The health care system has greatly changed with noticeable reforms and restructuring taking place as a result of technological advancements and innovations. The introduction and implementation of information systems in the health care system has positively impacted on service delivery ("importance of increasing health literacy content in nursing education," 2015). As a result, new roles and responsibilities have emerged in the nursing practice as the integration of information systems continues to be adopted. Information systems have proved to be an effective management tool in the health care system (Mitchell & Hull, 2020). The new roles and responsibilities have challenged nurses to seek more education on the operation of the systems and to remain relevant in the practice. The complexity and cost of different information systems as well as the rate of change in computer technology are various factors that should be considered when evaluating the appropriate information system to implement (Lin, 2017). Nurses and nurse leaders need a sound knowledge of health care information system and understanding of nursing practice to be able to obtain the computer tools relevant to the practice (Sahay, Sundararaman, & Braa, 2017).
Nursing is a critical profession in the health care system and their input cannot be ignored. Nurses interact with patients more compared to any other health care profession (Lin, 2017). Their primary goal of providing health care to patients gives them an upper hand in understanding the patients’ needs and preferences. This makes it necessary to involve nurses in the evaluation and implementation of information systems in the health care. Implementation of informatics in the health care has aided in the effective communication significantly impacting on interprofessional collaboration (Lin, 2017). As a result, better patients’ outcomes have been achieved and reduced medical and human errors. Nurses are almost in every health care system involved in the process of delivering care at all levels. Be it in the hospital, outpatient clinics, rural health centers, schools, nurses are present to provide care to individuals to promote health, prevent and treat illnesses, or help people recover to the best level of functioning they can. In our present time, this care is enabled by technology while enhancing patient safety by translating certain simple and complex functions into automation (Cipriano & Hamer, 2013). Combined with the nurse’s training and education, her experience in the day-to-day processes involved in patient care, nurse informaticists are in the best position to be consulted for policy and process improvements to make patient care delivery safer and more efficient (Cipriano & Hamer, 2013). Nurse informaticists mediate clinical and technology which makes them an essential part of the team in designing systems to improve quality and safety in delivery of care (Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, 2014).
As an example, a few years ago, our hospital has involved nursing, led by a nurse informaticist, in developing a new systematic way of minimizing laboratory errors. This was in response to an increasing rate in laboratory test errors that compromised patient safety. The workflow, half of it significantly involved nursing, specifically in the collection and sending of specimen, has been redesigned by automating from the point of receiving the laboratory order from the physician to bedside collection, and sending the specimen to the laboratory. The automation ensures scanning of the patient’s armband and collection at the bedside. If not for the in-depth knowledge and experience of the nurse informaticist in every detail of bedside patient care, the process improvement could have not been realized and successful.

I will always be proud of how our nursing education is strongly hinged on good documentation and effective, therapeutic communication, giving it the reason why nurses are in such an important position to lead and influence change. My change proposal is being implemented in the ICU. The intervention is implementing communication tools and processes that are evidence based to improve nursing sensitive indicators in the ICU. The internal stakeholders are the ICU staff and the hospital. The external stakeholders are the community that is served by the hospital. Our hospital works off of a relationship-based care (RBC) model. RBC is a culture transformation model and an operational framework that improves safety, quality, patient satisfaction, and staff satisfaction by improving every relationship within an organization (Gallison & Kester, 2018). The core of workforce engagement is the reignighting of joy and meaning for nurses. The joy and satisfaction in having a sense of accomplishment and significance in the work through processes leading to successful outcomes. RBC speaks to how we treat patients, family, and each other. Internal stakeholders all work off this model in this organization. However, covid greatly challenged relationship based care principles by limiting how we interact with each other and our patient families. That in person piece is missing for many patients still. In this organizational transition back to pre-covid practices, meetings, and policies staff are looking for guidance to unify and strengthen the workforce. It is a good segway into external stakeholders. Our nurses and other staff are also members of the community served by the hospital. Therefore, the internal stakeholders all face the real fact that they too receive their care here and have an interest in the quality of care provided. This community funded hospital has been influenced by local donors, architects and artists. Donors play a large role in celebrating the staff and creating this sense of meaning and significance for hospital staff. In a relationship based care model, these gestures serve a huge purpose and allow the hospital to recognize staff in unique ways. The positive factor here is that the nurses have come out of this powerless feeling covid left them with. Small gestures that build trust between nursing and management create a more productive work environment. Due to diversity, heterogeneity and complexity of the existing healthcare structure, providing suitable
healthcare services is a complicated process. This work describes the conceptual design of an e-healthcare
system, which implements integration strategies and suitable technologies that will handle the
interoperability problem among its essential components. The proposed solution combines intelligent agent
technology and case based reasoning for highly distributed applications in healthcare environment.
Intelligent agents play a critical role in providing correct information for diagnostic, treatment, etc. They
work on behalf of human agents taking care of routine tasks, thus increasing speed and reliability of the
information exchanges. CBR is used to generate advices to a certain e-healthcare problems by analyzing

