor treatment. A subtype of metastatic castration-resistant prostate cancer with CDK12 variants has been identified that has high levels of neoantigens recognized by the immune system as foreign, as well as greater T-cell infiltration. These CDK12-variant prostate cancers may have higher response rates to PD-1 inhibitors than other genomic subtypes of prostate cancer. Pending clinical trial results, CDK12-variant prostate cancer may become the second defined tumor subtype able to be treated with PD-1 inhibitors.

Medline Plus has this to say about bone marrow transplants (here used interchangeably to refer to hematopoietic stem cell transplantation [HSCT]): “Bone marrow is the soft, fatty tissue inside your bones. The bone marrow produces blood cells. Stem cells are immature cells in the bone marrow that give rise to all of your different blood cells.” According to the literature, HSCT has become a standard of care for hematologic malignancies, congenital or acquired disorders of the hematopoietic system, and it is also applied as a therapeutic option in some of the solid tumors.

Triple-Negative Breast Cancer: 2018 Status Update
Up to 20% of all invasive female breast cancer diagnoses are defined by the clinically significant absence of three hormone receptors i.e., ER, PR and HER2. This group of highly heterogeneous tumors exhibit aggressive growth patterns and are known as TNBCs. Although most TNBCs are ductal carcinomas (no special types), the identification of specific histologic/molecular subtypes potentially open up further modes of treatment for a disease that has thus far mainly been treated with cytotoxic chemotherapies. Biologic features in tumor subsets that carry such implications include BRCA pathway inhibition, increased tumor infiltrating lymphocytes (TILs), detection of other biomarkers paving the way for immunotherapies such as elevated PD-L1 expression and AR expression. Here, is some of the relevant information about TNBCs.

HIV Vaccines
Before the discovery of highly active antiretroviral therapy (HAART), acquired immunodeficiency syndrome – the final stage of human immunodeficiency virus (HIV) infection – spelled cancers and death. HIV destroys a type of white blood cell, the CD4+ T cell, which leaves the body vulnerable to other opportunistic infections and complications. Human challenges and triumphs in the age of AIDS are now embedded in our collective memories and captured in 2016 statistics from the World Health Organization. AIDS deaths have declined by 47% since 2005, according to a Los Angeles Times Article. Additionally, more than half of the 36.7 million people across the globe that are living with HIV (in 2016) are also receiving ART.
This points to the transformation of an often-fatal disease into a chronic condition. Today, only 1.1 million people in the USA are estimated to be infected with HIV. However, 1 in 7 Americans do not know their HIV status. Moreover, global treatment gains have been offset by knowledge of geographic “hot spots,” gender gaps in terms of access and care, delays in starting treatment, variable adherence ART, and the development of resistance to drugs. Even if an implicit public health goal of containing clinical sequelae in every HIV-infected individual is accomplished, the burden of exposure to years of ART may take a long-term socioeconomic toll on each patient. Therefore, researchers continue to search for ways to aid the public in the prevention of getting HIV/AIDS in the first place or, failing that, to facilitate a timely cure.
One strategy in a multipronged public health program against the stealth pathogen continues to be the development of preventive/therapeutic vaccines. Currently, there are no approved HIV vaccines, but promising research is underway.

The Vaccine Debate
Vaccines are designed to elicit an immune response against the particular microbe or bits of the microbe from which the vaccine is made. This idea dates back several centuries, when British surgeon, Edward Jenner developed the first vaccine against a lethal infectious disease, small pox. Between the 18th century and now, more than 65 products have been approved which, together with public health and other developments, have contributed to the tapering and, in some cases, eradication of infectious diseases that used to kill millions. The problem is that the design is based on the physical attributes of the microbe. So, one person might be infected with virus x, which mutates rapidly to become 10 or more different strains. So, between approval and reaching the public, effectiveness may drop or wane over time. The sheer logistics of designing a trial means that follow-up periods are not long enough to account for every possible safety issue. Nevertheless, they remain our go-to defense for lethal infections, such as Ebola, and ones that reduce productivity. In other cases, timely inoculations may protect against the risk of developing specific cancers later in life. They have also contributed to the fact that most people are not at home sick with polio or some of the other ancient plagues.
However, anti-infective vaccines are typically given to healthy children and people on the basis that it will not make them sick or that it will reduce the risk of premature death. Because vaccines need to be preserved, properly stored and kept free of other contamination before it reaches many distribution sites, other ingredients are added to the mix. And some people, especially those with weakened immune systems, may have severe/life-threatening allergies to additives or a contaminated batch.
So, one in a million complications/deaths is one in a million too many. To this end, I have compiled a summary culled from various sources, to foster a positive dialogue towards improvements.
Management of Diabetes and Heart Disease
This document discusses the link between diabetes and cardiovascular disease. It provides guidance on screening and managing both conditions, including maintaining optimal blood sugar levels, treating risk factors like high blood pressure and cholesterol, and adopting a healthy lifestyle. Close monitoring of patients can help prevent diabetes-related complications like heart attack, stroke, and peripheral artery disease. While intensive glucose control shows long-term cardiovascular benefits in type 1 diabetes, the evidence in type 2 diabetes is more mixed and depends on individual patient factors.

Management of Prostate Cancer
Basic information for discussion with a healthcare professional is provided here together with some background:
• An enlargement or the presence of tumors in the gland below a man’s bladder that produces fluid for semen ie, the prostate, may suggest benign prostatic hyperplasia (BPH) or prostate cancer
o Almost 8% of new cancer cases worldwide are attributed to this highly curable disease (proportion of patients surviving after 5 years = 98.9%)
o In the USA alone, prostate cancer is the most common non-skin cancer, diagnosed more often in African-American (1 in 5 cases) than white men (1 in 6 cases)
o Prostate cancer is strongly correlated with age, starting at about 50 years old and rising over the ensuing decades
o While debates over under- or over-treatment of prostate cancer continue, it is clear that management of the disease costs the USA an aggregate annual loss in productivity of $3.0 billion
o Moreover, prostate cancer is the third-leading cause of cancer-related deaths in the USA, mainly due to advanced or metastatic disease

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.
O estabelecimento da amamentação e o manejo das principais intercorrências é contemplada.