Any change in process is difficult, especially when dealing with a large team or a complex system such as healthcare. When transition into a more advanced digital technology it becomes even more difficult when dealing with the variety of backgrounds and ages the team ranges. Sullivan places it in 5 easier steps to help the transition into a more digital area easier. Those steps include starting with a baseline, conducting a needs assessment, building out the technology and process roadmap, executing and delivering, and ensuring ongoing holistic services and support (Sullivan, 2019). Starting with a baseline is assessing the current system in place and conducting a needs assessment is identifying areas of improvement. Building out the technology and process roadmap is correctly identifying to proper system that best resolves those needs and creating a plan. Executing and delivering is executing the plan that was set with a full team exciting and engaged in the process change. The development of SDLC for a healthcare organization should include a needs assessment that encompasses the needs of the organization’s healthcare workers, including physicians and nurses (McGonigle & Mastrian, 2017). The organization’s needs as a whole need to be represented, so solutions get proposed to meet the needs or address issues (McGonigle & Mastrian, 2017).
The waterfall model is one of the oldest methods used in SDLC and is linear; therefore, the model is sequential, and each stage provides information for the following stage (McGonigle & Mastrian, 2017). The waterfall model has six phases: feasibility, analysis, design, implementation, test, and maintain (McGonigle & Mastrian, 2017). Feasibility helps decide if the project could be initiated and typically addresses the following: technological, economics, legal, operational, and scheduling feasibility (McGonigle & Mastrian, 2017). Operational feasibility specifically helps determine if the project will be effective, meet the set expectations to achieve the goals of the project, or addressing the problem at hand (McGonigle & Mastrian, 2017). The analysis phase helps examine the requirements for the system and business needs (McGonigle & Mastrian, 2017). Analysis is an important step to assess the workflow (McGonigle & Mastrian, 2017). Excluding nurses from these crucial beginning phases could be detrimental to implementing a new health information technology system. Nurses work on the frontlines of healthcare and would be essential to decide if the system is feasible to the operations and workflow. For example, implementing a new EHR could be more cost-conscious; however, the documentation could cause inefficiency in the workflow. The design phase helps the team decide what programs are necessary and discusses how they will interact, how the individual programs work, and what the look or feel will be (McGonigle & Mastrian, 2017). For example, nurses would be great resources to help design and decide specifics to be included in an EHR, as nurses are often documenting several times a shift and on multiple patients. Nursing informatics is a field of nursing that is continuously growing and evolving in the United States. Nursing informatics is the bridge between nursing knowledge and technology. Nurses work around the clock taking care of patients and are at the bedside to identify any patient care issues that may arise. The introduction of new technology into the healthcare system, such as electronic health records (EHRs), brings additional nursing problems, such as the loss of the art of caring (McGonigle & Mastrian, 2018). Nursing informatics is relatively new within the organization that I work. The interactions I have noted between nurses and the informatics staff are not endearing as a feeling of overwhelmingness takes over with the addition of new processes regarding technology.
The art of caring includes therapeutic communication, mindfulness, and active listening. Although technology has positive attributes such as increased patient involvement in care with EHRs, nurses must remember to be present and actively engaged with whomever they interact. Many times a nurse may be consumed with documentation at the computer, and communication lacks between patients and other staff; for example, the clinician documents in the EHR while the patient is speaking of their symptoms, all while the clinician does not make eye contact with the patient. Technology should not replace the day to day interactions between clinicians and patients or other staff. Self-care, such as mindfulness and being present, can improve the relationships between co-workers and patients alike (McGonigle & Mastrian, 2018).
Dr. Watson first termed the Theory of Human Caring in 1979, which included the 10 Caritas Processes. The 10 Caritas Processes include engaging in loving-kindness, being present, refining spiritual practices, and creating a healing environment. The Watson Caring Science Institute has a Caritas Coach Education Program (CCEP) that consists of a six-month mentorship regarding Watson’s caring theory. Patient and staff satisfaction scores increased after nurses applied the 10 Caritas Processes into practice routinely (Norman et al., 2016). With the growing demand in healthcare technology, staff must remember to communicate their needs with one another and not just focus on technology to cultivate a positive work environment and build rapport between staff.
While technology continues to cultivate, nurses must remember patient-centered care and remain engaged with their co-workers. Healthcare technology has many benefits, such as EHRs and increased patient involvement with their care, but technology should never replace the interaction between patients and staff. Nurses must become actively involved in assisting technology companies with what is useful for excellent patient care through topics such as better workflows. By helping technology companies to capture patient care better, nurses will have more quality time at the bedside (Glassman, 2017).

Patient care decisions should be supported by timely clinical information, reflecting the best evidence possible (Institute of Medicine 2013). Present and future professional nurses must be able to use informatics and technology to facilitate critical decision-making for optimal patient outcomes (Massachusetts Department of Higher Education Nursing Initiative 2016). Nursing clinical informatics competencies involve the collection and use of patient data for analysis and dissemination. Nursing informatics with computer science was established to create innovative ways to provide quality care to our patients. Evidence based practice is a cornerstone to making NI better for the future. Without EBP there would be no such thing.
What I see for the future is that technology would be expanding at a faster rate. There would be less hands on and more robotics. What I mean by this is that robots would be administering meds at the bedside, documenting task, and even doing patient consults. I believe that this would be in the near future to come because it would cut the need for nursing staff in half and it would be cost effective for the company. The only thing that would be the downfall to this is that robots do not pose interpersonal skills so patient questions would go unanswered and in would block the communication to building a rapport. We have already advanced to telemedicine which in some cases can be beneficial to the patient being seen by the physician.
Another thing that would probably be good for the up and coming future would be if we could use the pyxis like we use Alexa and Siri. If we could give them voice commands where it could dispense patient medications that are due just by giving the first and last name of the patient would cut down on the time spent pulling meds. Instead of your taking 30mins to 1 hour pulling meds could just take about 15 to 20 mins. Modern technology is advancing so much that some of us cannot not keep up with the new demands of the workplace. In nursing we are always learning new things and we must continue to learn as we advance in our careers in order to be successful. New media offer new possibilities in teaching and learning. However, the appropriation of new knowledge and skills in dealing with technology, especially for older adults, is a critical challenge. This needs to be considered against the background of the digital divide. Which describes, among other things, the lack of information on the ability of older adults to use technology among those who have access to ICTs and new media in healthcare. In conclusion informatics will be changing for the better.

What I mean by this is that robots would be administering meds at the bedside, documenting task, and even doing patient consults. I believe that this would be in the near future to come because it would cut the need for nursing staff in half and it would be cost effective for the company. The only thing that would be the downfall to this is that robots do not pose interpersonal skills so patient questions would go unanswered and in would block the communication to building a rapport. We have already advanced to telemedicine which in some cases can be beneficial to the patient being seen by the physician.
Another thing that would probably be good for the up and coming future would be if we could use the pyxis like we use Alexa and Siri. If we could give them voice commands where it could dispense patient medications that are due just by giving the first and last name of the patient would cut down on the time spent pulling meds. Instead of your taking 30mins to 1 hour pulling meds could just take about 15 to 20 mins. Modern technology is advancing so much that some of us cannot not keep up with the new demands of the workplace. In nursing we are always learning new things and we must continue to learn as we advance in our careers in order to be successful. New media offer new possibilities in teaching and learning. However, the appropriation of new knowledge and skills in dealing with technology, especially for older adults, is a critical challenge. This needs to be considered against the background of the digital divide. Which describes, among other things, the lack of information on the ability of older adults to use technology among those who have access to ICTs and new media in healthcare. Joseph Campbell, an American mythological researcher, wrote a famous book entitled The
Hero with a Thousand Faces. In his lifelong research Campbell discovered many common patterns
running through hero myths and stories from around the world. Years of research lead
Campbell to discover several basic stages that almost every hero-quest goes through (no matter
what culture the myth is a part of). He calls this common structure “the monomyth.”
George Lucas, the creator of Star Wars, claims that Campbell’s monomyth was the inspiration
for his groundbreaking films. Lucas also believes that Star Wars is such a popular saga because it
taps into a timeless story-structure which has existed for thousands of years.
Many followers of Campbell have defined the stages of his monomyth in various ways,
sometimes supplying different names for certain stages. For this reason there are many different
versions of the Hero’s Journey that retain the same basic elements.

Fantastic quests don’t happen in everyday life. Heroes must be removed from their typical
environment. Most heroes show a reluctance to leave their home, their friends, and their life to
journey on a quest. But in the end they accept their destiny.
Usually there is a discovery, some event, or some danger that starts them on the heroic path.
Heroes find a mystic object or discover their world is in danger. In some cases, heroes happen
upon their quest by accident. Campbell puts it like this, “A blunder—the merest chance—reveals
an unsuspected world.”
The new world the hero is forced into is much different than the old one. Campbell describes
this new world as a “fateful region of both treasure and danger…a distant land, a forest, a
kingdom underground, beneath the waves, or above the sky, a secret island, lofty mountaintop,
or profound dream state…a place of strangely fluid and polymorphous beings, unimaginable
torments, superhuman deeds, and impossible delight”. This description may seem pretty vague,
but think of all the various fantasy realms characters have entered throughout the years:
Middle-Earth, Oz, Narnia, Wonderland. It could even be outer space, a
haunted house, or the Matrix. Regardless of the details, the new world is sure to be filled with
adventure.

1. The health care system has greatly changed with noticeable
reforms and restructuring taking place as a result of
technological advancements and innovations. The introduction
and implementation of information systems in the health care
system has positively impacted on service delivery ("importance
of increasing health literacy content in nursing education,"
2015). As a result, new roles and responsibilities have emerged
in the nursing practice as the integration of information systems
continues to be adopted. Information systems have proved to be
an effective management tool in the health care system
(Mitchell & Hull, 2020). The new roles and responsibilities
have challenged nurses to seek more education on the operation
of the systems and to remain relevant in the practice. The
complexity and cost of different information systems as well as
the rate of change in computer technology are various factors
that should be considered when evaluating the appropriate
information system to implement (Lin, 2017). Nurses and nurse
leaders need a sound knowledge of health care information
system and understanding of nursing practice to be able to
obtain the computer tools relevant to the practice (Sahay,
Sundararaman, & Braa, 2017).
Nursing is a critical profession in the health care system and
their input cannot be ignored. Nurses interact with patients
more compared to any other health care profession (Lin, 2017).
Their primary goal of providing health care to patients gives
them an upper hand in understanding the patients’ needs and
preferences. This makes it necessary to involve nurses in the
evaluation and implementation of information systems in the
health care. Implementation of informatics in the health care
has aided in the effective communication significantly
impacting on interprofessional collaboration (Lin, 2017). As a
result, better patients’ outcomes have been achieved and
reduced medical and human errors. In my organization, nurses
have been actively involved in the evaluation and
2. implementation of health care systems. In the process of
evaluating and implementation of these information systems
nurses need to be trained and educated on a number of issues.
When introducing a new information system nurses need to be
trained on the navigation of the various tools. Some of the
information systems are complex and requires a wide scope of
ICT knowledge ("importance of increasing health literacy
content in nursing education," 2015). It is therefore necessary to
equip nurses with the relevant knowledge and skills to
effectively and efficiently use the information systems to offer
quality services. As earlier stated, nurses interact more with
patients compared to other health care professionals. In
organizations that offer telehealth services patients need to be
conversant with the system being used by the organization. To
educate patients on how navigate the system, nurses are best
placed to take them through. It is therefore necessary to equip
nurses with teaching skills that will give them capacity to
educate patients ("importance of increasing health literacy
content in nursing education," 2015). Nurses too need to be
trained on how to operate health care machines that are linked
to the information systems. Innovations in technology have led
to development of health care machines and equipment that do
not necessarily require manual operation but are rather operated
from the computers ("importance of increasing health literacy
content in nursing education," 2015). In order to effectively use
equipment to offer health care services, nurses have to be
trained. Application of informatics can not be avoided in the
modern world as it has become a way of life.

What is and/or should be nursing’s role in decision
making regarding selecting information
systems?
There are so many systems in the healthcare workplace that it
can easily be assumed that all systems work for every person. In
healthcare, these systems “aim to improve the processes of data
handling in order to extract useful information for health
planning, decision-making, and resource allocation through
different sources to provide quality services” (Yazdi-Feyzabadi,
2015). Therefore, since our nurses work closely with these
systems, our opinion should be considered when it comes to
selecting new systems or altering those in place.
In 2010, The Institute of Medicine published a report called,
The Future of Nursing: Leading Change and Advancing Health
. This report offered goals for the future of nursing. Two of the
ten recommendations states that healthcare should
“enable nurses to lead change to advance health and expand
opportunities for nurses to lead collaborative improvement
efforts” (National Academies of Sciences, Engineering and
4. Medicine, 2016). Nurses are our front-line resource to give
feedback to improve the quality of patient care. “In the past, a
common mistake has been selecting healthcare systems that
affect patient care with little or no input from providers across
the healthcare team” (Nelson & Staggers, 2014).
Examples of information systems include telehealth services,
mobile health applications, installing new EHRs technology
(electronic health records). In this case nurses can be vital in
selecting which patients can benefit from telehealth
services, which interventions are best used through telehealth
services and which are more appropriate for in-
person appointments. With mobile applications nurses can give
their opinions on how the application is operating, how it can be
improved and in which instances would be the best situation for
an application implementation. In my workplace, we have a
mobile app that we use for clocking in and out of our shift.
Using the system for almost a year now, I have found many
difficulties in its use. It requires a GPS location to log-in for
the shift and on most occasions it does not recognize my
location even if I am standing at the patient’s front door. It
also only offers the patient address under the documents tab. If
the app included the eMAR and POC we could eliminate the use
of paper each month, transferring our company closer to an all-
digital environment. These are just a few examples of how
nurses can implement change with information
systems. Establishing new systems takes time and care from an
interdisciplinary team of healthcare workers.
